Segmentation of the cervical spine in tandem with three cranial bones, hard palate, basion, and opisthion using X-ray images is crucial for measuring metrics used to diagnose traumatic atlanto-occipital dislocation (TAOD). Previous studies utilizing automated segmentation methods have been limited to segmenting parts of the cervical spine (C3 ~ C7), due to difficulties in defining the boundaries of C1 and C2 bones. Additionally, there has yet to be a study that includes cranial bone segmentations necessary for determining TAOD diagnosing metrics, which are usually defined by measuring the dist

STUDY DESIGN: Retrospective descriptive study.
OBJECTIVE: The aim of this study was to create topographical maps of occipital bone thickness and venous sinus (VS) presence to assess the risks of screw insertion in four commercially available occipital plates.
SUMMARY OF BACKGROUND DATA: Craniocervical junction instability and deformity are serious pathological conditions that require posterior fixation of the occipital bone to the cervical vertebrae. Insertion of occipital bone screws requires evaluation of both occipital bone thickness for effective internal fixation and intracranial VS pre

PURPOSE: To describe a case of an anomalous posterosuperior course of the V3 segment of the right vertebral artery (VA) that penetrated the occipital bone (wall of the jugular foramen).
METHODS: A 33-year-old healthy woman underwent cranial magnetic resonance (MR) imaging and MR angiography from the upper cervical to the intracranial region using a 3-Tesla scanner to screen for asymptomatic brain lesions, including cerebrovascular diseases.
RESULTS: MR angiography showed no pathological arterial lesions such as aneurysms; however, there was an anomalous posterosuperior course of the V3 segme

BACKGROUND: Atlanto-occipital fusion may be symptomatic or asymptomatic in nature. The anomaly may be incidentally detected at autopsies or during routine cadaveric dissections. The fusion of the atlas with occipital bone may result in the compression of vertebral artery and first cervical nerve.
METHODS: A total of 55 dried occipital bones in the Department of Anatomy, Universiti Kebangsaan Malaysia (UKM) and Department of Anatomy, Universiti Malaya (UM) were included in the study. The presence of atlantooccipital fusion was closely observed and morphometric measurements were taken.

Plagiocephaly has traditionally been corrected by unilateral or bilateral frontal bone advancement or rotation using bone-molding forceps and distraction devices. Complete symmetrical correction of deformed frontal bones is considered almost impossible because the curvature of each frontal bone varies. We evaluated the feasibility of measuring the optimal curvature of frontal and occipital bones using a plaster skull model and applying these measurements to "switch" them for simultaneous correction of frontal and occipital plagiocephaly. A 2-year-old girl suffering from unifrontal flattening v

BACKGROUND: The squamous segment of occipital bone consists of cartilaginous and membranous origin. The cartilaginous part develops to supra-occipital bone. The membranous part has three primary ossification centers on each side. The first pair ossification center lies above the cartilaginous part between the superior nuchal line and the highest nuchal line and fuse with the cartilaginous part to form a supra-occipital segment of occipital bone. The second and third pairs have two nuclei each forming lateral and medial plates. All of these ossification centers fuse to form squamous segments of

BACKGROUND: Condylar screws have broadened fixation options for Craniocervical Fusion techniques and are used when occipital squama anchorage is inadequate. They may also outperform barplate constructs in patients with connective tissue disorders.
METHODS: The condyles are entirely dissected, sacrificing the Posterior Condylar Emissary Vein. An entry 3 mm lateral to the foramen magnum is created. Hand drilling converges 20° medially, aiming toward the basion under fluoroscopy, under continuous hypoglossal EMG monitoring. A 34-mm lag screw is inserted and confirmed by intraoperative CT.

Skull-base metastasis is extremely rare. Various syndromes have been identified based on the anatomical involvement of the metastatic tumor. Occipital condyle syndrome (OCS) occurs with involvement of occipital bone and compression of the hypoglossal canal. OCS is very rare and usually has an underlying widely disseminated metastatic cancer. We present a 66-year-old female who initially presented with tongue deviation and occipital headache. MRI revealed a mass compressing the occipital bone and hypoglossal canal. Further work-up revealed metastatic breast cancer.

Intraparenchymal or intradiploic epidermoid cysts are very rare. Most of these cysts, when present, tend to involve the frontal and temporal lobes, and occasionally, the pineal gland or the brain stem. Here, we report a 45-year-old female, who presented with localized occipital headache and a tender occipital swelling, gradually increasing in size. She was hemodynamically and neurologically stable and did not have any focal neurological deficits. Whole skull and brain imaging revealed a well-demarcated expansile lytic lesion in the right occipital bone, which was hypointense on T1-weighted and

Constructional defects of skull base synchondroses related to a fatal birth injury result only from bottom-upwards pressure of the vertebral column and always combine with a crack or displaced fracture of the sapheno-occipital suture. A postnatal injury having the form of occipital synchondrosis fracture occurs in case of a local impact on the cervico-occipital region and is not associated with a shear strain of lateral synchondrosis and any damage to the sapheno-occipital suture. The spinal birth injury is due to the axial load on the vertebrae in combination with excessive bending and rotati

We report on a 53-year-old woman with osteosarcoma of the skull who underwent radiation therapy for metastatic brain tumor. She had a history of uterine endometrial cancer treated with chemotherapy and surgery 9 years previously. FDG PET/CT for surveillance showed nodular accumulation at the right suprainguinal region and very avid accumulation at the left side of the occipital bone. Bone scan showed increased accumulation at the same portion of the occipital bone. The occipital tumor was surgically removed and diagnosed as radiation-induced osteosarcoma.

Unilateral lambdoid synostosis is the rarest form of craniosynostosis. Surgical correction is technically challenging from a morphological standpoint and is used to decompress intracranial pressure. Herein, the authors present a "rotational advancement bone flap" technique for the repair of posterior plagiocephaly. A bicoronal skin incision was performed, to expose the occipital region. The rotational advancement bone flap was designed to hinge off the nonsynostotic side, and it was set up to include an occipital protrusion on the normal side. On the premature side, an advancement bone flap wa

An anatomic approach is developed to sphenooccipital synchondrosis in newborn infants and nurslings. After removal of the squamous portion of the occipital bone by the method of Medvedev, opening of the cerebrospinal canal, removal of the spinal cord and hypophysis, two parallel cuts converging at an angle of approximately 90 degrees were made with a dissecting blade saw along both sides of sella turcica towards the anterior edge of the great occipital foramen to transect the sphenoid bone and main part of the occipital bone. The resulting osteochondrous fragment showed up defects in synchondr

BACKGROUND: Pediatric occipitothoracic fusion can be challenging because of small size pedicles and thin occipital bone. Three-dimensional (3D) printing technology can help with accurate screw insertion but has not been described for occipital keel plate positioning so far.
OBJECTIVE: To describe the novel use of 3D technology to position occipital keel plates during pediatric occipitothoracic fixation.
METHODS: A young boy with segmental spinal dysgenesis presented with asymmetrical pyramidal paresis in all limbs. Developmental abnormities of the cervical spine caused a thinned spinal cord,

BACKGROUND: We present unique case of a giant extracranial atretic occipital lipoencephalocele in an adult patient with new bone formation within it which was not associated with any developmental malformation of brain. Resection of the lipoencephalocele was performed for esthetic reasons.
CASE REPORT: 18 year old female patient presented to the surgery OPD with complains of a large mass in the occipital region present since birth. It was of size of a betel nut at the time of birth and gradually increased in size over a long period of time. It was painless and not associated with any other co

OBJECTIVE: The sonographic diagnosis of fetal myelomeningocele (MMC) has improved mainly because the diagnostic focus has shifted from observation of the spinal abnormality to observation of cranial abnormalities. We describe an abnormality in the position of the occipital horn in fetuses with MMC. The occipital horn appears to be too posterior in location when compared to healthy fetuses.
METHODS: We searched for all cases in which fetal MMC was sonographically detected from 1999 to 2009. Random controls from normal pregnancies were identified. We then measured the shortest distance of the e

The new conception of mechanism and sequence of damages typical for labor trauma is presented: injury of ligaments of atlas and axis and anterior atlanto-occipital membrane, ruptures of synchondrosises of cranial bones because of vertebral column pressing on occipital bone; tears of dura mater of brain upon basilar impression in the area of focal spot; subarachnoid hemorrhage and intracerebral bleeding in case of cerebral compression by bone borders where they overly override; ruptures of ligaments of cervico-occipital and intervertebral joints in case of excess flexion and overextension of he
