Abnormal Head Posture in Unilateral Superior Oblique Palsy.

Purpose: To evaluate the manifestations and severity of abnormal head posture (AHP) in unilateral congenital and acquired superior oblique palsy (SOP) patients and to assess the effect of AHP on facial appearance. Patients and methods: This case series study was performed on 60 consecutive SOP patients, which consisted of 47 patients with congenital SOP and 13 patients with the acquired source. The exact type of AHP in congenital and acquired cases was determined based on direct observation. In addition, a close-up picture from 40 cm with habitual AHP was captured from all patients. For evalua

Surgical Management of Unilateral Superior Oblique Palsy: Thirty Years of Experience.

INTRODUCTION AND PURPOSE: We describe the clinical characteristics of 252 patients with unilateral superior oblique palsy who underwent strabismus surgery. We assess if a predetermined surgical strategy, based on preoperative alignment and motility measurements, was effective in treating these patients. On this basis, the patients were divided into three different treatment groups. METHODS: Two-hundred fifty-two patients were identified retrospectively and classified into three groups according to the performed procedures: 1) inferior oblique weakening; 2) inferior rectus recession; 3) combin

Torsional Diplopia.

BACKGROUND AND PURPOSE: Torsional diplopia can result in failure of fusion in an individual without a measureable strabismus. When presented with a patient with complaints of binocular diplopia, physicians and orthoptists should consider cyclovertical muscle dysfunction when the source of the complaint is not readily apparent. METHODS: A thorough review of the literature combined with the author's own personal experience in treating adult patients with strabismus was used to evaluate the different potential causes of torsional diplopia. Predisposing factors, diagnostic techniques, and strabis

Bilateral sequential trochleitis as the presenting feature of systemic lupus erythematosus.

A 26-year old woman presented with headache and pain in the left superonasal orbit, which worsened with vertical eye movements. She had no relevant medical history, and ophthalmologic evaluation was unremarkable. An orbital ultrasound showed enlargement of soft tissue in the region of the left trochlea consistent with trochleitis. Treatment with prednisone and multiple local injections of corticosteroids and analgesic nerve blocks failed to relieve her symptoms. The patient subsequently experienced right trochleitis, and 2 years after the onset of her initial symptoms, she developed systemic s

Absence of the fourth cranial nerve in congenital Brown syndrome.

PURPOSE: To elucidate the aetiology of congenital Brown syndrome. METHODS: Four consecutive patients diagnosed with unilateral congenital Brown syndrome had a comprehensive standardized ocular motility examination. Any compensatory head posture was measured. Brain magnetic resonance imaging (MRI) with regard for the IV cranial nerve (CN) was performed in all patients. Orbital MRI was performed in 2/4 patients, with images acquired in eight directions of gaze and superior oblique (SO) muscle areas compared. RESULTS: CN IV could not be identified bilaterally in two patients, but was absent onl

Traumatic trochlear nerve palsy following minor occipital impact--four case reports.

Four patients presented with transient trochlear nerve palsy following occipital impact. The impact was slight in all patients, and neuroimaging found no lesions of the nerve. Three patients had delayed onset. Two patients did not complain of diplopia, but were only aware of blurred vision. In such patients, the head tilting test was useful for diagnosis. Minor occipital impact can cause trochlear nerve palsy, which requires careful neurological examinations to identify.

Traumatic trochlear nerve palsy diagnosed by magnetic resonance imaging: case report and review of the literature.

Although head trauma is the leading cause of acquired trochlear nerve dysfunction, it receives little attention in the neurosurgical literature. A case is reported of closed head injury that resulted in a right superior oblique palsy in association with incoordination on the left side. Diagnostic imaging revealed a normal cranial computed tomographic scan and a left dorsal midbrain lesion on magnetic resonance imaging scan. The relevant anatomy is reviewed, as well as the action of the superior oblique muscle, its agonists and antagonists, and the clinical manifestations of superior oblique dy

Paresis of cranial nerves III, IV, and VI: clinical manifestation and differential diagnosis.

Successful identification of the cranial nerve and ocular muscle responsible for a subjective complaint of diplopia requires an evaluation of the type and character of the double vision and not infrequently the use of a red glass or Maddox rod, especially in incomplete and subtle cases. An isolated third nerve lesion is most commonly seen with a supraclinoid aneurysm (pupil dilated and fixed), vascular disease (pupil spared), and trauma. Mild frontal head trauma and vascular disease are the most common etiologies associated with an isolated fourth nerve paresis. Tumor, vascular disease and tra

Third, fourth, and sixth cranial nerve palsies.

Clinical manifestations of ocular motor palsies may differ according to the type and the localization of the lesions involving the third, fourth, and sixth cranial nerves. Topical diagnosis of the third, fourth, and sixth nerve palsies is therefore required before imaging studies and workup are performed. The development of modern imaging techniques has significantly improved the diagnosis of the disorders affecting the ocular motor nerves. This review covers the most important aspects, in terms of clinical signs and symptoms and differential diagnoses, of these cranial nerve palsies. The more

Fourth and sixth nerve palsies due to Herpes Simplex 1 infection.

Ocular motor cranial nerve palsies of viral etiology are uncommon and, when accompanied by skin lesions, zoster ophthalmicus is the most frequent diagnosis. We describe the case of a 68-year-old woman who developed fourth and sixth nerve palsies 3 days after appearance of a painful vesicular skin rash on the left side of her forehead. Neuroimaging was normal but polymerase chain reaction (PCR) testing of the cerebrospinal fluid was positive for Herpes Simplex 1 and negative for Varicella Zoster. The patient was treated with intravenous acyclovir, and the cranial nerve palsies resolved over 7 w

[Oculomotor, trochlear, and abducens nerve palsies].

Patients with oculomotor, trochlear, or abducens nerve palsies mainly complain of binocular double vision, but sometimes merely of blurred vision or vertigo. The clinical signs comprise strabismus, pathologic head posture, and disturbed saccades. The characteristic motility deficits are picked up by measuring the strabismic angles at different directions of gaze. Documentation of all three spatial strabismic components is advantageous. Nonparetic strabismus and orbital diseases are important differential diagnoses. Conclusions about the cause of a palsy can be drawn from the time course of dou

Usefulness of posterior transpetrosal approach for the large solid cerebellopontine angle hemangioblastoma fed from multiple blood supplies: A technical case report.

BACKGROUND: Extra-axial cerebellopontine angle (CPA) hemangioblastomas are rare clinical entity and surgical treatment is challenging due to the anatomical difficulties and multi-directional blood supplies. On the other hand, the risk of endovascular treatment for this disease has also been reported. Herein, we successfully applied a posterior transpetrosal approach to remove a large solid CPA hemangioblastoma without preoperative feeder embolization. CASE DESCRIPTION: A 65-year-old man presented with a complaint of diplopia during downward gaze. Magnetic resonance imaging revealed a solid tu

Trochlear nerve palsy in Sjögren's syndrome.

Sjögren's syndrome (SS) is a systemic lymphoproliferative, autoimmune disease, which is characterized by dryness of the eyes, mouth, and other mucous membranes. The nervous system may be affected in up to 20% of the cases of primary or secondary SS. We present a case of a 54-year-old woman with trochlear nerve palsy complicating Sjögren's syndrome secondary to rheumatoid arthritis. We suggest that all patients with multiple cranial neuropathies, especially when associated with rheumatoid arthritis, should be carefully examined for the possible presence of secondary SS.

Medial transposition of the lateral rectus muscle in combined third and fourth nerve palsy.

Surgical treatment of combined third and fourth nerve palsy is a challenging problem in strabismology. Five of the 6 extraocular muscles are paralyzed, which leaves the lateral rectus muscle with no antagonist to counteract its activity and usually results in a maximal exotropia. The goal of surgery is to achieve orthophoria in primary position with limited ductions. Because some believe that a conventional recession-resection procedure will inevitably result in a drift back to exotropia,(1) several other methods have been proposed to treat this disorder. These include temporal mattress suture
