Effect of trotting speed, load and incline on hindlimb stance-phase kinematics.

The objective was to understand how the stance-phase kinematics of the hindlimb of trotting horses change with speed under 3 conditions (level, loaded and incline), to compare our results with the predictions of the spring-mass model and to help focus our future studies of muscle function. Video recordings were made of 5 Arabian horses trotting on a treadmill. Five consecutive strides were digitised and averaged for each trial. The angle-time diagrams were qualitatively similar to those reported previously. As speed increases, the range of motion of the hindlimb increases, as predicted by the

The treatment of Lisfranc injuries.

Thirty-one patients with fracture-dislocations of the tarsometatarsal joint were examined to assess the functional end results after a mean follow-up of 2.9 years (range 20 to 56 months). Sixty-one percent were polytrauma patients; 39% suffered isolated fractures. Forty-five percent had associated lesions of the tarsal joint complex (Chopart and subtalar joint). According to the Baltimore Painful Foot Scoring System (PFS), 52% achieved an excellent or good result and 48% a fair or poor result. Of the four different treatment modalities, open reduction and temporary screw or K-wire fixation yie

Selective hindfoot arthrodesis for the treatment of adult acquired flatfoot deformity: an in vitro study.

An acquired flatfoot deformity with significant laxity at the transverse tarsal joint was created experimentally and the amount of correction that was obtained with selective hindfoot fusions was measured radiographically. Results showed that the talonavicular, double (talonavicular and calcaneocuboid), and triple arthrodeses were able to fully correct the deformity, including correction of hindfoot valgus with just a talonavicular fusion. Subtalar and calcaneocuboid fusions failed to completely correct the deformity. This study provides experimental evidence that although the triple joints ar

Cuboid plantar and dorsal subluxations: assessment and treatment.

Plantar flexion and inversion stresses at the ankle may cause the tarsal cuboid to sublux in a plantar or dorsal direction resulting in pain and impaired joint function. Subluxation of the tarsal cuboid alters the passive physiological motion and accessory glides of the cuboid joints in characteristic patterns of motion restriction. Identification of the pattern of motion restriction is necessary for appropriate diagnosis and treatment. This paper presents the case reports of two patients with lateral foot pain. A subluxed cuboid bone was believed to be the cause of the patient's pain. Assessm

Tarsal coalitions. A new surgical procedure.

Coalition is a congenital fusion of two or more tarsal bones. Symptoms, if present, usually begin in early adolescence. The typical symptom complexes are aching pain in the rearfoot exacerbated by activity, progression of the flatfoot deformity, decreased tarsal motion, and sometimes peroneal spasm. When patients are asymptomatic, no treatment is necessary. If symptoms begin, the individual should be treated conservatively. If that fails, surgical intervention should be undertaken. Calcaneonavicular coalitions respond well to excision with extensor digitorum brevis interposition. Talocalcaneal

Subtleties of Lisfranc fracture-dislocations.

Tarsometatarsal (Lisfranc) fracture-dislocations can be very difficult to recognize radiographically. The evaluation of the Lisfranc joint can be simplified by meticulously studying the alignment of the metatarsal bases with their corresponding tarsal bones. The usefulness of this observation has been confirmed by: anatomic descriptions, biomechanical analysis, and clinical foot radiographs. Subtle abnormalities identified at the Lisfranc joint using this observation included: metatarsal subluxations identified only on a single projection, associated tarsal subluxations and dislocations, irred

Subtalar coalitions in the adult.

Tarsal coalitions, while relatively uncommon, are typically identified in adult patients during an evaluation for ankle instability, sinus tarsus pain, and/or pes planovalgus. The true incidence of tarsal coalition is unknown with estimates ranging from 1% to 12% of the overall population. The most common area of involvement of the subtalar joint is the middle facet, and heightened awareness should be present in adult patients with limited motion of the subtalar joint. Standard radiographic imaging, to include a Harris heel view, is recommended initially, although computerized tomography scan

The Importance of Reducing Medial Talonavicular Subluxation in Arthrodesis Surgery for Müller-Weiss Disease. The Guangdong China Experience.

Progressive compression of the lateral navicular and medial talonavicular subluxation constitute the primary pathomechanical processes in Müller-Weiss disease (MWD). Secondary deformities arise in the mid and hindfoot with ensuing peri-navicular pain and arthritis. The key to successful radiological and clinical, mid-to long-term outcomes for arthrodesis in MWD is to incorporate the reduction of medial talonavicular subluxation at the time of fusion. This approach helps address the various components of complex mid and hindfoot deformities. It also slows down the progression of adjacent joint

Clinical and radiological long-term outcomes after resection of tarsal coalitions in children and adolescents: a case series with a minimum of a six year-follow-up.

PURPOSE: Resection of tarsal coalitions aims to relieve pain and restore hindfoot mobility, especially in young, active patients over the long term. This study investigated (1) long-term differences in clinical outcomes between calcaneonavicular (CN) and talocalcaneal (TC) coalition resections; (2) the frequency and clinical relevance of re-ossification; and (3) radiological signs of osteoarthritis in adjacent hindfoot joints. METHODS: In this case series, 29 feet from 24 patients with a minimum follow-up of 80 months after tarsal coalition resection were examined. Clinical assessments includ

Ultrasonographic detection of joint lesions in canine leishmaniasis caused by Leishmania infantum.

Canine leishmaniasis (CanL), caused by Leishmania infantum, is a multisystemic disease with substantial musculoskeletal involvement. We aimed to assess the usefulness of USG in diagnosing joint abnormalities in dogs with CanL using a clinical approach. Forty-three L. infantum PCR-positive dogs underwent comparative orthopedic physical examination and joint USG; 20 (20/43) also underwent radiography. Tarsal joints were systematically imaged, whereas other joints were imaged if they showed orthopedic clinical signs. Tarsal lesions were detected by joint USG in 86% (37/43) of the dogs, with osteo

Multi-organ spreading of Actinobacillus pleuropneumoniae serovar 7 in weaned pigs during the first week after experimental infection.

Actinobacillus (A.) pleuropneumoniae is normally considered strictly adapted to the respiratory tract of swine. Despite this, scattered case reports of arthritis, osteomyelitis, hepatitis, meningitis or nephritis exist, in which A. pleuropneumoniae remained the only detectable pathogen. Therefore, the aim of this study was to investigate whether spreading to other organs than the lungs is incidental or may occur more frequently. For this, organ samples (blood, liver, spleen, kidney, tarsal and carpal joints, meninges, pleural and pericardial fluids) from weaners (n = 47) infected experimentall

Two stage surgical treatment of cuboid osteomyelitis. A case report and review of the literature.

Isolated osteomyelitis of the cuboid bone is an extremely rare condition. A 32 year old man was evaluated for a painful and swollen right foot after a penetrating trauma and the presence of a sinus in the lateral aspect the midfoot. MRI findings were consistent with the presence of cuboid osteomyelitis. The patient underwent a two stage procedure which included partial excision of the cuboid bone, the use of a cemented spacer for lateral column length preservation, followed by arthrodesis of the calcaneocuboid joint with a tricortical autologous bone graft harvested from the ilium and preserva

Ankle and foot deformities and malformations in proximal femoral focal deficiency.

PURPOSE: To describe foot abnormalities in proximal femoral focal deficiency and their correlation to the severity. METHODS: Eighty-nine extremities in 87 patients were evaluated between 1996 and 2020 clinically and radiologically. Fibula length, ankle shape, tarsal coalitions, and the number of foot rays were recorded. Extremities with proximal femoral focal deficiency were classified according to Pappas and divided into severe (classes II and V), medium severe (classes III and IV), and mild groups (classes VII, VIII, and IX). RESULTS: The fibula was short in 89% and absent in 11% of cases.

[Arthrodesis of the ankle. A study of the complications and long term results (author's transl)].

The authors have studied the complications and long term results of 91 cases of tibio-talar arthrodesis and 43 cases of tibio-talar arthrodesis associated with subtalar arthrodesis. Minor complications such as limited skin necrosis were noted in 13 p. 100 of the cases, and no-fusion occurred in 7 p. 100. The results were satisfactory in 75 p. 100 of the cases, fair in 13 p. 100, poor in 12 p. 100. Most of the poor results were related to painfull subtalar joints after isolated tibio-talar arthrodesis. This happened often following conditions : excessive equinus, protrusion of screws in the sub

Partial tarsal arthrodesis with closing wedge ostectomy for treatment of bilateral twisted leg deformity in a cat.

A 5-month-old male domestic shorthair cat was presented with severe bilateral hind-limb deformities that caused the cat to walk on the dorsal aspect of the metatarsals. Computed tomography (CT) images revealed that the calcaneus was externally rotated, and the distal end of the calcaneus was turned medially to the talus in both hind limbs. The cat was diagnosed with twisted leg deformity, a congenital tarsal hyperextension deformity (clubfoot). Based on CT images, closing wedge ostectomy was done at the level of the tarsometatarsal joint with the wider part facing laterally. Partial transectio

Tibial and Plantar Intraneural Ganglion Cysts in the Ankle Region: Analysis Supports Articular Origin to Subtalar and Tibiotalar Joints and Differing Patterns of Propagation.

BACKGROUND AND OBJECTIVES: Intraneural ganglion cysts (IGCs) can arise in different nerves but are uncommon in the tibial nerve (TN) and its branches near the ankle. According to the unified articular theory, a joint connection is always present in IGCs. This study aimed to identify joint connections of tibial-region IGCs and characterize their clinical, radiological, and propagation patterns. METHODS: A prospectively collected international database of >1100 IGCs was reviewed to identify tibial and medial plantar nerve (MPN) and lateral plantar nerve (LPN) IGCs around the ankle. RESULTS: Si
