Mesenchymal Wnt signaling promotes formation of sternum and thoracic body wall.

Midline defects account for approximately 5% of congenital abnormalities observed at birth. However, the molecular mechanisms underlying the formation of the ventral body wall are not well understood. Recent studies linked mutations in Porcupine-an O-acetyl transferase mediating Wnt ligand acylation-with defects in the thoracic body wall. We hypothesized that anomalous Wnt signaling is involved in the pathogenesis of defective closure of the thoracic body wall. We generated a mouse model wherein Wntless (Wls), which encodes a cargo receptor mediating secretion of Wnt ligands, was conditionally

Chest wall deformities and thoracic scoliosis after costal cartilage graft harvesting.

Donor-site complications, specifically chest wall deformities and thoracic scoliosis, occurring after harvest of costal cartilage grafts are presented and discussed. The cases of 18 patients (12 male and 6 female), who underwent costal cartilage grafts for microtia reconstruction from 1975 to 1993, were reviewed for donor-site complications using radiography and physical examination. Ribs from which costal cartilage had been harvested showed increased inward bowing on radiographs in 16 of 32 donor sites. The frequency of rib deformity in donor sites was 20.0 percent when cartilages were harves

Anterior thoracic wall trauma in elderly homicide victims. The "CPR defense".

During the course of medicolegal postmortem examinations, forensic pathologists often encounter the sequelae of cardiopulmonary resuscitation (CPR) that was administered by medical and paramedical personnel. A wide variety of CPR-related injuries have been described since the institution nearly 30 years ago of this now common-place emergency technique. The forensic pathologist must be aware of both typical and unusual CPR-related trauma patterns in order to differentiate between injury produced during emergency therapy procedures and injury sustained through other mechanisms (for example, duri

Precision Medicine in Thoracic Surgery: Chest Wall, Pleural, and Mediastinal Malignancies.

Precision medicine has transformed cancer care by tailoring treatments to the molecular profiles of tumors. While advances have been significant in common cancers, rare malignancies of the chest including primary chest wall tumors, mesothelioma, mediastinal germ cell tumors, and thymic epithelial tumors have progressed more slowly due to their rarity and heterogeneity. Unique biological alterations in these tumors offer opportunities for targeted therapies, which may enhance surgical outcomes and improve survival. This article reviews the evolving precision medicine landscape for these thoraci

[Video-assisted Thoracoscopic Surgery for Mediastinal Tumor and Thoracic Wall Tumor].

"A final conceptual change concerns the appreciation that all major thymoma subtypes can behave in a clinically aggressive fashion and, therefore, should no longer be called benign tumors, irrespective tumor stage. Accordingly, their International Classification of Disease for Oncology( ICD)-O codes now have a /3 suffix, thymomas, for as malignant.1)" This manuscript indicated that almost all mediastinal tumors, which could not be easy to diagnose per-operatively, should be removed. There were two large problems for mediastinal tumors resection with video-assisted thoracoscopic surgery (VATS).

Cardiopulmonary Function in Thoracic Wall Deformities: What Do We Really Know?

Patients with pectus excavatum (PE) frequently present with complaints of exercise intolerance and cardiopulmonary symptoms. There continues to be controversy regarding the physiologic benefits of repair. The aim of this review is to summarize and discuss recent data regarding the cardiopulmonary effects of PE deformity and the evidence for improvement obtained after surgical repair including (1) a greater efficiency of breathing (chest wall mechanics), (2) improvement in pulmonary restrictive deficits, (3) an increase in cardiac chamber size and output, with improved cardiac strain and strain

Case Report: Cryoanalgesia for intractable thoracic pain due to chest wall tumor.

INTRODUCTION: Postoperative pain in thoracic surgery is a major concern, often delaying recovery. Cryoanalgesia, using pressurized nitrous oxide, offers long-term pain relief by temporarily blocking peripheral nerve function. It takes effect within 72 hours, so an intercostal nerve block is used for immediate pain relief. Cryoanalgesia is effective for severe pain, with minimal side effects and no drug interactions. CASE REPORT: A 63-year-old man with severe thoracic pain, unresponsive to opioids, was diagnosed with a typical carcinoid tumor after a CT scan revealed lesions in his lung, thora

The preoperative assessment of thoracic wall adhesions using four-dimensional computed tomography.

OBJECTIVE: Pleural adhesions are challenging during lung cancer surgery and may be associated with a long surgery time and excessive blood loss due to pleural adhesiolysis. We used preoperative four-dimensional computed tomography to quantitatively assess parietal pleural adhesions and determine its diagnostic accuracy. METHODS: A total of 216 patients with lung cancer underwent four-dimensional computed tomography during the study period. Pleural adhesions were subsequently confirmed by surgery in 85 of these patients, whereas 126 patients had no adhesions. The movements of the tumor or targ

Wall Shear Stress Estimation of Thoracic Aortic Aneurysm Using Computational Fluid Dynamics.

An attempt has been made to evaluate the effects of wall shear stress (WSS) on thoracic aortic aneurysm (TAA) using Computational Fluid Dynamics (CFD). Aneurysm is an excessive localized swelling of the arterial wall due to many physiological factors and it may rupture causing shock or sudden death. The existing imaging modalities such as MRI and CT assist in the visualization of anomalies in internal organs. However, the expected dynamic behaviour of arterial bulge under stressed condition can only be effectively evaluated through mathematical modelling. In this work, a 3D aneurysm model is r

Fistulization of hydatid mediastinal cyst to the anterior thoracic wall: case report.

Echinococcosis/hydatidosis is a frequent parasitic and zoonotic disease in the population engaged with agriculture and stockbreeding. It is seen most frequently in the liver and lung in adults. Mediastinal location of the disease is very rare. In this study we aimed to present a 31-year-old man having hydatid disease in the anterior mediastinum fistulated to the skin in the right subclavicular region, together with the literature. The patient was operated on through a partial sternotomy incision, and excision of the cyst and the fistula was performed.

Thoracic actinomycosis in an adolescent mimicking chest wall tumor or pulmonary tuberculosis.

Actinomycosis is a rare disease in children and young adolescents and its thoracic manifestations accounted for a minority of all cases. We report a case of a 12-year-old boy who presented with a right anterior chest wall mass for one week together with weight loss and low grade fever for one month. His symptoms and signs as well as the results of the radiological investigations (i.e. chest X-ray and computed tomography (CT) of thorax with contrast) mimicked pulmonary tuberculosis or chest wall tumor. The definite diagnosis of actinomycosis relies on the Gram stain microscopy and culture of th

Wall Stress Distribution in Bicuspid Aortic Valve-Associated Ascending Thoracic Aortic Aneurysms.

BACKGROUND: Bicuspid aortic valve-associated ascending thoracic aortic aneurysms (BAV-aTAAs) carry a risk of acute type A dissection. Biomechanically, dissection may occur when wall stress exceeds wall strength. Our aim was to develop patient-specific computational models of BAV-aTAAs to determine magnitudes of wall stress by anatomic regions. METHODS: Patients with BAV-aTAA diameter greater than 4.5 cm (n = 41) underwent electrocardiogram-gated computed tomography angiography. Three-dimensional aneurysm geometries were reconstructed after accounting for prestress and loaded to systemic press

Rib fracture patterns predict thoracic chest wall and abdominal solid organ injury.

Blunt trauma patients with rib fractures were studied to determine whether the number of rib fractures or their patterns were more predictive of abdominal solid organ injury and/or other thoracic trauma. Rib fractures were characterized as upper zone (ribs 1 to 4), midzone (ribs 5 to 8), and lower zone (ribs 9 to 12). Findings of sternal and scapular fractures, pulmonary contusions, and solid organ injures (liver, spleen, kidney) were characterized by the total number and predominant zone of ribs fractured. There were 296 men and 14 women. There were 38 patients with scapular fracture and 19 p
