This paper describes the development, fabrication and testing of Playcuff, a wearable device designed to act as a videogame controller for children with motor disabilities, which also provides an orthotic action to improve the control of the upper limb. The aim of this device is to empower children with motor impairment and enable them to access and enjoy gaming despite their disabilities. The videogame controller function was achieved through on-board gesture classification using a two-tiered Fine Tree machine learning algorithm integrated into the device's firmware. Based on features extract

Straps are the most commonly used elements in the wearable robotic field, where it is employed as a physical human robot interface (pHRI) between the wearable robot and wearer's body. The main role of the strap is to transmit power from the robot to the wearer by pressing and attaching onto specific body parts to reduce the relative motion. To maximize the efficiency and performance using the appropriate function, a high stiffness and tension is required in the strap. However, the continuous force applied by the strap on the wearer's body is one of the main factors that reduces the wearing com

BACKGROUND: Minor lower extremity amputations (LEA) compromise postural control and increase the risk of falls. This issue is exacerbated by conditions such as diabetes, which affect proprioception and sensorimotor mechanisms. While orthopedic devices, including prostheses and orthoses, are frequently prescribed, their specific role in restoring postural control remains underexplored. This scoping review aimed to synthesize current research on postural control in individuals with a minor LEA and to evaluate the interventions that may improve balance.
METHODS: A systematic search was conducted

Patients with weakness or abnormal posture of their lower leg may benefit greatly from appropriate orthoses. This paper describes the sorts of problems that can be helped in neurological practice and the range of devices commonly used, and also highlights some of the factors influencing selection. With greater understanding of their use, clinicians will feel more confident about referring patients for early orthotic assessment.

Orthotic devices usually prescribed for ambulatory treatment of Legg-Perthes disease do not meet the severe kinematic requirements necessary for a comfortable gait. This paper describes the IMA II brace which offers several advantages in comparison with conventional braces. It is shown that improved kinematic characteristics are obtained by the use of six joints placed symmetrically at two levels of the brace.

The goals of bracing in polio and postpolio are to optimize joint position and support weak muscles with the goal of reducing falls, reducing deformities, and optimizing energy conservation. Orthoses are primarily used in the lower extremities to optimize gait. Less frequently, upper extremity orthoses are required. Polio survivors are at increased risk of falls and injuries. Appropriate bracing and compliance with the prescribed device can prevent falls and injuries. The best orthotic results are often achieved with patients who have significant deficits but walk regularly, are well motivated

Managing those with adult flatfoot deformities can be quite challenging, and the methods and devices used are wide-ranging based on the experience of the managing physician and the experience of the provider of the orthotic devices. A thorough biomechanical assessment is paramount to provide the most successful treatment due to the wide range of pathologic abnormalities and pathomechanics that lead to this painful disorder. Taking away pain while improving function for any patient is a rewarding aspect of clinical foot care, and the information covered in this article should arm the practition

BACKGROUND: Orthotic braces play a key role in the correction of spinal deformities. The effectiveness of these devices depends on the design and distribution of corrective forces transmitted through the corset shell.
OBJECTIVE: The present study aimed to reduce the weight of the orthosis and improve its functionality while maintaining its corrective function.
METHODS: The distribution of corrective forces transmitted by the orthosis was evaluated using the finite element method (FEM). Areas of the orthosis, which had minimal impact on the overall stiffness, were identified and material from

BACKGROUND: People with pes cavus frequently suffer foot pain, which can lead to significant disability. Despite anecdotal reports, rigorous scientific investigation of this condition and how best to manage it is lacking.
OBJECTIVES: To assess the effects of interventions for the prevention and treatment of pes cavus.

While the aetiology of adolescent idiopathic scoliosis is unknown, it is widely believed that the deformity can be controlled by mechanical forces applied by a corset and acting, via skin and soft tissues, to exert pressure on vertebral growth-plates (Hueter-Volkmann Law). The reported efficacy of bracing in reducing the incidence of small degrees of progression reinforces this concept. The supposed mode of action conditioned the model of pathomechanism and precludes other hypotheses. Since the critical Cobb angle changes are usually less than the probable measurement error, another criterion

BACKGROUND: Orthotic and prosthetic devices for general-use and activity-specific function can provide critical preventive health benefits for individuals with limb loss, limb difference, and mobility impairments, and yet coverage remains inconsistent across U.S. states.
OBJECTIVE: To evaluate the fiscal impact of clarifying insurance coverage for orthotic and prosthetic devices across 23 states lacking comprehensive coverage.
METHODS: A cost consequence analysis was conducted using data from the U.S. Census Bureau, Kaiser Family Foundation, Government Accountability Office, and a recent act

BACKGROUND: Flaccid paralysis is the result of many different causes presenting with a multitude of symptoms ranging from affecting only the efferent pathways up to involvement of both the afferent and efferent loops of the peripheral neuoromotor system. The peripheral distribution corresponds to the damage.
OBJECTIVES: The functional limitations of flaccid paralysis are the result of a permanent or progressive muscular weakness and ensuing contractures, deformities and instabilities which require compensatory and adaptive mechanisms from the patient to cope with these impairments.

PURPOSE: To determine the role of night orthosis use after surgical correction of Dupuytren contracture.
METHODS: We searched MEDLINE, EMBASE, CINAHL, AMED, OTSeeker, and CENTRAL for articles published from inception of the databases to August 2015. Assessment was undertaken by 2 independent reviewers (O.A.S. and S.A.). Methodological quality of randomized controlled trials was assessed using the Cochrane risk of bias tool and the Newcastle-Ottawa instrument.
RESULTS: Seven studies met the standard for inclusion in this review. A total of 659 patients across these 7 studies were included in

Human walking. It's so simple that we almost forget that it works. It's only when the locomotion give pain that we take care about our feet, ankle, knee, hip or back. First of all, the podiatrist must perform a biomechanical examination of the foot and lower extremities. Angular measurements are taken of all the lower joints to determine the range of motion and, therefore, the level of any structural or functional deformities. The podiatrist will also do a walking and/or running gait analysis of the patient to determine how their foot and lower extremities functions. Understanding the biomecha

Acute anterior shoulder dislocations, when managed non-operatively, have traditionally been treated by placing the arm in a sling. There is no formal evidence that this treatment is of benefit. Three recently reported studies, one in cadavers and two in patients, suggest that the detachment of the structures in the front of the shoulder is made worse when the shoulder is placed in internal rotation, as when the arm is in a sling. By contrast, the structures are realigned when the arm is placed in external rotation. Shoulder dislocations, if managed non-operatively, should not be treated by pla

Infants are born with flexible flatfeet, and the normal arch develops in the first decade of life. Flexible flatfeet rarely cause disability, and asymptomatic children should not be burdened with orthotics or corrective shoes. Flexible flatfeet with tight heelcords may become symptomatic and can be addressed with a stretching program. Surgical intervention for flexible flatfeet is reserved for patients who have persistent localized symptoms despite conservative care. Rigid or pathologic flatfeet have multiple etiologies and many will require treatment to alleviate symptoms or improve function.
