Risk-reducing mastectomy for the prevention of primary breast cancer.

BACKGROUND: Recent progress in understanding the genetic basis of breast cancer and widely publicized reports of celebrities undergoing risk-reducing mastectomy (RRM) have increased interest in RRM as a method of preventing breast cancer. This is an update of a Cochrane Review first published in 2004 and previously updated in 2006 and 2010. OBJECTIVES: (i) To determine whether risk-reducing mastectomy reduces death rates from any cause in women who have never had breast cancer and in women who have a history of breast cancer in one breast, and (ii) to examine the effect of risk-reducing maste

Alteration in estrogen receptor status in metachronous contralateral breast cancer among unilateral early breast cancer patients with BRCA 1/2 mutations.

BACKGROUND: BRCA1/2 mutations significantly increase the risk of contralateral breast cancer (CBC). However, their influence on estrogen receptor (ER) status changes between primary breast cancer and metachronous CBC remains unclear. This study investigates ER status alterations in CBC patients with BRCA1/2 mutations to clarify their clinical implications. METHODS: In this multicenter retrospective study (2004-2020), patients with unilateral early breast cancer who underwent BRCA1/2 testing and developed metachronous CBC were grouped by BRCA1/2 mutation status. ER status of primary and CBC tu

Heart sparing radiotherapy in breast cancer: the importance of baseline cardiac risks.

BACKGROUND: Patients with left-sided breast cancer have an increased risk of cardiovascular disease (CVD) after radiotherapy (RT). While the awareness of cardiac toxicity has increased enormously over the last decade, the role of individual baseline cardiac risks has not yet been systematically investigated. Aim of the present study was to evaluate the impact of baseline CVD risks on radiation-induced cardiac toxicity. METHODS: Two hundred ten patients with left-sided breast cancer treated in the prospective Save-Heart Study using a deep inspiration breath-hold (DIBH) technique were analysed

Sertoli-leydig cell tumour of ovary with menorrhagia: a rare case report.

Sertoli-Leydig cell tumours (SLCTs) are rare sex cord stromal neoplasms of ovary accounting for less than 0.5% of all ovarian tumours. These are found in women of all age groups (2-75 y), but are most common in reproductive age group with an average age of 25 y. Mostly these are unilateral, confined to ovaries and usually stage I at the time of clinical diagnosis. The common presenting complaints in these patients are due to either mass occupying lesion (mostly pelviabdominal mass and/or pain) or hormonal production (mostly androgen and more rarely oestrogen). Androgenic manifestations, seen i

Gastric dose-volume parameters and toxicity in left-sided breast radiotherapy: A single-center retrospective comparison of helical tomotherapy and 3-dimensional field-in-field techniques.

During left-sided breast radiotherapy (LSBRT), several organs may be exposed to irradiation and develop treatment-related side effects. Compared with cardiopulmonary toxicity, gastric complications remain relatively under-investigated, although gastrointestinal complications are not uncommon in patients treated with LSBRT. This retrospective single-center study aimed to evaluate the relationship between gastric dose-volume parameters, gastric toxicity, and target volume coverage in patients treated with Helical Tomotherapy (HT) and 3-dimensional field-in-field (3D-FIF) techniques. A rare avascolar osteonecrosis of the knee related to biphosphonate treatment in a patient with metastatic breast cancer.

Bisphosphonates are widely used both in the multidisciplinary management of bone metastases, especially osteolytic lesions from solid tumors or multiple myeloma, and of osteoporosis.Aseptic osteonecrosis, especially of the jaw, is among the well-known, although uncommon, side effects of bisphosphonates. Osteonecrosis of other bones has been very rarely reported with the use of bisphosphonates. We describe a rare case of osteonecrosis of the distal femur associated with the use of bisphosphonates in a 74 years old female patient with metastatic breast cancer.

Clinical phenotypes combined with saturation genome editing identifying the pathogenicity of BRCA1 variants of uncertain significance in breast cancer.

Characterizing the pathogenicity of BRCA1 variants of uncertain significance (VUSs) is a major bottleneck in clinical management of BRCA1-associated breast cancer. Saturation genome editing (SGE) was recently reported as an innovative laboratory-based approach to assess the pathogenicity of BRCA1 variants. We combined clinical phenotypes and SGE score to identify the pathogenicity of BRCA1 VUSs detected in a cohort of 8,085 breast cancer patients. According to SGE function score, 33 out of 144 BRCA1 VUSs detected were classified into "loss of function" (n = 13), "intermediate" (n = 2), and "fu

Dosimetric comparison of DIBH and FB for left-sided breast cancer radiation therapy.

This study compared deep inspiration breath-hold (DIBH) versus free-breathing (FB) techniques in postoperative radiotherapy for left-sided breast cancer. Dosimetric parameters for 94 patients were analyzed. DIBH significantly improved target conformity while reducing cardiac exposure, with mean heart dose decreasing by 2.631 Gy (EQD2). Significant dose reductions were also observed in bilateral lungs, esophagus, and spinal cord. A weak correlation was identified between left lung volume expansion and organ-at-risk dose reduction. Receiver operating characteristic analysis determined that a lef

Assessing the Effect of Lifetime Contralateral Breast Cancer Risk on the Selection of Contralateral Prophylactic Mastectomy for Unilateral Breast Cancer.

INTRODUCTION: Contralateral prophylactic mastectomy (CPM) rates are rising, with fear implicated as a contributing factor. This study used a contralateral breast cancer (CBC) risk stratification tool to assess whether the selection of CPM is reflective of future CBC risk. PATIENTS AND METHODS: This retrospective study evaluated 404 women with unilateral breast cancer treated with breast conservation, unilateral mastectomy, or bilateral mastectomy within a single multidisciplinary clinic. Women were evaluated by the Manchester risk tool to calculate lifetime CBC risk. Logistic regression analy

A three day coaching protocol for deep inspiration breath hold in left breast radiotherapy using active breathing control.

The deep inspiration breath-hold (DIBH) technique is recommended for left-sided breast radiotherapy to reduce heart dose exposure. However, this technique introduces uncertainties due to possible anatomical variations across successive breath-holds, requiring patient coaching to ensure reproducibility. This study aims to establish an objective coaching framework to optimize DIBH effectiveness and achieve the reproducibility of chest anatomy in left-sided breast radiotherapy. This study implemented a three-day DIBH coaching protocol using the Active Breathing Coordinator (ABC) system (Elekta AB
