BACKGROUND: Intact meniscus roots are a prerequisite for normal meniscal function, including even distribution of compressive forces across the knee joint. An injury to the root disrupts the hoop strength of the meniscus and may lead to its extrusion and the development of osteoarthritis. A medial meniscus posterior root tear (MMPRT) is often thought to have a primary degenerative pathogenesis. However, there is mention of some cases of MMPRTs where the patients have a solely traumatic injury to a previously healthy meniscus.
PURPOSE: To describe a subpopulation of patients with traumatic MMP

Arthroscopic debridement for degenerative joint disease has not been shown to be beneficial, and initial nonoperative management is now recommended for degenerative meniscal tears. Indications for arthroscopic partial meniscectomy (APM) are traumatic tears with a clear history of mechanical symptoms, such as locking and catching, with joint-line pain or acute onset of symptoms that have failed nonsurgical treatment. Predictors of dissatisfaction with APM include female sex, obesity, and lateral meniscal tears. Beyond such demographic factors, concomitant joint pathology negatively affects outc

A paucity of evidence exists by which to inform clinical decision-making in the management of repair of horizonal cleavage tears of the meniscus. Available data suggest reasonable outcomes and low failure rates; however, high-quality research is required to refine our understanding of optimal indications, techniques, and long-term outcomes with respect to function and joint preservation.

BACKGROUND: Bucket-handle meniscus tears (BHMT) are often displaced and unstable. The inside-out technique of repairing such tears is currently the gold standard. All-inside repair with meniscal fixators is getting increasingly popular. Shortcomings of the inside-out technique include neurovascular complications, especially saphenous nerve palsy, and retention of a non-resorbable suture which can result in discomfort to patient, granuloma formation, and a foci of infection. Hence, the purpose of this project was to innovate a novel all-inside technique to precisely reduce and fix BHMT while av

Meniscal root tears or avulsions compromise the biomechanical function of the menisci to a greater extent than simple meniscal tears do. As such, if left untreated, root injuries render the menisci incapable of properly distributing axial load and resisting rotation and translation. The clinical diagnosis of meniscal root abnormalities may be difficult as the signs and symptoms typically associated with meniscal body injuries, such as mechanical locking and catching, may not be present in patients with root injury and there may not be a history of an acute traumatic event. Arthroscopic partial meniscectomy (APM) has been demonstrated to be effective when performed in the appropriately indicated patient. However, a recent study published in the New England Journal of Medicine (NEJM) questioned whether or not the procedure actually had any clinical benefit whatsoever. Despite being a prospective, Level 1, randomized study, there are several aspects of the study that must be taken into consideration when interpreting the findings, including but not limited to the patient selection criteria, limited sample size, and lack of information regarding meniscal tear patter

BACKGROUND: Arthroscopic meniscal repair techniques are continuing to evolve. Most studies to date comparing the healing rate of inside-out to all-inside meniscal repair techniques are confounded by associated anterior cruciate ligament reconstruction or deficiency.
PURPOSE: This review was conducted to compare the effectiveness and complications of the inside-out repair technique to that of the all-inside repair technique in isolated unstable peripheral longitudinal ("bucket-handle") meniscal tears.
STUDY DESIGN: Systematic review.

Methods for the repair of meniscal tears include inside-out, outside-in, and all-inside techniques. In this article an all-inside technique is proposed for the posterior horn of the lateral meniscus that takes advantage of the capacious posterolateral recess when the knee is flexed. A device consisting of a large needle with a buttonhole in the tip (previously used for inside-out suture of the medial meniscus) is used. This technique uses only anterior portals (anteromedial and anterolateral portals and an accessory lateral or transpatellar tendon portal). The arthroscope is inserted through t

The arthroscopic "all-inside" meniscus suturing technique offers the arthroscopist a way of placing vertically oriented sutures through peripheral posterior horn tears located posterocentral without the risks of nerve, vessel, or posterior capsular entrapment inherent in both the "outside-in" and the "inside-out" arthroscopic methods. This technique introduces new instrumentation that allows the surgeon to both place sutures and tie suture knots intraarticularly under arthroscopic control.

A case of a rare, large discoid medial meniscus with a bucket-handle tear is described. The radiogram was normal and the clinical impression was torn medial meniscus. A diagnostic arthroscopy revealed an unusually large medial discoid meniscus with a bucket-handle tear. Open partial excision was performed following the arthroscopy.

Posterior medial meniscal root tears represent a clinically significant injury with biomechanical consequences comparable to total meniscectomy, underscoring the need for effective repair strategies to restore joint function and prevent degeneration. Current meniscal root repair techniques vary in biomechanical performance, with different constructs demonstrating trade-offs between ultimate failure load and resistance to cyclic displacement. Although constructs with higher failure loads may appear advantageous, emerging evidence suggests that cyclic stability may be more critical for maintaini

BACKGROUND: Limited biomechanical evidence exists describing how horizontal meniscus tears (HMTs), meniscal repair (MR), and meniscectomy alter the knee's biomechanical environment.
PURPOSE: To evaluate changes in knee contact mechanics following HMTs, MR, and meniscectomy.
STUDY DESIGN: Systematic review; Level of evidence, 5.
METHODS: PubMed, EMBASE, and CINAHL databases were systematically searched for biomechanical cadaveric studies of HMTs up to January 16, 2023, using the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) guidelines. Studies that (1) did not e

Because of the crucial role of the menisci in maintaining cartilage and joint health, meniscal tears affect the long-term health of the knee. Although partial meniscectomy has a role in the treatment of complex degenerative tears and tears with low healing capacity, advances in the concepts and understanding of meniscal repair, along with improvements in repair techniques and instrumentation, have expanded the indications for meniscal repair. With appropriate patient selection and preoperative planning, repair of meniscal tears can lower the rate of degenerative changes when compared with meni

PURPOSE: To compile and analyze structural and clinical outcomes after meniscus root tear treatment as currently described in the literature.
METHODS: A review was conducted to identify studies published since 2011 on efficacy of repair, meniscectomy, and nonoperative management in the treatment of meniscus root tears. Patient cohorts were grouped into treatment categories, with medial and lateral root tears analyzed separately; data were collected on patient demographics, structural outcomes including joint space width, degree of medial meniscal extrusion, progression to total knee arthropla

INTRODUCTION: The past 2 decades have been marked by substantial progress in our knowledge of meniscus anatomy, function, and biomechanics, and also by the shifting of the surgical treatment of meniscal lesions from traditional meniscectomy towards arthroscopic repair to get away from the early osteoarthritis associated with meniscectomy. Posterior horn injuries of the lateral meniscus (LM) have been less studied due to their lower incidence and also due to the historical technical complexity of performing a repair in the posterolateral compartment.
MATERIALS AND METHODS: A retrospective anal

A transection (root tear or complete radial tear) injury of the medial meniscus posterior horn is not rare in the oriental area and needs to be repaired to restore the hoop tension and to reduce the extruded meniscus, which leads to osteoarthritis of the knee. In cases with transection of the medial meniscus posterior horn, the meniscus can be repaired by a pull out suture technique. However, it is difficult to manipulate a suture hook and drill a tibial tunnel in the narrow medial joint space using the traditional anterior arthroscopic technique. This article describes a new pull out suture t

We introduce a suture technique to repair a peripheral tear near the posterior tibial attachment of the posterior horn. A suture hook was inserted through the posteromedial portal, and the peripheral capsular rim was penetrated from superior to inferior by the sharp hook. Both relay limbs were brought out through the posteromedial portal. The outer limb of the superior peripheral capsular rim was identified with a hemostat. An 18-gauge spinal needle loaded with a No. 0 polydioxanone suture (PDS) was introduced into the joint from the anteromedial portal; it was passed through the joint space u

Meniscal lesion does not mean meniscectomy and this is particularly true for the lateral meniscus. The reputation of mildness of the meniscectomy is usurped. The rate of joint space narrowing after lateral meniscectomy is of 40% at a follow-up of 13 years compared to 28% for the medial meniscus (symposium SFA 1996). Several arguments explain those results: biomechanical: the lateral meniscus contributes to the congruence; particularly the lateral meniscus is the zone where antero-posterior translational during knee flexion is 12 mm. The pejorative effects of lateral meniscectomy have conducted

CASE: We present a rare "flipped lid" type of fracture of the lateral tibial plateau with dislocation of the lateral meniscus into the notch with intact roots. This was fixed using headless screws through arthrotomy and peripheral suturing of the meniscus. At the 1-year follow-up, although there was terminal restriction of 10° flexion with redislocation of the lateral meniscus, the patient was able to perform all daily activities and declined further surgery.
CONCLUSION: Flipped lid-type fracture is rare and results in good outcome when fixed early. Meniscal dislocation repair requires close
