Is unplanned out-of-hospital birth managed by paramedics 'infrequent', 'normal' and 'uncomplicated'?

BACKGROUND: Unplanned out-of-hospital birth is often perceived as precipitate in nature, 'infrequent', 'normal' and 'uncomplicated'. However, international studies report unplanned out-of-hospital birth is associated with increased rates of maternal and neonatal morbidity and mortality. This research describes intrapartum, immediate postpartum and neonatal care provided by paramedics in Queensland, Australia. The objectives were to (1) determine the number of cases where the paramedic documented birth or imminent birth during the study period (2) to describe the incidence of births prior to or

[Prehospital analgesia performed by paramedics: quality in processes and effects under medical supervision].

BACKGROUND: In the context of the new education program for German paramedics, a discussion has been fueled regarding the emergency care competencies of paramedics. In 1999 a system for training and qualification of paramedics was established in the emergency medical service (EMS) area of Reutlingen (Germany), including an algorithm for analgesia using ketamine and midazolam. Under defined circumstances analgesia may be administered to patients in the absence of a physician. AIM: The objectives of the study were to evaluate the effectiveness of pain reduction measured by a recognized assessme

Descriptive analysis of exposure levels of organophosphate esters among firefighters, paramedics, and office workers.

BACKGROUND: Organophosphate esters (OPEs) are a class of chemicals used as flame retardants and plasticizers in many products and industries. While research shows that OPE exposure is widespread in the general population, studies on the working population are scarce, despite their potential of being at higher risk of exposure due to their handling of OPE containing materials and products across their lifecycle. OBJECTIVES: This study aims to characterize OPE exposure among firefighters, paramedics, and office workers, to describe internal exposure levels, evaluate changes over a 24-h period i

Paramedics' attitudes and self-assessed competencies for acute pain management: A cross-sectional study.

INTRODUCTION: The management of acute pain in prehospital care is inadequate worldwide, despite the prevalence of pain among prehospital patients. Studies have indicated that the attitudes and competencies of paramedics are linked to effective pain management. This study aims to describe the attitudes and self-assessed competencies of paramedics and to identify the relationships between these factors and various background variables. METHODS: The data for this descriptive cross-sectional study were collected through an electronic survey distributed to paramedics via social media. The sample i

Preparing undergraduate student paramedics to consider their mental health during clinical placement in Australia.

INTRODUCTION: This article explores the impact of mental health issues on paramedics in Australia, particularly Post-Traumatic Stress Disorder, caused by their exposure to high levels of stress. The prevalence of Post-Traumatic Stress Disorder is higher among paramedics than any other occupation, and this could be a cause for concern, especially for undergraduate student paramedics. The article examines the need to build resilience among student paramedics to help them handle the trauma they may experience during clinical placement. METHODS: This study conducted a two-step process to review l

Sleep and Mental Health among Paramedics from Australia and Saudi Arabia: A Comparison Study.

Paramedics face many challenges while on duty, one of which is working different types of shifts. Shift work has been linked to a number of health issues such as insomnia, depression, and anxiety. Besides shift work, Saudi paramedics, a group that has not been investigated for sleep or mental health issues previously, may be facing more demands than Australian paramedics due to lower numbers of paramedics in comparison to the general population. The aim of this study was to investigate the prevalence of sleep and mental health disorders among paramedics in Saudi Arabia and Australia. Paramedic

The critical incident inventory: characteristics of incidents which affect emergency medical technicians and paramedics.

BACKGROUND: Emergency medical technicians (EMTs) and paramedics experience critical incidents which evoke distress and impaired functioning but it is unknown which aspects of incidents contribute to their impact. We sought to determine these specific characteristics by developing an inventory of critical incident characteristics and testing their relationship to protracted recovery from acute stress, and subsequent emotional symptoms. METHODS: EMT/paramedics (n = 223) completed a retrospective survey of reactions to an index critical incident, and current depressive, posttraumatic and burnout

Ambulatory blood pressure and heart rate in paramedics: effects of cynical hostility and defensiveness.

Ambulatory blood pressure and heart rate responses were obtained in 33 male paramedics during a 24-hour work shift to examine the effects of episodes of occupational stress on cardiovascular reactivity and subjective reports of stress. The aim of this study was to determine how individual differences in cynical hostility and defensiveness interacted with situational demands to affect cardiovascular responses in a natural setting. Defensiveness was found to interact significantly with cynical hostility in predicting subjects' heart rate responses in different work contexts. Specifically, in a h

Paramedic recognition of posterior circulation stroke: a vignette and focus group study.

INTRODUCTION: The main pre-hospital screening tool used for stroke in Ireland and the United Kingdom is the FAST (face, arms, speech, time) test. However, posterior circulation stroke (PCS) patients may be FAST negative on exam, presenting with sudden dizziness, headaches, visual problems, nausea and vomiting and balance/co-ordination problems. There is a lack of research into paramedic recognition of PCS, and this study sought to evaluate recognition rates among paramedics (Ps) and advanced paramedics (APs) in Ireland. METHODS: A cross-sectional sequential exploratory design was chosen. An a

Adaptive coping strategies in male infertility, paramedical counselling as a way of support.

Aims: In the last decades, the number of infertile males increased worldwide which gained more focus. The extent to which a person or a couple is able to cope adaptively with the problem of infertility depends on the combined effect of several variables. Our aim was to apply counselling among males suffering from infertility problems. During the therapy - besides providing information - we aim to elaborate the effects of the treatment and experiences, to process information, to develop adaptive coping strategies against stress and to indirectly or directly change health behaviours influencing

Paramedic-performed Focused Assessment with Sonography in Trauma (FAST) in the emergency department.

BACKGROUND: Our objective was to evaluate the accuracy of paramedic-performed Focused Assessment with Sonography in Trauma (PFAST) for detection of free fluid in patients admitted to the Emergency Department (ED) following trauma. METHODS: After four hours of didactic and four hours of hands-on training, four paramedics prospectively evaluated trauma patients. Our gold standard was the official radiologist reports of ultrasonography and computerized abdominal tomography (CAT). The sensitivity, specificity, positive and negative likelihood ratios, and diagnostic odds ratio of PFAST were calcul

Prevalence of childhood exanthematic disease antibodies in paramedical students: need of vaccination.

The immunity to common childhood exanthematic diseases such as varicella, rubella, mumps and measles was evaluated in 1024 students of the degree courses of health professions at Padua University Medical School. Subjects were subdivided according to gender and age (25 years old or less, and older than 25 years). Results showed that the prevalence of positive antibodies (IgG) to varicella and rubella (94.1 and 94.5%, respectively) was significantly higher (p<0.001) than mumps (78.6%) and measles (86.3%). In addition, measles showed a significant higher prevalence than mumps (p<0.001). Prevalenc

Paramedics in general practice in Cornwall: what roles and what work do they undertake?

INTRODUCTION: General practice faces increasing pressure, especially in deprived rural areas (Laccobucci, 2019). The Additional Roles Reimbursement Scheme (ARRS) was introduced to employ extra health professionals, including paramedics, to support the delivery of general practice services. In Cornwall, over 60% of general practices employ paramedics, as identified in a preliminary search of general practice websites by the author. However, it was unclear what roles and tasks they undertook and how they were funded. This study's aim, the first to focus on a specific rural area in the UK, was to

An evaluation of National Health Service health checks delivered by paramedics in the community.

BACKGROUND: As community-based healthcare providers, NHS ambulance services may be able to offer easily accessible National Health Service Health Checks (NHSHCs). There is no previous evidence to indicate if this is feasible or if this approach can identify cases of cardiovascular disease (CVD) risk factors. We report a service evaluation of an ambulance service delivered NHSHC in North East England. METHODS: Mixed methods including data from an NHSHC delivered by paramedics and short telephone interviews with participants failing to follow referral advice. RESULTS: There were 462 NHSHCs del

How confident are Queensland paramedics with seizure recognition, differentiation, and management? A pilot study.

BACKGROUND: Paramedics in Australia present as the opportune medical personnel to initiate medical intervention of seizure presentations and perform a fundamental role in the prevention of seizure associated morbidity and mortality. Despite being well equipped to manage seizures, no literature exists regarding their confidence to recognise, differentiate, or manage seizures. METHODS: An online cross-sectional survey was undertaken by 168 paramedics practicing clinically for the Queensland Ambulance Service in Australia. The survey assessed paramedic perceived self-confidence surrounding seizu

The recovery processes among paramedics who encountered violence during work-a narrative interview study.

BACKGROUND: Almost all paramedics encounter workplace violence (WPV) during their careers. The most common form of WPV is verbal, and the perpetrator is usually the patient. It is known that paramedics suffer from post-traumatic stress disorder and other mental health problems, and WPV is one of the reasons behind that. Nevertheless, little is known about the recovery processes paramedics have had after encountering WPV. The research question was: What kind of recovery processes have paramedics had after encountering WPV? METHODS: A qualitative, narrative interview study was done. Data was co
