A Nurse-Driven Protocol for Neonatal Enteral Access Device Placement Confirmation.

BACKGROUND: Preterm infants require the use of nasogastric and orogastric enteral access devices (EADs) to provide nutrition and medications. Confirmation of the location of the tip of the EAD is essential to minimize complications. At the study site, EAD location was limited to verifying the centimeter marking at the lip/nares and nonevidence-based methods of visual observation of aspirate and auscultation. PURPOSE: Implement an evidenced-based EAD placement confirmation protocol, and by 90 days post-education and implementation, achieve adherence of 90%. METHODS: This quality improvement p

The Role of Stigma in the Nursing Care of Families Impacted by Neonatal Abstinence Syndrome.

BACKGROUND: The current US opioid crisis has resulted in a significant increase in opioid use disorder among pregnant and parenting women. Substance use disorders, in general, are highly stigmatized conditions. Stigma serves as a well-documented global barrier to health-seeking behaviors and engagement in healthcare. While extensive research exists on the stigma of mental illness, few studies have explored the stigma experienced by families impacted by neonatal abstinence syndrome (NAS). PURPOSE: Therefore, the purpose of this article is to explore the role of stigma in the care of families i

Transient Neonatal Diabetes Mellitus: A Challenge and Opportunity for Specialized Nursing Care.

Transient neonatal diabetes mellitus (TNDM) is a rare disorder, with a reported incidence of approximately 1 in 450,000 live births. It is characterized by insulin-requiring hyperglycemia in the neonatal period. The disease improves by early childhood, but the patient may relapse in later life. Diagnosis is made after genetic testing following presentation with hyperglycemia not conforming to Type 1 or Type 2 diabetes. Management is based on insulin and possible sulfonylurea administration. Three genetically distinct subtypes of TNDM are recognized. Type 1 TNDM is due to overexpression of gene

Japan and its healthcare challenges and potential contribution of neonatal nurse practitioners.

Challenge has the potential to create opportunities for innovation. This applies to new developments aimed at enhancing existing systems of healthcare delivery. The development of the neonatal nurse practitioner's role in the United States is undoubtedly 1 such example of an innovation that was made in response to the challenge in neonatal healthcare delivery. This challenge and concomitant change was generated by a significant increase in neonatal patient needs, survival of smaller and sicker neonates, a rising acuity level, and the accompanying need to elevate the standard of healthcare for

Effects of antimanic mood-stabilizing drugs on fetuses, neonates, and nursing infants.

Pregnancy presents a special problem to the clinician treating bipolar disorders in women. Since the first episode of mania typically occurs before the age of 30, many women in their prime childbearing years may be exposed to potentially teratogenic mood-stabilizing agents. This exposure may also continue for the nursing infant during lactation. Pregnancy itself can exacerbate bipolar symptoms and also alter the pharmacokinetics of mood-stabilizing drugs. Risks to mother and fetus can be reduced with a number of simple strategies, including monotherapy with the lowest effective dose of a drug

Collective strategies to cope with work related stress among nurses in resource constrained settings: An ethnography of neonatal nursing in Kenya.

Kenyan neonatal nurses are asked to do the impossible: to bridge the gap between international standards of nursing and the circumstances they face each day. They work long hours with little supervision in ill-designed wards, staffed by far too few nurses given the pressing need. Despite these conditions, a single neonatal nurse can be tasked with looking after forty sick babies for whom very close care is a necessity. Our 18-month ethnography explores this uniquely stressful environment in order to understand how nurses operate under such pressures and what techniques they use to organise wor

Knowledge, attitude and practices of registered nurses regarding neonatal jaundice at the neonatal intensive care unit in a tertiary hospital in Khomas region, Namibia.

Neonatal jaundice is a significant cause of neonatal morbidity worldwide and accounts for 75% of hospital readmissions in the first week of life. New-born babies can develop severe neonatal jaundice that may cause irreversible brain damage or even death. To assess the knowledge, attitude and practices of registered nurses on neonatal jaundice among neonates at Neonatal Intensive Care Unit at Windhoek Central hospital. A quantitative, descriptive, cross sectional research design was used. The population was all 34 registered nurses working at Windhoek Central Hospital neonatal intensive care un

Neonatal Nurse Practitioner Use of Ultrasonography to Verify Umbilical Venous Catheter Placement in the Neonatal Intensive Care Unit.

BACKGROUND: Literature dating back to 1982 supports the use of ultrasound to confirm UVC position as a more reliable method compared to x-ray. The use of ultrasound to confirm UVC position remains infrequent in the NICU. PURPOSE: To determine the feasibility of training an NNP in the acquisition and interpretation of a parasternal short-axis (PSAX) image of the heart to confirm UVC position. METHODS: A retrospective review of 48 charts evaluated descriptive data using standard technique of an x-ray to confirm UVC placement. An NNP was trained to acquire and interpret a parasternal short-axis

It is important to study the awareness of retinopathy of prematurity (ROP) among neonatal care nurses in hospitals. Unfortunately, there is a lack of studies conducted among nurses on this subject in Palestine. Thus, this study purposed to assess the knowledge, attitudes, and practices toward ROP among neonatal intensive care nurses in Palestine. A cross-sectional was used to conduct this study. A convenience sampling method was utilized to recruit 289 neonate intensive care nurses working in private and governmental hospitals. The findings showed that around 48.0% of the nurses had low knowle

Tiny Infants, Positional Head Deformity, Developmental Positioning and Neonatal Nursing Practice.

BACKGROUND: Low-birth weight infants are at risk for neurodevelopmental complications. Prolonged mechanical ventilation and endotracheal tube stability can compromise developmentally supportive positioning and result in positional head deformity (PHD). Nationally, PHD prevalence is 22%-66%, yet an internal audit of 93 neonatal intensive care unit (NICU) discharge summaries found normal head assessments. PURPOSE: To evaluate PHD prevalence among NICU preterm infants and neurodevelopmental positioning practice. METHODS: A convenience sample of 50 NICU preterm infants >72 hours of life discharg

Development and evaluation of a clinical nursing decision support system for the prevention of neonatal hypoglycaemia.

BACKGROUND: Hypoglycaemia is one of the most common complications during the neonatal period. Recurrent hypoglycaemia episodes can result in neurodevelopmental deficits and even sudden death. Available evidence indicates that healthcare professionals ought to promptly assess the risk of hypoglycaemia in newborns immediately following birth and formulate the most suitable preventive strategies. Consequently, this study was designed to develop a clinical nursing decision support system for neonatal hypoglycaemia prevention based on the prediction model for neonatal hypoglycaemia risk that was de
