OBJECTIVES: Failure of mechanical conventional ventilation (MCV) after cardiac surgery portends a dismal prognosis, with extracorporeal membrane oxygenation frequently utilized as a salvage therapy. We describe our experience with high-frequency percussive ventilation (HFPV) as a rescue therapy for hypoxaemia refractory to MCV after cardiac surgery.
METHODS: In a 6-year retrospective analysis from 2009 to 2015, we identified 16 subjects who required HFPV after cardiac surgery. Data regarding demographics, intraoperative details, postoperative ventilatory settings including length of time on H

Home mechanical ventilation represents a valuable therapeutic option to improve alveolar ventilation in patients with chronic respiratory failure. For this purpose both invasive ventilation via tracheostomy and noninvasive ventilation via facemasks are available. The primary goal of home mechanical ventilation is a reduction of symptoms, improvement of quality of life and in many cases reduction of mortality. Elective establishment of home mechanical ventilation is typically provided for noninvasive ventilation in respect to clinical symptoms and partial pressure of carbon dioxide depending on

For many patients with chronic respiratory failure requiring ventilator support, noninvasive ventilation (NIV) is preferable to invasive support by tracheostomy. Currently available evidence does not support the use of nocturnal NIV in unselected patients with stable COPD. Several European studies have reported benefit for high intensity NIV, in which setting of inspiratory pressure and respiratory rate are selected to achieve normocapnia. There have also been studies reporting benefit for the use of NIV as an adjunct to exercise training. NIV may be useful as an adjunct to airway clearance te

A 56-year-old man recovering from a glossectomy and radical neck dissection presented severe oral bleeding, tracheal deviation with an asphyxiating hematoma and cyanosis. When 2 attempts at orotracheal intubation with the patient awake failed, transtracheal jet ventilation was used temporarily until a definitive airway could be established. Transtracheal jet ventilation is highly useful for managing an airway and maintaining gas exchange in life-threatening situations in which intubation and ventilation has become impossible, yet it is rarely used for that purpose. An easy, fast procedure that

AIMS: Newborns affected by congenital diaphragmatic hernia (CDH) are high-risk patients: today the mortality is still elevated and is essentially due to severe pulmonary hypoplasia, pulmonary hypertension and the absence of surfactant. High-Frequency Oscillatory Ventilation (HFOV) seems to be a good pre- and postoperative technique in cases of CDH.
PATIENTS AND METHODS: We report our experience in the treatment of CDH; since 1987 we have followed 44 patients. We divided them into two different groups in accordance with the ventilation technique used: Group I (1987-1994): 25 patients treated w

Perfluorochemical (PFC) liquids have great potential for biomedical use and the support of respiration. Currently, there are several commercially available PFC fluids which meet the physiochemical property requirements as well as purity specifications necessary to perform many of the discussed biomedical applications. Moreover, state-of-the-art fluorine chemistry should enable production of new PFC liquids uniquely sculptured relative to the proposed specific application (ie. vehicle for pulmonary delivery of drugs, a diluent for pulmonary lavage, a medium for respiratory gas exchange). In add

Ten anesthetized normal dogs were each given two methacholine inhalational challenges to produce large amounts of low ventilation-perfusion (VA/Q) regions but little shunt. After one challenge, high-frequency ventilation (HFV) was applied, whereas after the other conventional mechanical ventilation (MV) was used, the order being randomized. Levels of both ventilatory modes were selected prior to challenge so as to result in similar and normal mean airway pressures and arterial PCO2 levels during control conditions. Gas exchange was assessed by both respiratory and multiple inert-gas transfer.

1. The extent to which the pattern of breathing at maximal ventilation in man is affected by the mechanical properties of the respiratory pump has been studied. 2. The maximal effort flow volume (MEFV) loop has been used to calculate the shortest possible inspiratory (TI) and expiratory (TE) durations associated with the highest ventilation for all tidal volumes (VT). These minimal TIS and TES hve been plotted on a VT-TI-TE diagram. 3. Such predicted minimal TIS and TES were compared with observed minimal values from five healthy subjects who tried to reach their maximal ventilations during th

INTRODUCTION: Chronic hypercapnic respiratory failure often necessitates non-invasive ventilation (NIV) at home. Our study aimed to assess the static and dynamic performance of six modern NIV home ventilators and one intensive care unit (ICU) ventilator and to identify performance clusters among the devices.
METHODS: A two-compartment lung model was connected to seven NIV ventilators (Sefam Stan, Philips A40, Philips DreamStation, Resmed Lumis 150, Löwenstein PrismaVent 30C, Löwenstein PrismaVent 40 and BellaVista 1000) in pressure-support mode. Static and dynamic (triggering and pressurisati

The development of the computer technology brought reform in the field of medical equipment. Originally the mechanical ventilator was an instrument only as for running by pressure and the tool that let you breathe. However, it has a function to assist a measurement (tidal volume, peek pressure, etc.) and to wean from a ventilator. There is a case to use a mechanical ventilator for after a chest surgical operation. After the operation without the complication, it seems that there is not the special administration. However, special respiratory management is necessary in case of chronic respirato

AIM: Two different ventilation protocols during general anesthesia have been compared regarding their effects on amount of epidural bleeding during lumbar microdiscectomy in the prone position. Previous studies have not not revealed a relationship between ventilation mode and epidural bleeding in the prone position while measuring the intra-abdominal pressure (IAP) level.
MATERIAL AND METHODS: This study was conducted on 40 American Society of Anesthesiologists (ASA) I and II patients that underwent a single level, unilateral lumbar microdiscectomy in the prone position. The anesthetic protoc

The cardiovascular and respiratory systems act as a functional unit. Mechanical ventilation modifies pulmonary volumes, which generates changes in autonomic nervous system reactivity and provokes tachy- or brady-cardia (depending on the tidal volume used). Mechanical ventilation also decreases cardiac filling volumes (pre-load) and alters pulmonary vascular resistances. In addition, intrathoracic pressures are enlarged, which usually produces a decrease in right atrium filling and an increase in right ventricle afterload. If coronary flow is impaired, myocardial contractility is reduced. Howev

OBJECTIVE: To review anaesthesia ventilators in current use in France by categories of ventilators.
DATA SOURCES: References were obtained from computerized bibliographic search. (Medline), recent review articles, the library of the service and personal files.
DATA SYNTHESIS: Anaesthesia ventilators can be allocated into three groups, depending on whether they readminister expired gases or not or allow both modalities. Contemporary ventilators provide either constant volume ventilation, or constant pressure ventilation, with or without a pressure plateau. Ventilators readministering expired

Noninvasive ventilation (NIV) has emerged as a powerful tool for the pediatric emergency management of acute respiratory failure (ARF). This therapy is safe and well tolerated and seems to frequently prevent both the need for invasive mechanical ventilation and the associated risks/complications. Although NIV can be the primary treatment of ARF resulting from multiple respiratory disease states, it must be meticulously monitored and, when unsuccessful, may aid in preoxygenation for prompt endotracheal intubation and invasive mechanical ventilation. The following article reviews the physiologic

There is mounting evidence that early continuous positive airway pressure (CPAP) from birth is feasible and safe even in very preterm infants. However, many infants will develop respiratory distress syndrome (RDS) and require surfactant treatment. Combining a noninvasive ventilation approach with a strategy for surfactant administration is important to ensure optimal outcome, but questions remain about the optimal timing, mode of delivery and value of predictive tests for surfactant deficiency. Key findings in this review include the following: (1) a noninvasive ventilation strategy with CPAP

Airway access for mechanical ventilation (MV) can be provided either by orotracheal intubation (OTI) or tracheostomy tube. During episodes of acute respiratory failure, patients are commonly ventilated through an orotracheal tube that represents an easy and rapid initial placement of the airway device. OTI avoids acute surgical complications such as bleeding, nerve and posterior tracheal wall injury, and late complications such as wound infection and tracheal lumen stenosis that may emerge due to tracheostomy tube placement. Tracheostomy is often considered when MV is expected to be applied fo
