Image-guided radiotherapy for prostate cancer by CT-linear accelerator combination: prostate movements and dosimetric considerations.

PURPOSE: Multiple studies have indicated that the prostate is not stationary and can move as much as 2 cm. Such prostate movements are problematic for intensity-modulated radiotherapy, with its associated tight margins and dose escalation. Because of these intrinsic daily uncertainties, a relative generous "margin" is necessary to avoid marginal misses. Using the CT-linear accelerator combination in the treatment suite (Primatom, Siemens), we found that the daily intrinsic prostate movements can be easily corrected before each radiotherapy session. Dosimetric calculations were performed to eva

Magnetic Resonance Imaging-Guided Adaptive Radiotherapy for Colorectal Liver Metastases.

Technological advances have enabled well tolerated and effective radiation treatment for small liver metastases. Stereotactic ablative radiation therapy (SABR) refers to ablative dose delivery (>100 Gy BED) in five fractions or fewer. For larger tumors, the safe delivery of SABR can be challenging due to a more limited volume of healthy normal liver parenchyma and the proximity of the tumor to radiosensitive organs such as the stomach, duodenum, and large intestine. In addition to stereotactic treatment delivery, controlling respiratory motion, the use of image guidance, adaptive planning and

Lymphatic mapping for image-guided radiotherapy in patients with locally advanced uterine cervical cancer: a feasibility study.

BACKGROUND: Lymph node metastasis is an important prognostic factor in locally advanced cervical cancer (LACC). No imaging method can successfully detect all (micro)metastases. This may result in (lymph node) recurrence after chemoradiation. We hypothesized that lymphatic mapping could identify nodes at risk and if radiation treatment volumes are adapted based on the lymphatic map, (micro)metastases not shown on imaging could be treated. We investigated the feasibility of lymphatic mapping to image lymph nodes at risk for (micro)metastases in LACC and assessed the radiotherapy dose on the node

Intra-prostatic gold fiducial marker insertion for image-guided radiotherapy (IGRT): five-year experience on 795 patients.

INTRODUCTION: Prostate cancer is the second most commonly diagnosed cancer in males. The use of intra-prostatic fiducial markers (FM) for image-guided radiotherapy (IGRT) has become widespread due to their accuracy, relatively safe use, low cost, and reproducibility. FM provides a tool to monitor prostate position and volume changes. Many studies reported low to moderate rates of complications following FM implantation. In the current study, we present our five years' experience regarding the insertion technique, technical success, and rates of complication and migration of intraprostatic inse

Endoscopically placed fiducial markers for image-guided radiotherapy in preoperative gastric cancer: Technical feasibility and potential benefit.

Background and study aims Fiducial markers have demonstrated clinical value in radiotherapy in several organs, but little is known about markers in the stomach. Here, we assess the technical feasibility of endoscopic placement of markers in gastric cancer patients and their potential benefit for image-guided radiotherapy (IGRT). Patients and methods In this prospective feasibility study, 14 gastric cancer patients underwent endoscopy-guided gold (all patients) and liquid (7 patients) marker placements distributed throughout the stomach. Technical feasibility, procedure duration, and potential

Evaluation of the geometric accuracy of anatomic landmarks as surrogates for intrapulmonary tumors in image-guided radiotherapy.

OBJECTIVES: The purpose of this study was to evaluate the geometric accuracy of thoracic anatomic landmarks as target surrogates of intrapulmonary tumors for manual rigid registration during image-guided radiotherapy (IGRT). METHODS: Kilovolt cone-beam computed tomography (CBCT) images acquired during IGRT for 29 lung cancer patients with 33 tumors, including 16 central and 17 peripheral lesions, were analyzed. We selected the "vertebrae", "carina", and "large bronchi" as the candidate surrogates for central targets, and the "vertebrae", "carina", and "ribs" as the candidate surrogates for pe

Feasibility of intensity-modulated and image-guided radiotherapy for functional organ preservation in locally advanced laryngeal cancer.

PURPOSE: The study aims to assess the feasibility of intensity-modulated and image-guided radiotherapy (IMRT, and IGRT, respectively) for functional preservation in locally advanced laryngeal cancer. A retrospective review of 27 patients undergoing concurrent chemoradiation for locally advanced laryngeal cancers (8 IMRT, 19 IGRT) was undertaken. In addition to regular clinical examinations, all patients had PET imaging at 4 months and 10 months after radiotherapy, then yearly. Loco-regional control, speech quality and feeding-tube dependency were assessed during follow-up visits. RESULTS: At

Personalized assessment of kV cone beam computed tomography doses in image-guided radiotherapy of pediatric cancer patients.

PURPOSE: To develop a quantitative method for the estimation of kV cone beam computed tomography (kVCBCT) doses in pediatric patients undergoing image-guided radiotherapy. METHODS AND MATERIALS: Forty-two children were retrospectively analyzed in subgroups of different scanned regions: one group in the head-and-neck and the other group in the pelvis. Critical structures in planning CT images were delineated on an Eclipse treatment planning system before being converted into CT phantoms for Monte Carlo simulations. A benchmarked EGS4 Monte Carlo code was used to calculate three-dimensional dos

Evaluation of Concomitant Imaging Dose in 4D-CBCT Guided Thoracic Radiotherapy.

IntroductionThis retrospective study evaluated the imaging dose of 4D-CBCT in patients treated with thoracic radiotherapy.MethodsA geometric model of a 4D-CBCT system was built for Monte Carlo (MC) calculations. Percentage depth dose (PDD) and profiles of the system were measured and compared with MC calculations to verify the model. Eight lung cancer patients were retrospectively selected for 4D-CBCT imaging dose assessment. For each patient, the imaging dose was calculated using the CBCT log file, simulating CT and the MC model. Organ at risk (OAR) dose parameters from the imaging dose were

Proposal for magnetic resonance imaging-guided salvage radiotherapy for prostate cancer.

BACKGROUND: A subset of patients experience a biochemical recurrence following radical prostatectomy. Radiotherapy can salvage those patients, provided that all disease is encompassed within the target volume. We hypothesized that this can be achieved more adequately with magnetic resonance imaging (MRI)-guided treatment planning. MATERIAL AND METHODS: From January 2009 to April 2014, 183 patients were referred to our department for salvage radiotherapy (SRT). According to protocol, patients received a planning computed tomography (CT) as well as an MRI in treatment position. All MRI scans we

Developing oxidative stress-responsive neuroprotective nanotheranostic agents for the image-guided treatment of radiotherapy-induced brain injury.

RATIONALE: Radiotherapy-induced brain injury (RIBI) is a chronic side effect, that affects up to ~90% of brain tumor survivors treated with radiotherapy. Chronic oxidative stress and neuroinflammation are key drivers of RIBI. Here, we developed oxidative stress-responsive polymeric nanotheranostic agents and evaluated their ability to reduce neuroinflammation in a preclinical mouse model of RIBI. METHOD: Two oxidative stress-responsive amphiphilic block copolymers possessing varied numbers of phenylboronic acid pinacol ester (BAPE) moieties to scavenge reactive oxygen species (ROS) were desig

Correction of image-guided radiotherapy and intrafraction motion using ExacTrac dynamic for head and neck cancer patients.

Daily cone-beam computed tomography (CBCT) based image-guided radiotherapy (IGRT) with six degrees of freedom (6DOF) is standard for head and neck cancer. We retrospectively analyzed 669 sessions from 20 patients, using CBCT guided 6DOF couch corrections followed by ExacTrac Dynamic adjustments. Surface-guided radiotherapy (SGRT) continuously monitored intrafraction motion, triggering verification images and couch shifts when tolerances were exceeded. Pitch and roll deviations were linked to translational misalignments up to 3 mm. SGRT prompted repositioning in 100 sessions (15%), although nea

Dynamic Cross-Task Representation Adaptation for Clinical Targets Co-Segmentation in CT Image-Guided Post-Prostatectomy Radiotherapy.

Adjuvant and salvage radiotherapy after radical prostatectomy requires precise delineations of prostate bed (PB), i.e., the clinical target volume, and surrounding organs at risk (OARs) to optimize radiotherapy planning. Segmenting PB is particularly challenging even for clinicians, e.g., from the planning computed tomography (CT) images, as it is an invisible/virtual target after the operative removal of the cancerous prostate gland. Very recently, a few deep learning-based methods have been proposed to automatically contour non-contrast PB by leveraging its spatial reliance on adjacent OARs

Risk Factors for Nodal Failure in Patients with FIGO IIIC Cervical Cancer Receiving Definitive Image-Guided Radiotherapy.

BACKGROUND: Nodal failure is a major failure pattern for patients with FIGO IIIC cervical cancer, which is further associated with worse survival. This study was designed to investigate risk factors for nodal failure in FIGO IIIC cervical cancer patients. METHODS: The characteristics of positive lymph nodes (LNs) and relevant clinical factors of 162 FIGO IIIC cervical cancer patients were collected. The chi-square test and logistic regression model were used to identify risk factors for nodal failure. RESULTS: In total, 368 positive LNs were identified, including 307 pelvic LNs and 61 para-a

Multimodal evaluation of hypoxia in brain metastases of lung cancer and interest of hypoxia image-guided radiotherapy.

Lung cancer patients frequently develop brain metastases (BM). Despite aggressive treatment including neurosurgery and external-radiotherapy, overall survival remains poor. There is a pressing need to further characterize factors in the microenvironment of BM that may confer resistance to radiotherapy (RT), such as hypoxia. Here, hypoxia was first evaluated in 28 biopsies from patients with non‑small cell lung cancer (NSCLC) BM, using CA-IX immunostaining. Hypoxia characterization (pimonidazole, CA-IX and HIF-1α) was also performed in different preclinical NSCLC BM models induced either by int

The importance of image guided radiotherapy in small cell lung cancer: Case report and review of literature.

AIM: Describe the anatomical changes and tumor displacement due to a rapid response of a patient's small cell lung cancer (SCLC) during definitive chemoradiotherapy (CRT). BACKGROUND: The treatment for SCLC is based on CRT. If interfractional changes during RT are incorrectly assessed they might compromise adequate coverage of the tumor or increase dose to organs at risk. Image guided RT with cone-beam computed tomography (CBCT) allows to identify daily treatment variations. MATERIAL AND METHODS: Describe a SCLC case with rapid changes in size, shape and location of the primary tumor during

Image quality enhancement of small field-of-view cone-beam CT for image-guided radiotherapy using a structure-preserving cycle-consistent generative adversarial network.

Cone-beam CT (CBCT) is widely used for patient positioning in image-guided radiotherapy (IGRT), but full-fan CBCT, which reduces patient dose and acquisition time, suffers from a smaller field of view (FOV) and degraded image quality. To address this limitation, we developed a generative adversarial network (GAN)-based image translation framework, specifically a Cycle-consistent GAN (CycleGAN), to enhance small-FOV CBCT images. The method incorporates dedicated preprocessing and multiple structure-preserving loss functions to mitigate boundary effects caused by truncation of patient anatomy. T
