Improving Pelvic Floor Muscle Training with AI: A Novel Quality Assessment System for Pelvic Floor Dysfunction.

The first line of treatment for urinary incontinence is pelvic floor muscle (PFM) training, aimed at reducing leakage episodes by strengthening these muscles. However, many women struggle with performing correct PFM contractions or have misconceptions about their contractions. To address this issue, we present a novel PFM contraction quality assessment system. This system combines a PFM contraction detector with a maximal PFM contraction performance classifier. The contraction detector first identifies whether or not a PFM contraction was performed. Then, the contraction classifier autonomousl

Pelvic floor muscle strength and the incidence of pelvic floor disorders after vaginal and cesarean delivery.

BACKGROUND: Pelvic floor disorders (including urinary and anal incontinence and pelvic organ prolapse) are associated with childbirth. Injury to the pelvic floor muscles during vaginal childbirth, such as avulsion of the levator ani muscle, is associated with weaker pelvic floor muscle strength. As weak pelvic floor muscle strength may be a modifiable risk factor for the later development of pelvic floor disorders, it is important to understand how pelvic floor muscle strength affects the course of pelvic floor disorders over time. OBJECTIVE: To investigate the association between pelvic floo

Quantification of abdominal and pelvic floor muscle synergies in response to voluntary pelvic floor muscle contractions.

The relative levels of pelvic floor muscle (PFM) activation and pressure generated by maximum voluntary PFM contractions were investigated in healthy continent women. The normal sequence of abdominal and PFM activation was determined. Fifteen women performed single and repeated maximum voluntary PFM contractions in supine, sitting and standing. PFM electromyographic (EMG) signals and associated intra-vaginal pressure data were recorded simultaneously. Surface EMG data were recorded from rectus abdominus (RA), external obliques (EO), internal obliques (IO) and transversus abdominus (TA). Abdomi

[Pelvic floor reconstruction-An interdisciplinary overview].

For patients presenting with pelvic floor defects and pelvic organ prolapse, both conservative measures and surgical pelvic floor reconstruction procedures are available, with the aim of restoring the anatomy and function of the pelvic floor. Conservative therapy (e.g., physiotherapy, pessaries) is frequently employed as a primary approach, whereas surgical interventions are indicated in cases of insufficient effect or severe findings. The choice of treatment is based on a precise diagnostic assessment and classification according to the three compartments of the pelvic floor. Various surgical

Multimodality imaging of pelvic floor anatomy.

The pelvic floor is composed of a network of muscles, ligaments, and fasciae, which provide active and passive support for the pelvic organs. Impairment of these pelvic floor elements can result in a variety of functional abnormalities and single or multicompartment organ prolapse. Knowledge of normal pelvic floor anatomy can aid the radiologist in understanding the complex nature of pelvic floor dysfunction and is important for comprehensive image interpretation. This article provides an overview of normal anatomy of the pelvic floor as seen on magnetic resonance imaging, ultrasound, and fluo

[Pregnancy, delivery and pelvic floor disorders].

Pregnancy and vaginal delivery affect pelvic floor by increased intra-abdominal pressure, direct muscle trauma, nerve injury and connective tissue damage. Even if pelvic floor dysfunction is more common after vaginal delivery compared to cesarean section, the differences are less clear after long-term follow up. This implies that other factors such as age, estrogen levels, genetic factors and overweight have also a role in the pathophysiology of pelvic floor dysfunctions. It is possible to prevent some of the birth injuries by careful obstetric management. Pelvic floor muscle training during p

Pelvic floor trauma following vaginal delivery.

PURPOSE OF REVIEW: Recent years have seen a steady increase in the information available regarding pelvic floor trauma in childbirth. A review of this information is timely in view of the ongoing discussion concerning elective caesarean section. RECENT FINDINGS: In addition to older evidence regarding pudendal nerve injury, it has recently been shown that inferior aspects of the levator ani and fascial pelvic organ supports such as the rectovaginal septum can be disrupted in childbirth. Such trauma is associated with pelvic organ prolapse, bowel dysfunction, and urinary incontinence. Elective

Pelvic floor education after vaginal delivery.

OBJECTIVE: To assess the effect of pelvic floor education after vaginal delivery on pelvic floor characteristics in nulliparous women. METHODS: We examined 107 nulliparas during pregnancy and at 9 weeks and 10 months after vaginal delivery. Methods used included a questionnaire, clinical examination, perineosonography, urethral pressure profiles, and intravaginal and intra-anal pressure recordings during pelvic floor contraction. After the second examination, the women were assigned in alternating manner to either 12 sessions of pelvic floor exercises with biofeedback and electrostimulation (

Diagnostic imaging of pelvic floor dysfunction.

Pelvic floor dysfunction encompasses a variety of fascial and anatomic defects that can include a combination of cystocele, rectocele, uterine prolapse, enterocele and vault prolapse. Accurate diagnosis of the coexisting abnormalities is essential in planning reconstructive procedures so that the risks of recurrence and reoperation can be minimized. At this time, dynamic magnetic resonance imaging is the study of choice to evaluate the female pelvis and delineate the possible components of pelvic floor dysfunction.

Pelvic floor rehabilitation in multiple sclerosis.

Although primarily used for treatment of stress incontinence, pelvic floor rehabilitation has been reported to have some value in the treatment of detrusor instability and urgency. In neurogenic bladder dysfunction due to Multiple Sclerosis, many authors have suggested the possible use of pelvic floor rehabilitation as a treatment modality. Therefore, we designed an open prospective trial to look at the possible role of pelvic floor rehabilitation in voiding dysfunction due to MS, concentrating upon the clinical and neurological parameters in relation to the outcome. Laparoscopic colposuspension and pelvic floor repair.

True progress was achieved when the laparoscope was first used to explore the extraperitoneal space. Up to 25 reports on laparoscopic bladder neck surgery have been published in the past five years, and a few present prospective data. Laparoscopic surgery for the other aspects of pelvic floor reconstruction is still only timidly mentioned and in the case study stage.

[Pelvic floor training for stress incontinence].

Genuine stress incontinence is a problem of considerable magnitude. Surgery is the treatment of choice, but there are conservative nonsurgical methods which facilitate the rehabilitation of the pelvic floor musculature. The aim of this study was to determine if our protocol for pelvic floor training is a realistic alternative for the patient who refuses operation. 34 women, aged 38-72 years (mean 48), entered a 3-month training program. It included 12 1-hour weekly meetings in the outpatient clinic with 2 motivated physiotherapists, and home exercises performed 4 times a day. After the trainin

Pelvic floor evaluation with dynamic fluoroscopy.

OBJECTIVE: To evaluate dynamic fluoroscopy of the pelvic floor for the study of women with pelvic floor disorders. METHODS: In a prospective, observational study in a tertiary care referral center, 30 women with prolapse beyond the introitus underwent comprehensive fluoroscopic imaging of the pelvic floor. RESULTS: Dynamic fluoroscopy of the pelvic floor was technically possible in all patients. Whereas the physical examinations appeared relatively similar in these patients, the fluoroscopic examination revealed distinct differences. Of the 30 women, 25 had a cystocele, 25 had a rectocele, a

Spinal control of pelvic floor muscles.

A prevalent notion in the literature is that the pelvic floor muscles behave as a unitary mass. We examined this proposition experimentally. In spinal cats, we recorded EMG activity from the following pelvic floor muscles: the sphincter ani externus (SAE), the abductor caudae internus (coccygeus), and the levator ani (pubiocaudalis) muscles. The epaxial sacrocaudalis dorsalis lateralis muscle was also exposed and prepared for recording. Electrical stimulation of S2 ventral roots elicited twitch responses of the sphincter ani externus and of the sacrocaudalis dorsalis lateralis muscles. Stimula

[Multiple sclerosis and pelvic floor disorders].

This review explores the impact of multiple sclerosis (MS) on pelvic organ function. Beyond the classical symptoms, MS might lead to voiding dysfunctions, faecal incontinence, and/or constipation, and/or sexual dysfunctions for both men and women. Pelvic organ dysfunction is stigmatising and often considered taboo. Therefore, it is crucial to address these issues through comprehensive medical history taking and examinations. Effective treatments are available for all dysfunctions, and a multidisciplinary approach is recommended to enhance the quality of life for patients.

[Pelvic floor reconstruction and sexual function].

Urogynecological surgeries aim to restore and preserve quality of life and enable social participation with as few impairments as possible. In addition to bladder and bowel function, sexual function is often affected in the context of pelvic floor dysfunction. This article aims to present the impact of various surgical techniques for pelvic floor reconstruction on female sexual function. Both anatomical knowledge and psychosexual aspects are discussed. Reconstructions using native tissue as well as synthetic mesh and different surgical approaches for the treatment of pelvic organ prolapse and
