A concern regarding the lack of effective communication experienced within Ayrshire in relation to patients with complex wounds initiated the development of a patient-held record. The aim of the patient-held record was to promote patient empowerment by providing a method by which patients and professionals share information. Interested stakeholders worked in partnership to produce a draft record which comprises three main sections: a core section, wound section and personal notes section. Other sections which can be included, where relevant, are nutrition, continence and podiatry. After obtain

AIM AND OBJECTIVES: This study aimed to characterize documentation patterns in hemodialysis nursing records using integrated text mining and nursing surveillance analysis, focusing on surveillance priorities and documentation imbalances.
BACKGROUND: Hemodialysis patients require continuous nursing surveillance due to frequent intradialytic complications; however, systematic analyses of nursing documentation in this setting remain limited.
DESIGN: A mixed-methods study integrating quantitative text mining and structured analysis of nursing surveillance activities.
METHODS: Free-text nursing

OBJECTIVE: To explore the reasonable nursing interventions of advanced schistosomiasis patients.
METHODS: The medical records of 52 advanced schistosomiasis patients hospitalized from 2008 to 2013 were collected, and the nursing interventions were summarized.
RESULTS: The 52 cases of advanced schistosomiasis included 38 men and 14 women, with a mean age of 65.8 years (57-75 years). Totally 53.8% of the subjects were schistosome positive by IHA test, 67.3% positive by ELISA, and 21.2% positive of HBsAg. There were 13 cases of ascites type, 34 cases of megalosplenia type, and 5 cases of dwarfi

Background/Objectives: Artificial intelligence (AI) is increasingly being applied across healthcare, but empirical evidence on its use in home-care and community nursing remains fragmented across technologies, clinical contexts, nursing roles, and stages of implementation. This scoping review aimed to map current AI applications in home-care and community nursing and synthesize reported implementation experiences, benefits, challenges, and future considerations. Methods: Guided by Joanna Briggs Institute methodology and reported according to PRISMA-ScR, eight databases and grey literature via

BACKGROUND: Inconsistencies in pediatric nursing documentation compromise patient safety and limit the profession's ability to demonstrate its clinical contributions. Standardized nursing language (SNL), encompassing NANDA International (NANDA-I) nursing diagnoses, Nursing Interventions Classification (NIC), and Nursing Outcomes Classification (NOC), provides a structured framework to improve documentation quality. However, inadequate nurse education in SNL remains a significant barrier to its effective implementation, particularly in pediatric care settings.
PURPOSE: This study examined the

Spinal cord injury is devastating to the victim, as well as being costly in terms of medical expenses, lost wages, and lost independence. The initial damage to the spinal cord results from several mechanisms of injury--flexion, extension, compression, penetration, rotation, and the disease process. When the spinal cord is injured and there is necrosis of the nervous tissue, no regeneration of that tissue occurs. Unlike in the peripheral nervous system, where regeneration is possible, the spinal cord is part of the central nervous system, as is the brain. The spinal cord extends from the base o

This study began following an observation that fluid balance (FB) charts seemed to be overloading medical records in Hong Kong hospitals. The major issues for this study were the efficiency and appropriateness of FB charting. The study design included an analysis of 250 medical records and an opinion survey. Seventy-five doctors and 98 nurses participated in the survey. The results indicated that 50% of all medical records had FB charts and in these cases they made up between 12 and 16% of the whole medical record. Intravenous infusion and urinary catheterization accounted for almost 70% of th

Good nutrition is essential throughout life and often patients have a poor understanding of what a balanced diet comprises. Nurses need to have a good comprehension of malnutrition. Where malnutrition is evident it can be resolved by working with the patient to encourage a healthy diet. One way to improve patient nutrition is by a nutrition audit of patient food. Numerous nutritional assessment tools exist which help to identify a patient's nutritional status. No nationally standardized nutritional assessment tool exists and there is a danger of assessment methods becoming as proliferate as pr

The high rate of infant mortality, defined as the number of deaths in the first year of life for every 1000 babies born alive (WHO, 1981), is recognized as an extremely serious problem in the developing world. Worldwide decreases in the number of infant deaths are occurring; however, the gap continues to increase in countries with high socioeconomic baselines and those with low per capita income. This article discusses ways in which relevant health care policies and the use of newborn assessment tools can decrease infant mortality in Malawi Central Africa. The description of the tool, Newborn

PurposeTo determine the accuracy of nursing diagnoses at hospital admission and discharge for patients with heart failure (HF).MethodsThis comparative study examined the documentation in 155 medical records of patients with an admitting diagnosis of HF during August 2018 and July 2019. An audit tool was used to record the diagnoses made by nurses during routine care at the time of admission and discharge. Two researchers (L.S. and A.C.) examined the records and evaluated the documented nursing diagnoses using the Nursing Diagnosis Accuracy Scale version 2. Kappa was used for agreement between

AIM: To critically synthesize how hospital digital transformation reconfigures nursing work and data burden, and to examine the distinct contributions of nurse-leader capability and organizational governance capacity.
BACKGROUND: Digital systems can support documentation, surveillance, coordination, and clinical decision-making. The same systems can add redundant entry, fragmented attention, excessive notifications, cognitive workload, and data-quality responsibilities. Technology effects, nursing burden, and implementation leadership are often discussed separately, limiting their usefulness

Seizures are a frequent neurologic complication of cancer. Seizures may develop as a consequence of direct extension of cancer into the brain or surrounding meninges, an underlying metabolic imbalance from treatment or the tumor, or the neurotoxic effects of cancer therapy. Emergency treatment is essential for status epilepticus and seizures that occur frequently to prevent permanent neurologic sequelae. In all cases, a diagnostic evaluation is required to discover the underlying cause of the seizures. Often, an easily reversible process will be uncovered, and the complete resolution of the se

SUMMARY: This 1-year cohort study of nursing home residents revealed that historical fractures of upper limbs or nonhip lower limbs were associated with hip fracture (hazard ratio = 2.14), independent of activities of daily living (ADL), mobility, dementia, weight, and type of nursing home. Prior nonhip fractures are useful for predicting of hip fracture in institutional settings.
INTRODUCTION: The aim of this study was to evaluate the utility of fracture history for the prediction of hip fracture in nursing home residents.
METHODS: This was a cohort study with a 1-year follow-up. Subjects w

This study aims to analyze unstructured nursing documentation of myocardial infarction patients using clinical practice guidelines and SNOMED CT. A total of 72,234 records from 491 patients were annotated using 17 domains derived from the CCCN and AHA/ACC CPGs and mapped to SNOMED CT. The most frequent concepts were "Taking patient vital signs (procedure)" (n = 4,432), and the most frequent domain was observation of patient condition (n = 31,181, 43%).

AIMS: One half of Norwegians die in nursing homes, where death certificates (DCs) are completed by two types of physicians: in-house physicians or physicians on call. The aims of this study were to examine differences in the quality of DCs due to type of physician and to uncover possible implications of errors for the public statistics.
METHODS: DCs from the year 2013 from nursing homes in the catchment area of Akershus University Hospital were examined with regard to logical deficiencies, garbage code diagnoses and type of certifying physician. In one third of cases, the registered causes of
