Does Your Wolff-Parkinson-White Patient Need to Slow Down?

In brief Two healthy young athletes, a 22-year-old college football player and a 31-year-old elite runner, were evaluated for paroxysmal tachycardia. The football player's resting ECG showed the short PR interval and delta wave characteristic of Wolff-Parkinson-White syndrome. Electrophysiologic studies were required to diagnose accessory-pathway conduction in the runner. The high incidence of tachyarrhythmias seen among people who have this preexcitation syndrome makes detection essential. When such arrhythmias cannot be controlled with drug therapy, catheter ablation helps.

Electroclinical Landscape of Infantile Epileptic Spasms Syndrome.

OBJECTIVES: To elucidate the electroclinical characteristics of infantile epileptic spasms syndrome (IESS) and to determine any potential association among these with underlying etiologies and response to therapy. METHODS: Sixty-eight, treatment-naive children with IESS underwent long-term video electroencephalogram (EEG) recording, which was used to characterize the semiology, ictal, and inter-ictal EEG patterns. Children were further followed up to assess electroclinical predictors of etiologies and short-term therapeutic response. RESULTS: Of 68 children enrolled (69% boys), the median ag

Short QT syndrome: a very rare arrhythmogenic entity.

Short QT syndrome (SQTS) is a gene-related arrhythmogenic syndrome harbouring a large spectrum of symptoms ranging from mild palpitations to sudden cardiac death. The mutation of genes (KCNH2, KCNQ1, and KCNJ2) encoding for cardiac potassium channels plays a central role in SQTS. Electrocardiography is the primary important step in the diagnosis (short QT interval along with T wave changes), but ECG findings may be easily ignored. Treatment of the syndrome is still controversial. Some specific antiarrhythmic drugs and an implantable converter/defibrillator (ICD) have been considered as main the

[Unmotivated malfunction of implanted cardioverter-defibrillator and ways of its correction].

The study included 149 patients with implanted cardioverters-defibrillators (CD) at high risk of development of ventricular tachyarrhythmia. It was found that unmotivated malfunction of CD occurs in supraventricular tachycardia, when a short episode of ventricular tachycardia spontaneously ends before CD starts to work, when T-wave is mistaken for ventricular contraction, in defective isolation, electrode dislocation. A method to reduce frequency of CD malfunction is proposed. To prevent unmotivated discharges it is recommended to use attendant drug therapy, upgrading equipment and algorithms

[Low frequency ultrasound therapy in chronic suppurative otitis media].

A novel low-frequency ultrasound unit "Tonsillor-2" equipped with a device for treatment of middle ear diseases and a set of ultrasonic wave guides was employed in the treatment of 434 patients. A short-term response was achieved in 93% of the patients. Indications to and techniques of treatment of otitis media purulenta chronica (OMPC) are described. The method of low-frequency ultrasound therapy of OMPC can be used in monotherapy, in preoperative preparation, in postoperative management. Low frequency 25-27 kHz ultrasound is shown not to diminish antimicrobial drug activity.

Beat-to-Beat P-Wave Analysis Outperforms Conventional P-Wave Indices in Identifying Patients with a History of Paroxysmal Atrial Fibrillation during Sinus Rhythm.

Early identification of patients at risk for paroxysmal atrial fibrillation (PAF) is essential to attain optimal treatment and a favorable prognosis. We compared the performance of a beat-to-beat (B2B) P-wave analysis with that of standard P-wave indices (SPWIs) in identifying patients prone to PAF. To this end, 12-lead ECG and 10 min vectorcardiogram (VCG) recordings were obtained from 33 consecutive, antiarrhythmic therapy naïve patients, with a short history of low burden PAF, and from 56 age- and sex-matched individuals with no AF history. For both groups, SPWIs were calculated, while the

Short QT syndrome: a case report and review of literature.

The short QT syndrome has been recently recognised as a genetic ion channel dysfunction. This new clinical entity is associated with an incidence of sudden cardiac death, syncope, and atrial fibrillation in otherwise healthy individuals. The distinctive ECG pattern consists of an abnormally short QT interval, a short or even absent ST segment and narrow T waves. A 30-year-old resuscitated woman with short QT syndrome is described together with an example of the classic ECG characteristics. A short-coupled variant of torsade de pointes was reveal on Holter recordings. The implantable cardiovete

[Laparoscopic cholecystectomy--current status].

Laparoscopic cholecystectomy (LCHE) is a safe and effective way to treat gall stones. The procedure was carried out for the first time in France in 1987 and since then it has spread rapidly all over the world, in view of the excellent results achieved. The major benefits are the low morbidity, the very short hospitalization, the early return to daily life and work and its excellent cosmetic results. LCHE is indicated in symptomatic non-complicated gall-bladder disease. The operative technique is standardized, the complication rate is low. Compared with other treatments such as litholysis, extr

Synergistic effects of ultrashort wave and bone marrow stromal cells on nerve regeneration with acellular nerve allografts.

Acellular nerve allografts (ANA) possess bioactivity and neurite promoting factors in nerve tissue engineering. Previously we reported that low dose ultrashort wave (USW) radiation could enhance the rate and quality of peripheral nerve regeneration with ANA repairing sciatic nerve defects. Meanwhile, ANA implanted with bone marrow stromal cells (BMSCs) exhibited a similar result. Thus, it is interesting to know whether it might yield a synergistic effect when USW radiation is combined with BMSCs-laden ANA. Here we investigated the effectiveness of ANA seeded with BMSCs, combined with USW thera

Laser-induced histopathological changes to the retina: A review.

Although the indications for retinal laser have diminished over the past 2 decades due to the rise of intravitreal injection therapy, retinal laser remains a useful clinical tool, with novel laser technologies continuing to emerge. Continuous wave photocoagulation remains the most well-characterized laser in terms of its histopathological effects. Information about the histological effects of newer retinal laser modalities is relatively scarce. Available evidence supports the notion that short pulse applications result in less collateral damage to the neurosensory retina, but that at sufficien

["Asymptomatic WPW" : Is treatment necessary?].

The term "asymptomatic WPW" (Wolff-Parkinson-White) is often used as a synonym for ventricular pre-excitation of the WPW type due to an accessory pathway characterized by a short PR interval and a delta wave on the electrocardiogram (ECG) without the clinical occurrence of paroxysmal tachycardia. Asymptomatic WPW is often diagnosed in young and otherwise healthy people. There is a small associated risk of sudden cardiac death due to rapid antegrade conduction over the accessory pathway during atrial fibrillation. This paper highlights aspects of noninvasive and invasive risk stratification, th

Acute myeloid leukemia maturation lineage influences residual disease and relapse following differentiation therapy.

Acute myeloid leukemia (AML) is a malignancy of immature progenitor cells. AML differentiation therapies trigger leukemia maturation and can induce remission, but relapse is prevalent and its cellular origin is unclear. Here we describe high resolution analysis of differentiation therapy response and relapse in a mouse AML model. Triggering leukemia differentiation in this model invariably produces two phenotypically distinct mature myeloid lineages in vivo. Leukemia-derived neutrophils dominate the initial wave of leukemia differentiation but clear rapidly and do not contribute to residual di

[Clinical characteristics of acute encephalopathies associated with influenza H1N1-2009 in children].

We report 12 cases of acute encephalopathy associated with influenza H1N1-2009 treated according to Japanese guideline (2009). In all 12 cases, electroencephalogram presented diffuse or localized high-amplitude slow waves. Brain CT and MRI showed abnormalities in 4 and 6 cases, respectively. We used hypothermia therapy for 5 patients. One patient showed impairment in short term memory, while the rest of the patients showed no sequelae. These 12 cases presented here suggest the early recognition and therapy according to the newly proposed guideline may reduce severe sequelae and mortality by ac

Two-Stage CD8+ CAR T-Cell Differentiation in Patients with Large B-Cell Lymphoma.

Chimeric antigen receptor (CAR) T-cell therapy has expanded therapeutic options for patients with diffuse large B-cell lymphoma (DLBCL). However, progress in improving clinical outcomes has been limited by an incomplete understanding of CAR T-cell differentiation in patients. To comprehensively investigate CAR T-cell differentiation in vivo, we performed single-cell, multimodal, and longitudinal analyses of CD28-costimulated CAR T cells from infusion product and peripheral blood (day 8-28) of patients with DLBCL who were successfully treated with axicabtagene ciloleucel. Here, we show that CD8
