Multimodality neurophysiological monitoring during tibial/fibular osteotomies for preventing peripheral nerve injuries.

Previously, intraoperative neurophysiological monitoring was not used routinely during tibial osteotomies. In an attempt to improve the post-operative outcomes and reduce known neurologic complications, we used discrete multimodality recording and stimulation models to identify areas of motor and sensory function in eighteen (18) pediatric patients presenting with Tibia Vara (Blount's Disease). By using both posterior tibial and peroneal nerves somatosensory evoked potentials (SSEPs) with transcranial electrical motor evoked potentials (TCeMEPs) precise status of sensory and motor pathways was

A neurophysiological dissociation between monitoring one's own and others' actions in psychopathy.

BACKGROUND: Psychopathy is a severe personality disorder often leading to violent and disruptive antisocial behavior. Efficient and proper social behavior crucially relies on monitoring of one's own as well as others' actions, but the link between antisocial behavior in psychopathy and action monitoring in a social context has never been investigated. METHODS: Event-related potentials were used to disentangle monitoring of one's own and others' correct and incorrect actions in psychopathic subjects (n = 18) and matched healthy control subjects (n = 18). The error-related negativity (ERN) was

Neuroprotective role of neurophysiological monitoring during endovascular procedures in the spinal cord.

The endovascular treatment of spinal vascular malformations places the spinal cord at risk for ischemia. When these procedures are performed using general anesthesia, the neurophysiological monitoring methods currently available provide the only means by which to assess the functional integrity of sensory and motor pathways. Neurophysiological monitoring allows a warning for the neuroradiologist of impending irreversible neurological damage so that action may be taken for the prompt restoration of adequate spinal cord perfusion. Muscle motor evoked potentials (mMEPs) better reflect spinal cord

Short-term neurophysiological monitoring in multiple sclerosis bouts. Evaluation of steroid treatment.

Visual (VEP) and brainstem auditory (BAEP) evoked potentials (EP) were recorded in 21 multiple sclerosis (MS) patients in acute relapse before and after steroid treatment. VEPs were abnormal in 14/21 patients and BAEPs in 10/21 patients before treatment. In 4 patients with acute optic neuritis (ON), an improvement of VEPs paralleled clinical evolution in 3 cases. Substantial and contrasting changes in VEPs or BAEPs, with no clinical counterpart, were related to a spontaneous fluctuation of EPs in acute relapses of MS. These changes suggest frequent subclinical (multifocal and, possibly, sequen

Intraoperative neurophysiological monitoring reduces postoperative constipation in unilateral biportal endoscopic lumbar spine surgery.

INTRODUCTION: Postoperative constipation is a frequent complication following lumbar surgery under general anesthesia, potentially associated with intraoperative nerve root irritation. However, the incidence of postoperative constipation following unilateral biportal endoscopic (UBE) lumbar surgery and the potential preventive role of intraoperative neurophysiological monitoring (IONM) remain underexplored. AIM: We aimed to evaluate whether the application of IONM, anal sphincter free-running electromyography (frEMG), and the bulbocavernosus reflex (BCR) reduced the incidence of postoperative

Anesthesia management of neonates and infants requiring intraoperative neurophysiological monitoring: A concise review.

Intraoperative neurophysiological monitoring is currently used to prevent intraoperative spinal cord and nerve injuries during neonatal and infant surgeries. However, its use is associated with some issues in these young children. The developing nervous system of infants and neonates requires higher stimulation voltage than adults to ensure adequate signals, thereby necessitating reduced anesthesia dose to avoid suppressing motor and somatosensory-evoked potentials. Excessive dose reduction, however, increases the risk of unexpected body movement when used without neuromuscular blocking drugs.

Spinal epidural arteriovenous fistula with improved sphincter impairment detected by intraoperative neurophysiological monitoring.

BACKGROUND: A spinal epidural arteriovenous fistula (SEAVF) is a rare type of arteriovenous shunt that occurs mainly in the thoracic or lumbar spine. Patients with SEAVF develop motor/sensory disturbances of the lower extremities and sphincter dysfunction. Among these symptoms, sphincter impairments show less improvement than others, and its relevance to neurophysiological monitoring has not been documented. CASE DESCRIPTION: A 77-year-old woman presented with progressive motor weakness and numbness in the lower extremities and urinary and fecal incontinence. Spinal magnetic resonance imaging

Effects of rocuronium dosage on intraoperative neurophysiological monitoring in patients undergoing spinal surgery.

WHAT IS KNOWN AND OBJECTIVE: Intraoperative neurophysiological monitoring (IONM) has been widely used in clinical practice. Therefore, the influence of neuromuscular blockers essential for spinal anaesthesia on IONM is worthy of our attention, but no randomized study has evaluated the dose-response effect. This study investigated the effects of different doses of rocuronium bromide on the intraoperative monitoring of motor evoked potentials (MEPs). METHODS: We conducted a randomized, double-blind trial to assess the effects of three rocuronium bromide doses (6.0, 9.0, 12 μg·kg-1 ·min-1 ) comb

Progress in Intraoperative Neurophysiological Monitoring for the Surgical Treatment of Thoracic Spinal Stenosis.

Thoracic spinal stenosis (TSS) is a group of clinical syndromes caused by thoracic spinal cord compression, which always results in severe clinical complications. The incidence of TSS is relatively low compared with lumbar spinal stenosis, while the incidence of spinal cord injury during thoracic decompression is relatively high. The reported incidence of neurological deficits after thoracic decompression reached 13.9%. Intraoperative neurophysiological monitoring (IONM) can timely provide information regarding the function status of the spinal cord, and help surgeons with appropriate performa

The Role of Intraoperative Neurophysiologic Monitoring in Asleep AVM Resection: Indication or Redundancy?

PURPOSE: The surgical resection of cerebral arteriovenous malformations (AVMs) presents a significant neurosurgical challenge, particularly because of the need to achieve complete obliteration of the nidus while preserving neurologic function. AVMs located within or adjacent to eloquent regions such as the motor or language cortex carry a high risk of postoperative deficits. To mitigate these risks, a variety of intraoperative tools have been developed to enhance surgical safety and decision making. METHODS: Cortical and subcortical mapping is a dynamic technique that helps identify and prese

Early Neurophysiological Monitoring of Train of Four Assists in the Detection of Pseudocholinesterase Deficiency.

A craniotomy with cortical and subcortical mapping was planned for a 64-year-old male with a large right frontotemporal brain mass. Total intravenous anesthesia was performed, and 200 milligrams of succinylcholine was administered at induction. A train of four prior to head pinning (52 minutes after succinylcholine administration) revealed zero of four twitches in the left hand and foot. The patient did not regain spontaneous breathing despite discontinuation of infusions and the surgeon canceled the case at 108 minutes from induction. The patient was safely extubated at 270 minutes.
