Congenital defects of C1 arches and odontoid process in a child with Down's syndrome: A case presentation.

We present the case of a 2-year-old child with Down's syndrome who presented to our unit with torticollis. Imaging studies revealed the rare occurrence of anterior and posterior C1 arch defects, absent odontoid process, and atlantoaxial subluxation. We managed her conservatively for 3 years without neurological deficits or worsening of atlantoaxial subluxation. We discuss the rare occurrences of anterior and posterior arch defects of the atlas, the radiological presentations of axis defects in patients, and the occurrence of atlantoaxial instability in patients with Down's syndrome. Management

A new appraisal of abnormalities of the odontoid process associated with atlanto-axial subluxation and neurological disability.

It is well known that abnormalities of the odontoid process may be associated with subluxation and neuraxial compression. Modern computerized imaging is demonstrating that existing classifications are in need of revision since they no longer reflect current views on development, nor do they adequately explain the patterns of abnormality commonly encountered in clinical practice. This study is a description of 62 varied clinical cases examined by high definition computerized myelography or MRI in flexion and extension, some before and after stabilization procedures. From these descriptions, and

Transoral odontoid process release, partial resection and antedisplacement for basilar invagination: a novel technique and preliminary results.

OBJECTIVE: Symptomatic basilar invagination (BI) is a complex craniovertebral junction malformation often accompanied by atlantoaxial dislocation (AAD). While posterior reduction and fixation is widely used, anterior odontoidectomy remains essential for decompression. However, conventional odontoidectomy carries a high risk of cerebrospinal fluid (CSF) leakage. We present a novel technique-Transoral Odontoid Process Release, Partial Resection, and Antedisplacement (TOPRA)-designed to achieve effective decompression while reducing CSF leakage risk. METHODS: Between April 2020 and September 202

Healing process of Type II odontoid fractures after C1-C2 posterior screw fixation: Predictive factors for pseudoarthrosis.

BACKGROUND: The healing process after C1-C2 posterior screw fixation (C1-C2 PSF) for odontoid fractures is not well understood. Here, we evaluated such processes and identified factors potentially contributing to pseudoarthroses following fusions for Type II odontoid fractures. METHODS: Pre- and post-operative cervical radiographs and computed tomography (CT) images from 15 patients with preoperative Type II odontoid fractures who underwent C1-C2 PSF were retrospectively reviewed. RESULTS: CT images identified three areas of bone fusion: The primary fracture site in the dens (9/15 patients,

Comparison of Anesthetic Management and Outcomes in Patients Having Either Transnasal or Transoral Endoscopic Odontoid Process Surgery.

BACKGROUND: Endoscopic neurosurgical procedures involving the upper cervical vertebrae are challenging due to a narrow operating field and close proximity to vital anatomical structures. Historically, transoropharyngeal (transoral) endoscopy has been the preferred approach. More recently, however, an endoscopic transnasal approach was developed as an alternative method in hopes to reduce postoperative dysphagia, a common complication following transoral neurosurgery. METHODS: Twenty-two endoscopic neurosurgical cases involving the odontoid or C1 vertebra were reviewed between January 1, 2005

Fracture of the odontoid process in a male individual from the medieval necropolis of Maro (Málaga, Spain).

Traumatic injuries are commonly reported in paleopathology. However, fractures of the odontoid process in past populations are uncommon and therefore such injuries may be easily overlooked. This paper describes alterations of the first and second cervical vertebrae, the atlas and axis, of a male individual of advanced age from the medieval necropolis of Maro (Málaga, Spain). These alterations were observed through macroscopic evaluation and radiological analysis. This individual's skeleton is well-preserved, with degenerative changes present in the appendicular skeleton and the rest of the ver

Endoscopic biopsy sampling of tophaceous gout of the odontoid process. Case report and review of the literature.

The authors present the case of a 71-year-old man who presented with neck pain, a history of gout, and a mass in the dens. Results of transoral endoscopic biopsy sampling demonstrated tophaceous gout. The patient was treated medically and the pain resolved. Tophaceous gout isolated in the dens is extremely rare and should be considered in the differential diagnosis of masses in this region. With the aid of transoral or transnasal endoscopic biopsy sampling, the diagnosis can be reached in a minimally invasive manner.

Solitary bone cyst in the odontoid process and body of the axis. A case report and review of literature.

A four-year-old boy presented with a solitary bone cyst in the odontoid process and body of the axis. Plain radiographs showed a radiolucent lesion with extreme thinning of the cortex and MRI demonstrated a high signal intensity in the interlesional matrix. The cystic component extended into the body of the axis through a defect in the epiphyseal plate. At operation, the cavity of the cyst was found to contain serosanguineous fluid, and histological examination showed that it was lined by a thin layer of connective tissue. The cyst may have originated from a defect in the epiphyseal plate.

Pathologic Fracture of Odontoid Process Due to Metastasis of Bronchial Carcinoma: A Rare Cause of Neck Pain.

The common cause of neck pain in the elderly is degenerative disease. Metastasis to the atlantoaxial spine is rare and is a sequela of prostate cancer, thyroid carcinoma, breast cancer, nasopharyngeal cancer, rhabdomyosarcoma, renal cell carcinoma, or bronchial carcinoma. We present the case of a healthy 66-year-old female patient with severe neck pain with loss of range of motion (ROM) as an index sign of pathologic fracture caused by odontoid process metastasis due to bronchial carcinoma. After staging C2 vertebroblasty was planned, but not performed because the general condition of the pati

Bergmann's ossicle (ossiculum terminale persistens): a brief review and differentiation from other findings of the odontoid process.

PURPOSE: The purpose of this paper was to review the literature on Bergmann's ossicle and provide an overview on its development, etiology, and clinical presentation while also differentiating it from similar structural anomalies. METHODS: A thorough review of the literature available on Bergmann's ossicle was performed. RESULTS: Bergmann's ossicle, also referred to as ossiculum terminale persistens, was defined as a developmental anomaly of the odontoid process in which an ossification center that gives rise to the tip of the dens fails to fuse properly with the body of the axis. CONCLUSIO

Odontoid process metastasis of bronchial carcinoma as a rare cause for nonmechanical neck pain: a case report.

INTRODUCTION: About 10% of spinal metastases are found in the cervical level. Magnetic resonance imaging is the gold standard for early detection of spinal metastases. The decision whether cervical spine magnetic resonance imaging is requested or not must be made clinically, taking into consideration the patients' detailed pain history. CASE PRESENTATION: The case of an 85-year-old patient with a long history of neck pain caused by known multi-level cervical spine degeneration is presented. As conservative treatment became ineffective, he was sent for surgery. Pain character had changed from

Acute neck pain caused by atlanto-axial instability secondary to pathologic fracture involving odontoid process and C2 vertebral body: treatment with radiofrequency thermoablation, cement augmentation and odontoid screw fixation.

INTRODUCTION: Cervical spine metastases are relatively rare entities. Only about 10 % of all spinal metastases can be found in this localization. Magnetic resonance imaging and computed tomography are routinely used for early detection. The initial, clinical examination and patients' complaints may not always be very prominent. Treatment of such lesions is very challenging and needs to consider patient's comorbidities, quality of life and life expectation. Surgery for these lesions should always be performed in specialized spine units. CASE PRESENTATION: We present here a clinical history of

Odontoid process type II and III fracture fixation using bone allograft screws versus cannulated screws: a biomechanical study.

INTRODUCTION: Fractures of the odontoid process are associated with high non-union rates, challenging treatment, and high incidence of screw-related complications. The aim of this study was to compare the biomechanical competence of a single biointegrative bone allograft screw versus two conventional cannulated screws for odontoid fracture fixation. MATERIALS AND METHODS: The odontoid process of intact C2 vertebral specimens was subjected to quasi-static loading until fracture. Specimens with an Anderson and d'Alonzo type II or III fracture (n = 47) were fixated with either two conventional c

Benign osteoblastoma of the odontoid process of the axis with secondary aneurysmal bone cyst component: a case report.

The authors report their experience on one patient with osteoblatoma of the odontoid process of the axis with secondary aneurysmal bone cyst. According to their knowledge, this is the first case, reported in the literature, of this kind of lesion in that particular anatomical region. Because of the rarity of this lesion, it was difficult to have a certain preoperative diagnosis. Therefore, the patient underwent a biopsy via a transoral route. The biopsy was performed over the noncalcified component of the lesion. The intraoperative histological examination showed the benign nature of the lesio

The expanded endonasal approach: a fully endoscopic transnasal approach and resection of the odontoid process: technical case report.

The transoral approach to the odontoid process is considered the "gold standard" for resection of extradural lesions at this location. A completely transnasal endoscopic approach is feasible based on anatomic studies and our experience with the expanded endonasal approach for neoplasms of the cranial base. An illustrative case is presented to demonstrate the technical details of a fully transnasal completely endoscopic approach for the resection of the odontoid process. A 73-year-old woman with a long-standing history of rheumatoid arthritis presented with progressive cervicomedullary compress

Duplication of the odontoid process with other congenital defects of the craniocervical Junction: case report and review of the literature.

Duplication of the odontoid process remains a rare developmental pathology that is underrepresented in the current literature. As the pivot point for the craniovertebral junction, the odontoid process is vital for the integrity of the atlanto-axial joint and the ability of the head and cervical spine to rotate correctly. The pathogenesis being incompletely understood, it has been proposed that odontoid process duplication involves faulty sclerotome migration and disruption of the axis ossification center. Patients presenting with this pathology usually have associated structural abnormalities.
