Therapeutic approach to malnutrition and sarcopenia.

Sarcopenia is a syndrome characterized by the progressive loss of muscle mass and strength with a risk of undesirable effects such as physical disability, poor quality of life and death, and it is a major contributing factor of disability and loss of independence in the elderly. Its etiopathogenics include different mechanisms that are both intrinsic to the muscle itself and related to changes in the central nervous system, as well as hormonal and lifestyle factors. Several hormones and cytokines affect muscle function and mass. The reduction in testosterone and estrogens associated with agein

Mechanisms of muscle wasting in sarcopenia.

Approximately 66% of the patients with rheumatoid arthritis (RA) have significant loss of cell mass (rheumatoid cachexia), mainly of skeletal muscle (rheumatoid sarcopenia). Sarcopenia is defined as muscle wasting associated with functional impairment. Patients with RA possess significant reduction in muscle strength, caused by muscle protein wasting, and loss of functionality. Various conditions leading to muscle wasting involve different pathways of intracellular signaling that trigger: (i) programmed cell death (apoptosis); (ii) increased protein degradation through autophagy, calcium-depen

Treatment strategies for sarcopenia and frailty.

Sarcopenia is the key feature of frailty in older people and a major determinant of adverse health outcomes such as functional limitations and disability. Resistance training and adequate protein and energy intake are the key strategies for the management of sarcopenia. Management of weight loss and resistance training are the most relevant protective countermeasures to slow down the decline of muscle mass and muscle strength. The quality of amino acids in the diet is an important factor for stimulating protein synthesis. Vitamin D deficiency should be treated, and new pharmacologic approaches

The role of nutrition in the prevention of sarcopenia.

Nutrition is regarded as one important contributing factors in the complex etiology of sarcopenia. Associations between several nutritional factors and muscle mass, strength, function and physical performance were reported in a growing number of studies in recent years. Accordingly, the avoidance of weight loss is crucial to prevent the concomitant loss of muscle mass. Adequate amounts of high-quality protein are important for optimal stimulation of muscle protein synthesis. Vitamin D, antioxidants and ω 3-polyunsaturated fatty acids may also contribute to the preservation of muscle function.

Functional and metabolic consequences of sarcopenia.

The capacity of older men and women to adapt to regularly performed exercise has been demonstrated by many laboratories. Aerobic exercise results in improvements in functional capacity and reduced risk of developing type II diabetes in the elderly. High intensity resistance training (above 60% of the 1 repetition maximum) causes large increases in strength in the elderly, and resistance training significant increases muscle size. Resistance training also significantly increases energy requirements and insulin action of the elderly. We recently demonstrated that resistance training has a positi

Pathogenesis and risk factors of sarcopenia.

Sarcopenia is an age-related degenerative disease characterized by progressive loss of skeletal muscle mass and function, resulting severe clinical outcomes such as falls, disability, and increased all-cause mortality, thereby significantly reducing the quality of life in elderly population. With China's rapid demographic aging, sarcopenia is emerging as a critical public health challenge. In this review, we elaborate the pathogenesis of sarcopenia, identifying metabolic imbalance and cellular oxidative stress as major contributing factors to muscle degeneration. Also, this article indicates t

Nutrition and exercise for sarcopenia treatment.

Sarcopenia, the age-related loss of muscle mass, strength, and performance, is a significant concern in the aging population. Despite extensive research, no consensus exists on its prevention and treatment. Sarcopenia increases the risk of functional disability, falls, hospitalization, long-term care, morbidity, and mortality among older adults. Currently, no approved pharmacological treatments for sarcopenia exist, making exercise and nutrition the most effective interventions. Evidence indicates that targeted exercise reduces risk factors, preventing or treating sarcopenia in older adults. P

Rethinking sarcopenia and frailty of the elderly.

Sarcopenia, or 'frailty of the elderly', is a condition of low muscle mass which is prevalent in older adults and post-menopausal women, leading to falls or worsening after falls. Age-related muscle thinning contributes to falls and fractures. Fractures lead to downward spiral toward the loss of independence in the elderly and ultimately, increased health care expenditures of approximately 4 billion pounds a year. The diagnosis of sarcopenia is based on muscle mass, muscle strength, and physical performance. Sarcopenia is caused by imbalance between the anabolic and catabolic processes on the

Is there a place for ultrasound in diagnosing sarcopenia?

BACKGROUND: Sarcopenia is a progressive and generalised skeletal muscle disorder which presents as loss of muscle mass and function and is associated with increased likelihood of adverse outcomes, reduced quality of life and increased mortality. In developed countries, the prevalence of sarcopenia is rising due to increasing life expectancy. Still, in many clinical settings, sarcopenia may be overlooked and undertreated. While several tools are available for assessment of muscle mass and quality, there remains a need for safe, reliable and accurate diagnostic methods which can be implemented f

Exerkines and myokines in aging sarcopenia.

Aging sarcopenia is an unavoidable condition that affects the majority of older adults in their later years. Exercise has been extensively researched as an effective intervention for sarcopenia. In particular, the release of exerkines and myokines during physical activity has beneficial effects on the body, which, as mediators, offer a novel therapeutic strategy for elucidating how exercise enhances skeletal muscle mass and function. In this review article, we summarize how exerkines exert protective effects on aging skeletal muscle mainly through the following mechanisms: (1) mediating energy

Sarcopenia - the need for an Asian perspective.

Sarcopenia research started in Asia about 25 years ago, and is now a rapidly developing field of research. The Asian Working Group in Sarcopenia was formed in 2014 with the publication of the first Consensus definition, followed and an update in 2019, with a most recent update in 2025. Cut off values used in the definition are different from those in Europe or the US, possibly due to a combination of social determinants of body composition and ethnicity. The importance of detection of sarcopenia in community and hospital settings is recognized; however a gap exists between research, public hea

The link between Mitochondria and Sarcopenia.

Sarcopenia, a widespread condition, is characterized by a variety of factors influencing its development. The causes of sarcopenia differ depending on the age of the individual. It is defined as the combination of decreased muscle mass and impaired muscle function, primarily observed in association with ageing. As people age from 20 to 80 years old, there is an approximate 30% reduction in muscle mass and a 20% decline in cross-sectional area. This decline is attributed to a decrease in the size and number of muscle fibres. The regression of muscle mass and strength increases the risk of fract

Definition and evolution of the concept of sarcopenia.

Sarcopenia and dynapenia are two terms associated with ageing that respectively define the loss of muscle mass and strength. In 2018, the European Working Group on Sarcopenia in Older People (EWGSOP) introduced the EWGSOP2 diagnostic algorithm for sarcopenia, which integrates both concepts. It consists of 4 sequential steps: screening for sarcopenia, examination of muscle strength, assessment of muscle mass and physical performance; depending on these last 3 aspects sarcopenia is categorised as probable, confirmed, and severe respectively. In the absence of validation of the EWGSOP2 algorithm

The Effects of Exercise in Patients with Sarcopenia.

The aim of this study was to assess the effects of different types of exercise interventions for treating sarcopenia compared to no specific treatment, a minimal intervention (e.g., education), or another active treatment (nutritional supplements). A review was conducted of the recent English literature searching PubMed and ScienceDirect databases. Ten randomized controlled trials (RCTs) were included in this review, presenting the results of 671 sarcopenic patients. The exercise interventions were resistance training (four studies), a multimodal program (five studies, encompassing resistance
