Ultrasonography (US) is a helpful imaging tool in the evaluation of the musculoskeletal system. It has some advantages over the other imaging techniques, such as plain radiography, computed tomography and magnetic resonance imaging, represented by the non-invasiveness and multiplanar imaging capability, repeatability, lack of radiation burden, good patient acceptance, and relatively limited costs. US offers an excellent resolution and a possibility for real-time dynamic examination of the joints and surrounding soft tissues, as well as enables monitoring of therapeutic response. The most commo

The floating shoulder is an uncommon but important injury pattern. Although it is frequently defined as an ipsilateral fracture of the clavicle and scapular neck, recent studies suggest that ligament disruption associated with a scapular neck fracture contributes to the functional equivalent of this injury pattern, with or without an associated clavicle fracture. Determining the specific injury patterns indicates the potential for significant instability, and correlating these patterns with clinical outcome is a challenge. Because the degree of ligament disruption is difficult to assess on rad

Among consumers, families, therapists, physicians, and other rehabilitation professionals, there has been an increasing interest in shoulder pain associated with spinal cord injury. These disorders primarily affect the soft tissues, including the tendons (eg, rotator cuff tendonitis and bicipital tendinitis), muscles (eg, myalgias and myofascial pain syndromes), and bursae. Disorders of bone and joints also have been of interest (eg, osteoporosis and osteoarthritis of the acromioclavicular and glenohumeral joints). This article addresses the historical context, epidemiology, pathophysiology,di

Magnetic resonance imaging of the shoulder is routinely used, especially in the evaluation of rotator cuff disease and glenohumeral instability. More recent studies have provided a more sophisticated understanding of what represents a pathologic rotator cuff. Similarly, there has been an increased focus on the role of the glenohumeral ligaments and their labral attachment sites in maintaining glenohumeral stability. There have been technical advances as well, including the increased use of magnetic resonance arthrography. In addition, newer studies have shed light on the pathophysiology of the

Shoulder sonography has been shown to be an effective noninvasive means of detecting rotator cuff tears and biceps tendon abnormalities. The examination requires careful attention to technique and a thorough knowledge of normal gross and sonographic anatomy. Rotator cuff tears generally cause a diffuse or focal decrease in the space between the humeral head and the deltoid muscle. Therefore, tears are easiest to detect by noting abnormally close apposition of the deltoid and humeral head.

A retrospective study involving 2027 patients with a limited range of motion of the shoulder in the period from 1970 to 1987 is described. Of these, 1219 patients (60%) were diagnosed as having frozen shoulder, with the highest incidence occurring in patients aged 50-60 years. The mechanism involved in the contracture is complicated. At first, the movement of the greater tuberosity at the subacromial level (also called the subacromial secondary joint) is limited due to bursitis. The external rotation of the upper arm then decreases, and the long rotator muscles contract. The relationship betwe

Pain in the shoulder arises from a wide variety of abnormalities. The most common cause of acute pain in the nontraumatized shoulder is calcific tendonitis or bursitis readily identified by plain film radiography. On the other hand, the evaluation of chronic pain usually requires some combination of arthrography, CT, ultrasonography, and MRI to identify the source of the patients' complaints with certainty. The shoulder is rarely the site of a monoarticular process and is infrequently the dominant site of abnormality in a generalized articular disease. Clues to generalized disease processes ar

The procedure for diagnostic shoulder arthroscopy and proper positioning of operative portals are discussed. The superior medial portal approach to the shoulder is described. Conditions visualized by arthroscopy include the following: glenoid labrum tears, synovitis, loose bodies, instabilities, adhesive capsulitis, rotator cuff lesions, and extra-articular decompressions.

With the aid of ultrasound we tried to visualize the muscles of the shoulder region, the collum scapulae, the axillary artery and vein and the brachial plexus. We were successful with exception of the vessels and the nerves. Knowledge of the anatomical landmarks is essential in ultrasound image reading.

Between December 1979 and June 1982, 74 arthroscopic examinations of the shoulder were done at our institution. Arthroscopy was done in 28 shoulders to evaluate glenohumeral instability, in 23 to assess the joint and tendons before acromioplasty for chronic supraspinatus tendinitis, in 8 to diagnose or stage arthritic diseases, in 7 to define rotator cuff tears, in 3 to assess long-standing periarthritis, in 2 to visualize intra-articular fractures, in 2 to treat infections, and in 1 to evaluate the extent of pigmented villonodular synovitis. Examination of the shoulder with the patient under

Arthrography of the shoulder is a simple, safe, and relatively painless diagnostic procedure that is used to demonstrate the integrity, shape, and capacity of the glenohumeral joint space.

The shoulder, a very common site of pain syndromes in medical practice, lends itself well to precise clinical analysis and identification of the pain-sensitive structure or structures. Once identified, rational and effective management can be applied, associated with predictably good prognosis. Early identification of the emerging specific syndrome is important in decreasing the duration of the clinical disorder and in achieving optimum return of shoulder function. Laboratory and X-ray studies are not commonly required in diagnosis and management. There are a confusing variety of names attache

Stemless humeral components for shoulder arthroplasty represent the fourth generation of modern prosthetic shoulder implants. Because of their metaphyseal fixation, the implantation technique is rather straightforward and preserves the humeral canal from violation. Substantial benefits have been highlighted with such a design, including less perioperative morbidity, independence from the proximal humeral anatomy, preservation of bone stock, ease of potential revision surgeries, and limited risk of complex periprosthetic fractures. Initially conceived to better re-create the center of rotation

Posttraumatic stiffness of the shoulder joint is a frequent and socioeconomically relevant result of injury; however, prior to making the diagnosis as the only cause of a persisting impairment of motion, it is necessary to rule out other sequelae of trauma. Even intensive conservative treatment is mostly accompanied by the stagnation of the impairments of movement. In many cases the treatment of choice is early arthroscopic arthrolysis with a standardized approach and intensive follow-up treatment. In the surgical treatment of proximal humeral fractures with humeral head preservation, arthrosc

Posterior shoulder instability is a relatively uncommon condition, occurring in ∼10% of those with shoulder instability. Because of the rarity of the condition and the lack of knowledge in treatment, it is often misdiagnosed or patients experience a delay in diagnosis. Posterior instability typically affects athletes participating in contact or overhead sports and is usually the result of repetitive microtrauma or blunt force with the shoulder in the provocative position of flexion, adduction, and internal rotation, leading to recurrent subluxation events. Acute traumatic posterior dislocation

CONTEXT: Posterior shoulder instability has become more frequently recognized and treated as a unique subset of shoulder instability, especially in the military. Posterior shoulder pathology may be more difficult to accurately diagnose than its anterior counterpart, and commonly, patients present with complaints of pain rather than instability. "Posterior instability" may encompass both dislocation and subluxation, and the most common presentation is recurrent posterior subluxation. Arthroscopic and open treatment techniques have improved as understanding of posterior shoulder instability has
