INTRODUCTION: Endotoxins are proinflammatory components of Gram-negative bacteria. The aim of this randomized clinical trial was to test the null hypotheses that (i) there is no significant difference between continuous chelation irrigation (5.25% sodium hypochlorite + 9% etidronic acid) and sequential chelation (5.25% sodium hypochlorite - 17% ethylene diaminetetra-acetic acid) in endotoxin reduction, and (ii) placement of calcium hydroxide (CH) does not further improve endotoxin reduction in previously treated teeth with symptomatic apical periodontitis.
METHODS: Previously treated teeth wi

Aim: To compare shock wave-enhanced emission photoacoustic streaming (SWEEPS) with sonic- and ultrasonically activated irrigation systems in removing Enterococcus faecalis biofilm from the root canal system. Methodology: Fifty human single-canalled mandibular premolars were included in the study. After access cavity preparation, the root canals were prepared to a standardized size and taper. Then, the entire root surface was covered with two layers of resin, and the root apices were sealed before sterilization. All root canals were inoculated with E. faecalis biofilm, and the samples were incu

The three main components of endodontic success are three-dimensional obturation, pulp space sterilization, and biomechanical preparation. Instrumentation techniques are insufficient to accomplish complete disinfection of the pulp space. It is essential to use additional tools, such as endodontic irrigants. This review article aims to provide a general overview of the different root canal irrigants and their clinical uses. Endodontic treatment focuses more on removing infectious material from the root canal system to facilitate the healing of an existing periapical lesion or to prevent the per

The study involved 300 elderly patients with chronic periapical periodontitis. The patients were randomly assigned into three groups. The treatment for group A used a Mtwo Ni-Ti rotary instrument combined with ultrasonic irrigation of a 2.5% NaOCl solution. The group B used the same instrument combined with ultrasonic irrigation of an active silver ion antibacterial solution. The group C used the same instrument combined with syringe irrigation of a 2.5% NaOCl solution. The root canal fillings were performed immediately after canal preparation. Twenty-four hours after the procedure, patients s

Background and Objectives: Residual filling material may persist on the walls of oval root canals after mechanical retreatment, and the influence of sodium hypochlorite (NaOCl) concentration and supplementary irrigation procedures on its removal remains unclear. This laboratory study used scanning electron microscopy (SEM) to evaluate the effects of three NaOCl concentrations and five final irrigation protocols on residual filling material in retreated oval canals of mandibular premolars. The protocols tested were conventional needle irrigation (CNI), EDDY, passive ultrasonic irrigation (PUI),

BACKGROUND: In certain clinical situations, root canal treatment in teeth with apical periodontitis is performed in multiple visits, with the use of intracanal dressing between visits, aiming to reduce microorganisms and their by-products of the root canal system prior to filling. However, in recent years, discussions have been growing about the real need for the use of intracanal dressing in these cases. The use of ultrasonic activation of the auxiliary chemical substance has increased the potential for decontamination promoted during the chemomechanical preparation of the root canal. Thus, t

INTRODUCTION: The long-term success of root canal treatment depends on effective elimination of microorganisms from the root canal system using chemical irrigants before obturation. Sodium hypochlorite (NaOCl) is the most commonly used irrigant in endodontic therapy; however, it has certain limitations, such as high surface tension that restricts penetration into lateral canals and isthmuses. Therefore, newer irrigants with improved antimicrobial properties are being explored. Recently, nanoparticles have been introduced as potential antimicrobial agents for root canal disinfection.

As the root canal system shows different and complicated anatomies, mechanical instrumentation alone has not the ability to provide a bacteria-free environment in root canals. On the other aspect, necrotic tissue remaining can decrease the effects of root canal irrigants and medicaments and also interfere with the adaptation of root canal fillings to dentin. As a result, certain disinfection and irrigation procedures are required to remove the remaining tissues from the root canal area thoroughly and also be able to eliminate the microorganisms. Triple antibiotic paste (TAP) containing metroni

Modern, biologic root canal therapy should be performed with suitable irrigating solutions and intracanal medicaments. The goal of endodontic treatment is to free the treated tooth from infection and prevent reinfection as thoroughly as possible by means which do not put the organism at risk. In this review of the literature, an evidence-based concept for irrigation and medication of root canal systems is presented. Irrigants and medicaments are discussed with respect to their antimicrobial, tissue-dissolving and endotoxin-decontaminating capacity in relation to their systemic toxicity. Recent

OBJECTIVE: To develop a new method to test the fracture strength of endodontically-treated root dentin.
METHOD: Bovine tooth roots were transversely cut into 2-mm thick sections and the root canals were enlarged with a taper of 0.06. An outer layer of resin composite was bonded to each section to make the root canal-to-outer radius ratio smaller than 1/3. The resulting discs were treated with irrigants at the inner surface and then fractured by inserting through the center a steel rod of the same taper attached to a universal test system. Fracture strength was calculated by using Lame's equat

This study examined the effect of various root canal irrigants and medicaments on dentin fluorescence elicited by 655 nm visible red laser light. To replicate clinical use, irrigants were applied onto dentin samples for 2 min, while medicaments were applied for 2 weeks. Fluorescence values tracked from baseline across the following to 24 h, starting 5 min after exposure. Sodium hypochlorite, hydrogen peroxide, and articaine local anaesthetic (4% articaine with 1:1000,000 adrenaline) all significantly quenched fluorescence (p < 0.0001), which then returned to baseline levels after 20 min. The primary goal of endodontic therapy is the reduction or elimination of microorganisms and their by-products from the root canal system. Although a number of instrumentation and irrigation techniques exist, debris is often left behind in the root canal system and proper canal cleaning, shaping, and irrigation are needed to reduce significantly or sometimes even eliminate microorganisms from the canals. Residual microbes in the root canal system are the primary cause of post-treatment apical periodontitis that may persist in both poorly and properly treated cases. Apical periodontitis is a se

BACKGROUND: The success of endodontic therapy depends on thorough biomechanical preparation and the use of effective intracanal irrigants to achieve a microbe-free canal before obturation. Candida albicans is the most commonly isolated fungus from infected root canals and has a strong dentin affinity. Owing to its resistance to several intracanal medicaments, including calcium hydroxide, alternative antifungal agents are required. Bovine milk, which contains caprylic acid - a medium-chain fatty acid with known antimicrobial properties, may provide a natural and biocompatible adjunct for root c
