A simplified method for determination of uroporphyrinogen decarboxylase activity in human blood.

The determination of erythrocyte uroporphyrinogen decarboxylase activity is essential for differentiating familial (type II) porphyria cutanea tarda from the sporadic (type I) form of the disease. A new technique for the determination of uroporphyrinogen decarboxylase activity in human blood is described. Haemolysate is incubated with uroporphyrinogen III as substrate. Uroporphyrinogen unconverted during the reaction is oxidised to uroporphyrin and measured directly as free acid by HPLC. The enzyme activity is then calculated from the amount of substrate consumed. The technique is simple, rapi

Ascorbic acid deficiency in porphyria cutanea tarda.

Porphyria cutanea tarda (PCT), the most common form of porphyria, is manifested as skin photosensitivity caused by excess hepatic production of uroporphyrin and heptacarboxylporphyrin. In experimental animal models, ascorbic acid modulates chemically induced uroporphyrin accumulation. The purpose of this study was to determine whether ascorbic acid is decreased in the plasma of patients with PCT. Plasma was obtained after an overnight fast from 21 PCT patients, 16 of whom were infected with hepatitis C virus (HCV), and from a separate group of 9 patients with HCV infection but not PCT. Thirtee

Chemically-induced formation of an inhibitor of hepatic uroporphyrinogen decarboxylase in inbred mice with iron overload.

An inhibitor of hepatic uroporphyrinogen decarboxylase (EC 4.1.1.37) was demonstrated in heat-treated extracts of livers from C57BL/10ScSn mice with iron overload after a single dose (100 mg/kg; 350 mumol/kg) of hexachlorobenzene (HCB). Inhibition was not due to accumulated uroporphyrin since this could be removed by a SEP-PAK C18 cartridge without affecting inhibitor activity. The presence of the inhibitor could be first demonstrated 2 weeks after mice received HCB and before major elevation of hepatic porphyrin levels. Maximum inhibitory potential was reached at about 8 weeks and was still d

[Association of idiopathic hemochromatosis and tardive cutaneous porphyria].

Porphyria cutanea tarda is a disorder of porphyrin metabolism that results from a deficiency of uroporphyrinogen decarboxylase, resulting in a characteristic pattern of porphyrin excretion. Elevated serum iron values are frequently observed among patients with porphyria cutanea tarda. The hypothesis has been advanced that a hemochromatosis allele is implicated in the clinical manifestation of porphyria cutanea tarda. We report the case of a patient suffering from both idiopathic hemochromatosis and porphyria cutanea tarda. The data of the medical literature concerning such an association are d

Neurovisceral Syndrome in a Patient with Monoclonal Gammopathy of Undetermined Significance: A Confirmed Case of Variegate Porphyria.

Variegate porphyria (VP) is a rare disorder presenting with cutaneous, neurological, and systemic symptoms. We report a 53-year-old man initially hospitalized for a depressive episode, during which blistering lesions and facial flushing were noted. He was misdiagnosed and treated for cellulitis and contact dermatitis without improvement. Repeated misdiagnoses led to extensive autoimmune, infectious, and hematologic workup, revealing coexisting monoclonal gammopathy of undetermined significance (MGUS). Over the following months, he developed abdominal pain, mood swings, cognitive impairment, ne

A prospective blinded evaluation of urinary porphyrins verses the clinical severity of autism spectrum disorders.

A prospective, blinded study evaluated the relationship between autism spectrum disorder (ASD) severity measured by Childhood Autism Rating Scale (CARS) scores and urinary porphyrins among a cohort of participants (n = 26). LabCorp (CLIA-approved) tested for uroporphyrins, heptacarboxylporphyrins, hexacarboxylporphyrins, pentacarboxylporphyrins, coproporphyrin (cP) I, and cP III levels. Participants with severe ASD had significantly increased cP I, cP III, and total cP levels in comparison to participants with mild ASD. A significant correlation was observed between increasing cP levels and CA

Case Report: Treatment of porphyria cutanea tarda with low dose hydroxychloroquine.

Background: Porphyria cutanea tarda (PCT) is a complex metabolic disease resulting from altered activity of the enzyme uroporphyrinogen decarboxylase (UROD) in the liver resulting in accumulation of uroporphyrin. PCT presents as a blistering photodermatitis with skin fragility, vesicles, scarring and milia. Case: We report a case of PCT in a 67-year-old man with hemochromatosis (HFE) gene mutation who, following a major syncopal episode in response to venesection was commenced on low dose hydroxychloroquine. Conclusions: Low dose hydroxychloroquine provided a safe and effective alternative to

[Premature cutaneous porphyria caused by oral contraceptives (author's transl)].

Four women, aged 23 to 26 years, fell ill with cutaneous hepatic porphyria. All of them had taken oestrogen-contaning oral contraceptives three to eight years before the illness. In one instance there was a time relationship with a progestogen preparation. The clinical and biochemical signs were those of porphyria cutanea tarda, but contrary to this disease there was a shift towards heptacarboxyporphyrin in the uroporphyrin/heptacarboxyporphyrin ratio (thin-layer chromatography, classified according to Doss). This difference and the onset at an early age suggest that the described disease may

Hepatic uroporphyrinogen decarboxylase activity in porphyria cutanea tarda patients: the influence of virus C infection.

Porphyria cutanea tarda (PCT) is caused by a decreased activity of the hepatic enzyme uroporphyrinogen decarboxylase (URO-D). This deficiency causes overproduction, hepatic deposition, and increased excretion of uroporphyrin. Iron overload and hepatic viral infections are considered aggravating factors of the disease. Two forms of PCT have been described, as follows: a familial one with an inherited decrease of URO-D activity in all tissues and a sporadic one with a decreased activity of URO-D restricted to the liver. To assess whether the hepatic URO-D returns to normal during a remission of

Phlebotomy treatment of porphyria cutanea tarda.

The effect of phlebotomy on 44 patients (33 men and 11 women) with porphyria cutanea tarda (PCT) is described. All had typical biochemical and clinical signs of PCT. The patients were bled until slight anemia and signs of iron deficiency. In patients who were bled frequently and who were not given iron supplements in conjunction with phlebotomy, urinary uroporphyrin excretion decreased to 1 mg (or 1.2 mumol) a day or less in 1-8 months (mean 4 months) and then continued to decrease long after the treatment had been stopped and became normal in many patients. A decrease in porphyrin excretion c

Porphyria cutanea tarda associated with HFE C282Y homozygosity, iron overload, and use of a contraceptive vaginal ring.

Porphyria cutanea tarda (PCT) is characterized by decreased uroporphyrinogen decarboxylase activity in hepatocytes, uroporphyrin I and heptacarboxyl porphyrin III accumulation, photosensitivity dermatitis, and increased storage iron. In women, estrogen therapy, including oral contraceptives, postmenopausal hormone replacement, and tamoxifen for breast cancer treatment, is a risk factor for PCT. We report the case of a woman who presented with PCT, HFE C282Y homozygosity, and hepatic iron overload and was using a contraceptive vaginal ring containing ethinyl estradiol, an estrogen. We discuss t

Porphyria cutanea tarda induced by estrogens.

Three patients who developed cutaneous lesions of porphyrin cutanea tarda are reported after nearly a year or more of administration of diethylstilbestrol (Stilboestrol) for alleged carcinoma of the prostate. Two patients were benefited by protection of their skin from heat and friction. The third was free of lesions after discontinuance of diethylstilbestrol without protection. Two patients still show excessive excretion of uroporphyrin and coproporphyrin in the urine and feces (one a year after discontinuance of the synthetic estrogen). It is apparent that porphyrin cutanea tarda after dieth

[The screening diagnosis of porphyria cutanea tarda].

A method is proposed which gives an idea of the type of the absorption spectra of the total porphyrines in the urine without requiring its recording. A single portion of 0,2 ml of urine is mixed with 1,8 ml of IM HCL. The extinctions at 380, 401, 405 and 430 nm are read. The doubled real extinctions (2RE) at 401 and 405 are calculated by the formulae 2RE401--(E380 + E430) and 2E405--(E380 + E430). The extinctions at 380 and 430 outline the "background" of substances which are not porphyrines. When using Spekol 10 the results are multiplied by 1,6 and for Spekol II--they are multiplied by 1,4.

Porphyrin synthesis by human hepatocytes and HepG2 cells--effects of enzyme inducers and delta-aminolevulinic acid.

Experiments were carried out to investigate the possibility of inducing porphyria in human hepatocytes and HepG2 cells in culture. After treatment with hexachlorobenzene, 3-methylcholanthrene, phenobarbital or dimethyl sulfoxide, protoporphyrin was the predominating porphyrin accumulating in presence of delta-aminolevulinic acid. The typical uroporphyrin accumulation, as is seen in hexachlorobenzene-induced porphyria in vivo, was absent. In HepG2 cells, the activities of uroporphyrinogen decarboxylase and porphobilinogen deaminase were not influenced by cytochrome P-450 inducers, hexachloroben
