The proper tidal volume target using volume guarantee ventilation in the course of neonatal respiratory distress syndrome: a crucial endpoint.

Volume guarantee ventilation (VGV) is an optional ventilation mode which has become the most widely and extensively studied in the last 10 years in premature infants. Nonetheless, theoretical expected endpoints are intriguing and even though VGV has not yet become 'the' standard ventilation mode, a great deal of information has been acquired and stored. In our experience, VGV during the course of respiratory distress syndrome is useful and can be considered a standard ventilation mode. Weaning occurs in real time as described earlier, and synchronized intermittent positive pressure ventilation

Evolution of tidal volume requirement during the first 3 weeks of life in infants <800 g ventilated with Volume Guarantee.

BACKGROUND: Volume-targeted ventilation is used in neonates to reduce volutrauma and inadvertent hyperventilation. Little is known about appropriate tidal volume (V(T)) settings in extremely low birthweight (ELBW) infants who remain intubated for extended periods. HYPOTHESIS: The V(T) required to maintain adequate partial pressure of carbon dioxide (P(CO2) levels changes as the underlying disease evolves in infants ventilated for prolonged periods. OBJECTIVE: To obtain normative data for V(T) associated with normocapnia in ELBW infants ventilated with Volume Guarantee over the first 3 weeks

Relative tidal volume and respiratory airflow estimation using tracheal sound and movement during sleep.

Airflow is the reference signal to assess sleep respiratory disorders, such as sleep apnea. Previous studies estimated airflow using tracheal sounds in short segments with specific airflow rates, while requiring calibration or a few breaths for tuning the relationship between sound energy and airflow. Airflow-sound relationship can change by posture, sleep stage and airflow rate or tidal volume. We investigated the possibility of estimating surrogates of tidal volume without calibration in the adult sleep apnea population using tracheal sounds and movements. Two surrogates of tidal volume: tho

Tidal breathing flow volume profiles during sleep in wheezing infants measured by impedance pneumography.

Overnight analysis of tidal breathing flow volume (TBFV) loops, recorded by impedance pneumography (IP), has been successfully applied in the home monitoring of children with wheezing disorders. However, little is known on how sleep physiology modifies the relationship between TBFV profiles and wheeze. We studied such interactions in wheezing infants. Forty-three infants recruited because of recurrent lower airway symptoms were divided into three groups based on their risk of asthma: high (HR), intermediate (IR), or low (LR). Sedated patients underwent infant lung function testing including as

Effects of different tidal volume ventilation on paraquat-induced acute lung injury in piglets.

BACKGROUND: The aim of this study was to explore the effects of different tidal volume (VT) ventilation on paraquat-induced acute lung injury or acute respiratory distress syndrome (ALI/ARDS) in piglets. MATERIAL AND METHODS: We developed ALI/ARDS models in piglets by intraperitoneal injection of paraquat (PQ). The piglets were randomly divided into three groups: small VT group (VT=6 ml/kg, n=6), middle VT group (VT=10 ml/kg, n=6), and large VT group (VT=15 ml/kg, n=6), with the positive end-expiratory pressure (PEEP) set as 10 cmH2O. The hemodynamics were monitored by pulse-indicated continu

Use of dual targeting during volume control ventilation is associated with increased delivery of non-lung-protective tidal volumes.

PURPOSE: Dual targeting (DT) is a patient-comfort ventilator feature that allows for increased flow in response to patient demand. Since flow is augmented above the set flow, larger tidal volumes (VT) than set may be delivered. This study assessed the impact of DT on the ability to maintain delivered VT ≤ 8 mL/kg of ideal body weight (IBW). MATERIALS AND METHODS: This was a single center retrospective cohort study of medical ICU patients receiving volume-targeted mechanical ventilation in the year before and after system-wide implementation of a ventilator featuring DT. The primary outcome wa

Reduced tidal volume-inflection point and elevated operating lung volumes during exercise in females with well-controlled asthma.

INTRODUCTION: Individuals with asthma breathe at higher operating lung volumes during exercise compared with healthy individuals, which contributes to increased exertional dyspnoea. In health, females are more likely to develop exertional dyspnoea than males at a given workload or ventilation, and therefore, it is possible that females with asthma may develop disproportional dyspnoea on exertion. The purpose of this study was to compare operating lung volume and dyspnoea responses during exercise in females with and without asthma. METHODS: Sixteen female controls and 16 females with asthma w

Comparison of Different Tidal Volumes for Ventilation in Patients with an Acute Traumatic Cervical Spine Injury.

BACKGROUND: There is scant literature comparing high tidal volume ventilation (HTV) over low tidal volume (LTV) ventilation in acute traumatic cervical spinal cord injury (CSCI). OBJECTIVE: The aim of this prospective randomized controlled parallel-group, single-blinded study was to compare the effect of two different tidal volumes (12-15 mL/kg and 6-8 mL/kg) in CSCI on days to achieve ventilator-free breathing (VFB), PaO2/FIO2 ratio, the incidence of complications, requirement of vasopressor drugs, total duration of hospital stay, and mortality. Work of breathing at different tidal volume targets in newborn infants with congenital diaphragmatic hernia.

UNLABELLED: Congenital diaphragmatic hernia (CDH) results in varying degrees of pulmonary hypoplasia. Volume targeted ventilation (VTV) is a lung protective strategy but the optimal target tidal volume in CDH infants has not previously been studied. The aim of this study was to test the hypothesis that low targeted volumes would be better in CDH infants as determined by measuring the work of breathing (WOB) in CDH infants, at three different targeted tidal volumes. A randomised cross-over study was undertaken. Infants were eligible for inclusion in the study after surgical repair of their diap

Comparison of "open lung" modes with low tidal volumes in a porcine lung injury model.

BACKGROUND: Ventilator strategies that maintain an "open lung" have shown promise in treating hypoxemic patients. We compared three "open lung" strategies with standard of care low tidal volume ventilation and hypothesized that each would diminish physiologic and histopathologic evidence of ventilator induced lung injury (VILI). MATERIALS AND METHODS: Acute lung injury (ALI) was induced in 22 pigs via 5% Tween and 30-min of injurious ventilation. Animals were separated into four groups: (1) low tidal volume ventilation (LowVt -6 mL/kg); (2) high-frequency oscillatory ventilation (HFOV); (3) a

Vibrating mesh nebulization increases tidal volume relative to jet nebulization during NIV in COPD exacerbation.

BACKGROUND AND IMPORTANCE: Noninvasive ventilation (NIV) is the first-line treatment for acute exacerbations of chronic obstructive pulmonary disease (COPD) with hypercapnic respiratory failure (HRF). Bronchodilator therapy is essential during NIV, but real-world clinical comparisons of nebulization devices in this acute setting remain limited. OBJECTIVE: To compare the clinical efficacy of vibrating mesh nebulizers (VMN) versus jet nebulizers (JN) in COPD patients undergoing NIV, using changes in tidal volume (VT) as the primary outcome. DESIGN, SETTINGS AND PARTICIPANTS: A longitudinal obs

A technique to measure tidal volume during noninvasive respiratory support by continuous-flow helmet CPAP.

PURPOSE: The coronavirus disease 2019 (COVID-19) pandemic has promoted the use of helmet continuous positive airway pressure (CPAP) for noninvasive respiratory support in hypoxic respiratory failure patients, despite the lack of tidal volume monitoring. We evaluated a novel technique designed to measure tidal volume during noninvasive continuous-flow helmet CPAP. METHODS: A bench model of spontaneously breathing patients undergoing helmet CPAP therapy (three positive end-expiratory pressure [PEEP] levels) at different levels of respiratory distress was used to compare measured and reference t

Diaphragm excursions as proxy for tidal volume during spontaneous breathing in invasively ventilated ICU patients.

There is a need to monitor tidal volume in critically ill patients with acute respiratory failure, given its relation with adverse clinical outcome. However, quantification of tidal volume in non-intubated patients is challenging. In this proof-of-concept study, we evaluated whether ultrasound measurements of diaphragm excursion could be a valid surrogate for tidal volume in patients with respiratory failure. Diaphragm excursions and tidal volumes were simultaneously measured in invasively ventilated patients (N = 21) and healthy volunteers (N = 20). Linear mixed models were used to estimate t

Predicting the outcome of respiratory disease in wheezing infants using tidal flow-volume loop shape.

INTRODUCTION AND OBJECTIVES: Wheezing (RW) infants with a positive asthma predictive index (API+) have a lower lung function as measured by forced expiratory techniques. Tidal flow-volume loops (TFVL) are easy to perform in infants, and sedation is not necessary. MATERIALS AND METHODS: A total of 216 wheezing infants were successfully measured, and 183 of them were followed for over a year. TFVL loops were classified into one of three categories depending of their geometric shape (symmetric, convex, and concave). Respiratory rate (Rr), presence of API+, and the number of exacerbations during

Effective ventilation at conventional rates with tidal volume below instrumental dead space: a bench study.

OBJECTIVE: The authors previously showed that 48% of infants <800 g were ventilated with tidal volume (VT) < dead space (DS) using volume guarantee (VG) ventilation. Here, The authors sought to confirm those findings under the rigorous conditions of a bench study. DESIGN AND METHODS: The authors measured the time to wash out CO2 from a 45-ml test lung using end-tidal CO(2) monitor (ETCO(2)). The test lung was filled with 100% CO(2), then ventilated using VG at VT ranging from DS+2 ml to DS-1.5 ml. With ventilation, ETCO(2) declined exponentially as CO(2) was washed out, the rate being proport
