Barriers and facilitators to a task-shifted stroke prevention program for children with sickle cell anemia in a community hospital: a qualitative study.

BACKGROUND: Children with sickle cell anemia (SCA) are at high risk for stroke. Protocols for stroke prevention including blood transfusions, screening for abnormal non-imaging transcranial Doppler (TCD) measurements, and hydroxyurea therapy are difficult to implement in low-resource environments like Nigeria. This study aimed to examine the contextual factors around TCD screening in a community hospital in Nigeria using qualitative interviews and focus groups. METHODS: We conducted a descriptive qualitative study in a community hospital in Kaduna, Nigeria, using focus groups and interviews.

The GluA1-Related BDNF Pathway Is Involved in PTSD-Induced Cognitive Flexibility Deficit in Attentional Set-Shifting Tasks of Rats.

Background: Post-Traumatic Stress Disorder (PTSD) is a severe psychological disorder characterized by intrusive thoughts, heightened arousal, avoidance, and flashbacks. Cognitive flexibility dysfunction has been linked with the emergence of PTSD, including response inhibition deficits and impaired attentional switching, which results in difficulties for PTSD patients when disengaging attention from trauma-related stimuli. However, the molecular mechanisms of cognitive flexibility deficits remain unclear. Methods: The animals were exposed to a single prolonged stress and electric foot shock (SP

A task-shifted speech therapy program for cleft palate patients in rural Nepal: Evaluating impact and associated healthcare barriers.

INTRODUCTION: Though access to surgical care for cleft lip/palate has expanded in low- and middle-income countries (LMICs), post-palatoplasty speech therapy is often lacking due to limited healthcare infrastructure and personnel. This mixed-methods study seeks to: 1) evaluate the impact of task-shifted speech therapy on a standardized speech score; 2) describe the experiences of families with post-operative cleft care and associated barriers; and 3) understand how to optimize cleft care by exploring the experiences of children who had nominal improvements after task-shifted speech therapy. Circadian phase-shifted rats show normal acquisition but impaired long-term retention of place information in the water task.

It is thought that circadian rhythms may influence learning and memory processes. However, research supporting this view does not dissociate a mnemonic impairment from other performance deficits. Furthermore, published reports do not specify the type of memory system influenced by the circadian system. The present study assessed the effects of phase shifting on acquisition and expression of place navigation in the water maze, a task sensitive to hippocampal dysfunction. The results showed that phase-shifting circadian rhythms in rats impaired the expression of place information on a retention

Point-of-care HIV viral load testing combined with task shifting to improve treatment outcomes (STREAM): findings from an open-label, non-inferiority, randomised controlled trial.

BACKGROUND: Monitoring HIV treatment with laboratory testing introduces delays for providing appropriate care in resource-limited settings. The aim of our study was to determine whether point-of-care HIV viral load testing with task shifting changed treatment and care outcomes for adults on antiretroviral therapy (ART) when compared with standard laboratory viral load testing. METHODS: We did an open-label, non-inferiority, randomised controlled trial in a public clinic in Durban, South Africa. We enrolled HIV-positive adults (aged ≥18 years) who presented for their first routine HIV viral lo

Demographic shifts in referral to and completion of lung cancer screening after the 2021 United States Preventive Services Task Force update.

BACKGROUND: The United States Preventive Services Task Force (USPSTF) expanded the age and pack-year criteria for lung cancer screening (LCS) in 2021 in part to improve equity in eligibility for women and racial minority groups. Prior research has examined characteristics of patients who participated in LCS (screened) based on completed low-dose computed tomography (LDCT) pre- and post-USPSTF update in majority White patient populations, but has not done so in more diverse settings. No prior studies have assessed changes in referral patterns related to the update. We therefore examined both sc

A randomized controlled trial of group cognitive remediation therapy for anorexia nervosa: Effects on set-shifting tasks for inpatient adults and adolescents.

OBJECTIVE: The aim of this randomized controlled trial with a parallel design was to evaluate the effect of brief, cognitive remediation therapy (CRT) for anorexia nervosa (AN) on set-shifting. METHOD: Two hundred seventy-five inpatient adults and adolescents with AN (mean age = 23.1; SD = 12.7) were randomly assigned (using simple randomization procedures) to either a CRT or control condition. All participants received treatment as usual; however, the CRT condition completed five CRT group sessions in lieu of other group therapies provided on the unit. Set-shifting abilities were evaluated b

Effects of changing reward on performance of the delayed spatial win-shift radial maze task in pedunculopontine tegmental nucleus lesioned rats.

Because it was designed to assess working memory, the delayed spatial win-shift (DSWS) radial maze task has been used to investigate the involvement of corticostriatal structures in executive processing. Excitotoxic lesions of the pedunculopontine tegmental nucleus (PPTg) produce profound deficits in performance of this task that are not accounted for by motor impairment. Thus, PPTg DSWS deficits are hypothesized to support a role for PPTg in complex cognitive processing. However, other studies indicate that the behaviour of PPTg lesioned rats varies depending on level of motivational exciteme

Comparative child mental health and academic outcomes after primary school teachers deliver task-shifted mental health care in India.

Teachers are promising lay counselors but have limited time for traditional therapy. To viably leverage teachers to deliver care, we developed Teachers Leading the Frontlines (Tealeaf), where teachers deliver a novel therapy, "education as mental health therapy" (Ed-MH); measures are an extension of classroom management, not an added task. We investigated whether Tealeaf teachers improved child mental health symptoms and academic achievement versus teachers delivering an enhanced usual care (EUC) comparator, Responding to Students' Emotions through Education (RE-SEED), a truncated version of T

When Grouped Cyclic Shift meets masked image modeling: Effective pre-training for data-scarce 3D ultrasound analysis tasks.

The inherent data scarcity in 3D ultrasound analysis demands data-efficient self-supervised learning (SSL) methods, yet prevalent approaches are often data-intensive. To bridge this gap, we present Grouped Cyclic Shift Masked Image Modeling (GCSMIM), a masked image modeling (MIM) pre-training framework designed for this low-data regime. GCSMIM employs a data-efficient architecture, where a convolutional neural network (CNN) backbone operates in shallow, high-resolution layers for hierarchical feature extraction, while a multilayer perceptron (MLP) operates in the deep, low-resolution layers to

Understanding the effects of a sudden directional shift in somatosensory feedback and increasing task complexity on postural adaptation in individuals with and without chronic ankle instability.

BACKGROUND: Individuals with chronic ankle instability (CAI) present somatosensory dysfunction following an initial ankle sprain. However, little is known about how individuals with CAI adapt to a sudden sensory perturbation of instability with increasing task and environmental constraints to maintain postural stability. METHODS: Forty-four individuals with and without unilateral CAI performed the Adaptation Test to a sudden somatosensory inversion and plantarflexion perturbations (environment) in double-, injured-, and uninjured- limbs. Mean sway energy scores were analyzed using 2 (group) ×

Teacher and caregiver perceptions of family engagement in teacher-led task-shifted child mental health care in a low-and-middle-income country.

The mental health needs of children in low-and-middle income countries (LMICs) often go unmet due to a lack of qualified mental health professionals. Task-shifting the provision of mental health services to teachers may facilitate access to care. Family engagement in task-shifting may support mental health outcomes but is understudied in this context. The current study explored teacher and caregiver perceptions of family engagement within a teacher-led, task-shifted mental health intervention in an LMIC. Primary school teachers from five schools in Darjeeling, India delivered evidence-based, i

Uptake and feasibility of task-shifting of Xpert MTB/RIF Ultra testing from laboratory technicians to nurses to increase access and reduce time to results: a multicountry mixed method research.

INTRODUCTION: Task-shifting tuberculosis (TB) rapid molecular testing from laboratory technicians to nurses could help decentralise TB diagnosis at primary health centres (PHCs) and allow in-ward testing to shorten treatment decision for very sick patients. We assessed the feasibility of Xpert MTB/RIF Ultra (Ultra) testing on nasopharyngeal aspirate (NPA) by nurses in children with presumptive TB at PHCs and in hospitalised children with severe pneumonia within the TB-Speed project in seven countries. METHODS: Of 23 PHCs and 15 paediatric wards, 9 and 4 respectively had nurses trained to perf

The effects of a task shifted multi-component mental health intervention to support prosthetic and orthotic service users in Cambodia: a non-randomised controlled study.

BACKGROUND: Mental health disorders are major public health challenges, particularly in low- and middle-income countries such as Cambodia, where chronic shortages in mental health services and human resources exist. These issues are compounded for marginalized groups such as persons with physical disabilities due to their being at a higher risk of psychological distress and PTSD symptoms. The development of effective and accessible mental health systems in Cambodia will require evidence-based culturally appropriate mental health interventions. There are insufficient good-quality studies which

Muscle Pain Induces a Shift of the Spatial Distribution of Upper Trapezius Muscle Activity During a Repetitive Task: A Mechanism for Perpetuation of Pain With Repetitive Activity?

OBJECTIVE: An association exists between repetitive movements and the development or perpetuation of neck-shoulder muscle pain. The mechanisms underlying this association remain unclear. This observational study investigated the effect of upper trapezius muscle pain on the distribution of upper trapezius activity during repetitive lifting. It was hypothesized that nociception would change the distribution of activity resulting in activation of muscle regions which would not normally be active during the task. MATERIALS AND METHODS: Healthy men repeatedly lifted a box with a cycle time of 3 se

Effects of transcranial direct current stimulation on improving performance of delayed- reinforcement attentional set-shifting tasks in attention-deficit/hyperactivity disorder rat model.

Behavioral flexibility (or set-shifting), which is regulated by the prefrontal cortex (PFC), is often impaired in patients with attention-deficit/hyperactivity disorder (ADHD), which is characterized by poor inhibitory control and reinforcement learning. Transcranial direct current stimulation (tDCS) has been proposed as a means of noninvasive brain stimulation and a potential therapeutic tool for modulating behavioral flexibility. Animal studies can pave the way to know if tDCS application can potentially benefit rule- and goal-based activities in ADHD. Spontaneously hypertensive rats (SHRs)

Strategies for knee stabilising and pivot-shift avoidance in a step-down and cross-over task observed sub-acutely after anterior cruciate ligament reconstruction.

BACKGROUND: Individuals with a recent anterior cruciate ligament reconstruction may demonstrate an altered movement strategy for protecting the knee and maintaining stability. Altered knee movement might lead to abnormal intra-articular load, potentially contributing to early knee osteoarthritis onset. A protective strategy may be particularly evident during active tasks that induce a pivot-shift manoeuvre, such as a step-down and cross-over task. In this study, we investigated whether knee joint mechanics and muscle activity differed between participants early (∼3 months) following reconstruc
