Disorders of Gut-Brain Interaction and the Rome V Process.

Disorders of gut-brain interaction (DGBI) are characterized by gastrointestinal symptoms related to any combination of motility disturbance, visceral hypersensitivity, altered mci function, altered gut microbiota, or altered central nervous system processing. Research on and clinical management of DGBI is based on selective diagnostic criteria, generated through the Rome Foundation. The Rome V process generated a broad overview of the knowledge accumulated since Rome IV was published 10 years ago, with a focus on evidence-based adjustments. It builds on (1) an update of the DGBI definitions ba

2023 International Rome consensus for the nomenclature of Sjögren disease.

Nomenclature for the disease widely known as Sjögren syndrome has proven unsatisfactory. Patients have perceived 'syndrome' as indicative of a vague collection of symptoms, prompting the Sjögren's Foundation to abandon the term. Furthermore, the traditional distinction between 'primary' and 'secondary' forms fails to account for the complex interplay between overlapping autoimmune diseases. Following a bibliometric analysis, systematic literature review and a Delphi consensus process with equal involvement of professional and patient representatives, five recommendations are now issued. First,

Ambient air pollution and hypertensive disorders of pregnancy in Rome.

BACKGROUND: Ambient air pollution has been associated with hypertensive disorders of pregnancy (HDP), but few studies rely on assessment of fine-scale variation in air quality, specific subtypes and multi-pollutant exposures. AIM: To study the impact of long-term exposure to individual and mixture of air pollutants on all and specific subtypes of HDP. METHODS: We obtained data from 130,470 liveborn singleton pregnacies in Rome during 2014-2019. Spatiotemporal land-use random-forest models at 1 km spatial resolution assigned to the maternal residential addresses were used to estimate the expo

The Prevalence of Rome IV Nonerosive Esophageal Phenotypes in Children.

OBJECTIVES: To assess the prevalence of Rome IV nonerosive esophageal phenotypes in children using multichannel intraluminal impedance testing and to describe the rates of proton pump inhibitor (PPI) responsiveness and the frequency of microscopic esophagitis in these patients. STUDY DESIGN: We conducted a retrospective review of all children ≥5 years of age who underwent esophagogastroduodenoscopy and multichannel intraluminal impedance testing off PPI therapy for evaluation of typical gastroesophageal reflux symptoms. Only children with symptoms during the multichannel intraluminal impedanc

Rome IV Diagnostic Questionnaires and Tables for Investigators and Clinicians.

The Rome IV Diagnostic Questionnaires were developed to screen for functional gastrointestinal disorders (FGIDs), serve as inclusion criteria in clinical trials, and support epidemiological surveys. Separate questionnaires were developed for adults, children/adolescents, and infants/toddlers. For the adult questionnaire, we first surveyed 1,162 adults without gastrointestinal disorders, and recommended the 90th percentile symptom frequency as the threshold for defining what is abnormal. Diagnostic questions were formulated and verified with clinical experts using a recursive process. The diagn

Substance use in the club scene of Rome: a pilot study.

OBJECTIVE: Over the last few years, a wide number of unregulated substances have been marketed on the Web and in smart and head shops; they are usually advertised as legal alternatives to commonly known drugs and are defined as "smart drugs," "legal highs," and "novel psychoactive substances" (NPS). Aim of our work is to describe use habits and distribution of NPS in a population of young adults in Rome club scene. METHODS: A self-administered questionnaire was proposed to subjects over 18 years of age at the entrance of 5 nightclubs in Rome. Socioeconomic characteristics and substance use we

Functional dyspepsia: not all roads seem to lead to rome.

BACKGROUND: The Rome criteria have been introduced to create order in the heterogeneity of functional dyspepsia. The applicability of these symptom-based classification systems remains controversial. GOAL: To evaluate the successive Rome criteria for functional dyspepsia in a large pool of patients with endoscopically verified functional dyspepsia. STUDY: Patients referred to a secondary care district hospital were asked to fill out a questionnaire on gastrointestinal symptoms 2 weeks before upper gastrointestinal endoscopy. Patients were classified according to the Rome I, II, and III crite

[Functional dyspepsia: the past, the present and the Rome III classification].

The author summarizes the historical development of our knowledge about functional dyspepsia and overviews the so-called "road to Rome" process. Between 1988 and 2006, expert committees developed using the Delphi method subsequent classifications of functional gastrointestinal disorders (Rome I-III). The Rome III classification reassessed the diagnostic criteria for functional dyspepsia and distinguished new subgroups as the postprandial distress and epigastric pain syndrome. The rationale for the proposed new classification was based on the inadequacy of prior approaches such as the predomina

Treat the patients' main dyspepsia complaint, not the ROME criteria.

This pragmatic, primary care study by Meineche-Schmidt in patients with presumed acid-related uninvestigated dyspepsia showed that a standard dose of omeprazole 20 mg daily was significantly more effective than a placebo in treating their self-worded main dyspepsia complaint. As a higher dose of omeprazole 40 mg daily was not more effective, the standard dose should be used. In this study, 9% of the patients had sole reflux-like symptoms while the rest had other accompanying symptoms and would be considered to have dyspepsia. In fact, patients had on average more than five symptoms. Patients d

Geographical clustering of scleroderma in a rural area in the province of Rome.

A geographical cluster of scleroderma and scleroderma-related features was identified in a rural area in the province of Rome. Two patients with scleroderma, three with CREST syndrome and one with eosinophilic fasciitis were living in a village where the total population included 572 persons of voting age. No kindred relationships were demonstrable among these patients. Clinical features of scleroderma such as Raynaud's phenomenon, bilateral hand edema, and digital scars were detected in an additional 10 cases. A group of apparently healthy subjects with scleroderma-related serological abnorma

The epidemiology and quality of life of functional gastrointestinal disorders according to Rome III vs Rome IV criteria: A cross-sectional study in primary care.

OBJECTIVE: To explore the differences in frequency and epidemiology of functional gastrointestinal disorders (FGIDs) in a primary care setting, and to examine the health-related quality of life (HRQOL) and healthcare utilization of FGID patients based on the Rome III and Rome IV criteria. METHODS: A cross-sectional study of consecutive adults in a primary healthcare setting was conducted. Differences in epidemiology, and HRQOL of common FGIDs (functional dyspepsia [FD], irritable bowel syndrome [IBS], functional diarrhea, functional constipation [FC]) between the Rome III and IV criteria were

Managing Dental Pain in Ancient Greece and Rome. Part I.

Herbal plants were an essential component of the healing process in ancient medicine. Although only limited evidence documents the practice of dental extractions, a review of major medical works from ancient Greece and Rome demonstrates that conservative approaches predominated. A survey of the work of Celsus, Scribonius Largus, Dioscorides, Galen, Oribasius, and Aurelianus show that numerous plants-such as Hyoscyamus, mandragora, poppy, gall, pine, and pomegranate-were frequently employed to treat dental pain and gingival inflammation. These same herbal remedies reappeared throughout the medi

Rome V Pediatric Upper Gastrointestinal Disorders of Gut-Brain Interaction.

Upper gastrointestinal disorders of gut-brain interaction (DGBI) present from infancy through adolescence. The Rome V Criteria have expanded to include DGBI of the esophagus, disorders of air-transit, and feeding disorders, as well as rumination syndrome, cyclic vomiting, chronic nausea syndrome, and functional dyspepsia. This expansion provides a diagnostic framework for patients presenting with chest and throat pain, feeding difficulties, belching, pain with eating, nausea, and vomiting. Given the advances in impedance technology and high-resolution manometry, testing plays a greater role in

Rome V: minimizing "functional", maximizing precision in gut-brain disorders.

Rome V represents a major conceptual and clinical evolution in the field of disorders of gut-brain interaction (DGBI). Its most important contribution is the deliberate move away from the term "functional", a label that has often implied that symptoms are less real, less biological, or merely psychological. By emphasizing DGBI, Rome V reframes these conditions as positive diagnoses arising from altered gut-brain regulation, visceral sensitivity, motility, mucosal and immune mechanisms, microbiota, autonomic function, and central processing. Beyond terminology, Rome V introduces greater diagnos
