Autonomic function and hemodynamic changes in type 2 diabetes: insights into sympathetic activation and vascular stiffness.

AIMS: This study aimed to investigate the relationship between autonomic function, arterial stiffness, and vascular compliance in patients with type 2 diabetes (T2D). METHODS: Eighty-one T2D patients were divided into two groups based on disease duration: <10 years (N = 50) and >10 years (N = 31). Autonomic function was assessed via heart rate variability, and vascular hemodynamics via pulse wave velocity (PWV). Linear and multivariate regression analyses explored associations between autonomic function and vascular parameters. RESULTS: Female participants predominated, accounting for 60 % i

Chronic esaxerenone treatment improves vascular function and lowers peripheral arterial stiffness in patients with idiopathic hyperaldosteronism.

OBJECTIVE: Primary aldosteronism is one of the most common types of secondary endocrine hypertension. Treatment with mineralocorticoid receptor antagonists is recommended for bilateral idiopathic hyperaldosteronism (IHA). The purpose of this study was to evaluate the effects of esaxerenone, a nonsteroidal mineralocorticoid receptor antagonist with high mineralocorticoid receptor-binding specificity, on vascular function and peripheral arterial stiffness in patients with IHA. METHODS: This study was a single-center, observational prospective cohort study. Forty-four patients with IHA (18 men a

Decoding vascular aging: Substrate stiffness and shear stress orchestrate endothelial inflammation and remodelling via mechanosensitive pathways.

Vascular ageing is a major contributor to cardiovascular disease and is closely linked to vascular stiffness. Stiffening of the vascular wall disrupts endothelial homeostasis and promotes chronic inflammation, yet the underlying mechanisms remain poorly understood due to technological limitations. Here, we used a previously developed in vitro microfluidic model to investigate how biomechanical forces, specifically substrate stiffness and shear stress, interact to regulate endothelial cell behaviour. Using RNA sequencing and functional assays, we found that endothelial cells exposed to both phy

The relationship of renal function to segmental vascular stiffness, ankle-brachial index, and peripheral artery disease.

The authors consecutively assessed various arterial pulse-wave velocity (PWV) indices and ankle-brachial index (ABI) by an automatic device (VP2000, OMRON Health Care Co. Ltd., Kyota, Japan) in outpatients with ≥ 1 cardiovascular risk. PAD was defined as ABI ≤ 0.9. Among 2309 outpatients (mean age 62.4 years), worse renal function was associated with higher brachial-ankle PWV, heart-carotid PWV, heart-femoral PWV (hf-PWV), and lower ABI (all P < .001). Multivariate regression models showed independent associations between lower eGFR, lower ABI (Coef: 0.42 & 0.41 for right and left), higher hf-

Determination of brachial artery stiffness prior to vascular access creation: reproducibility of pulse wave velocity assessment.

BACKGROUND: Despite routine ultrasound mapping of upper extremity arteries and veins, early thrombosis and nonmaturation remain frequent complications following vascular access (VA) surgery. Besides vascular diameters, brachial artery stiffness is assumed to play an important role; however, reproducibility of measurements has never been established. The purpose of this study was to determine within-session and between-session variabilities of pulse wave velocity (PWV) assessment by using ultrasonography and blood pressure registration. METHODS: Beat-to-beat changes in brachial artery diameter

The Role of Renin-Angiotensin-Aldosterone System and Its New Components in Arterial Stiffness and Vascular Aging.

Many cardiovascular diseases present renin-angiotensin-aldosterone system (RAAS) hyperactivity as an important pathophysiological mechanism to be target in the therapeutic approaches. Moreover, arterial stiffness is currently considered as a new independent risk factor for cardiovascular disease in different clinical conditions, including hypertension and chronic kidney disease. In fact, excessive stimulation of angiotensin type 1 (AT1) receptors, as well as mineralocorticoid receptors, results in cellular growth, oxidative stress and vascular inflammation, which may lead to arterial stiffness

Carotid artery stiffness as an early marker of vascular lesions in children and adolescents with cardiovascular risk factors.

INTRODUCTION AND OBJECTIVES: The availability of a noninvasive marker of vascular lesions that enables their detection in the preclinical phase would be of great benefit for cardiovascular disease prevention. The aim of this study was to investigate the usefulness of a range of indices of arterial wall stiffness in the common carotid artery, as derived using high-resolution Doppler ultrasonography, for identifying vascular damage in children with risk factors. METHODS: The study involved 99 children (age, 8-16 years) divided into two groups: 65 had cardiovascular risk factors (45 obesity, 20

Opposing Responses of the Calcium Channel Blocker Nicardipine to Vascular Stiffness in the Elastic and Muscular Arteries in Rabbits.

AIM: The cardio-ankle vascular index (CAVI) consists of intrinsic and functional arterial stiffness mainly regulated by vasoactive compounds. A new stiffness index of the aorta (aBeta) and iliac-femoral arteries (ifBeta) was determined by applying the CAVI theory to the whole aorta and iliac-femoral arteries. We investigated the changes in aBeta and ifBeta in response to decreased blood pressure (BP) induced by the Ca2+ channel blocker nicardipine to elucidate the involvement of Ca2+ in aBeta and ifBeta. METHODS: Pressure waves at the origin of the aorta (oA), distal end of the abdominal aort

[The diagnostic value and clinical significance of a method for estimating the arterial stiffness by cardio-ankle vascular index].

The review presents the data of clinical studies of a new method for estimating the arterial stiffness by cardio-ankle vascular index (CAVI). The most significant factors influencing the vascular wall are analyzed and a CAVI measuring procedure and a calculation algorithm are described in detail. An association of the estimation of CAVI with other procedures determining the rigidity of the arterial wall is analyzed. A possibility for the practical application of this index to the study of the elastic properties of the vascular wall and for its use as a surrogate criterion to evaluate the vasop
