Robot-guided Ventriculoperitoneal Shunt in Slit-like Ventricles.

BACKGROUND: Ventriculoperitoneal shunt (VPS) is the most common procedure used in the management of hydrocephalus regardless of the etiology. The standard free-hand technique is used for the placement of VPS in patients with enlarged ventricles. In patients with very small ventricles, CSF access through ventriculostomy becomes challenging and free-hand technique may be associated with high failure rates. In these situations, stereotactic-guided VPS becomes very useful. OBJECTIVE: To validate and describe the technique of robotic-guided VPS in cases with very small ventricles. METHODS: Three

Investigation of ventriculoperitoneal shunt disconnection for hydrocephalus treatment.

OBJECTIVE: This investigation is aimed at gaining a better understanding of the factors that lead to mechanical failure of shunts used for the treatment of hydrocephalus, including shunt catheter-valve disconnection and shunt catheter fracture. METHODS: To determine the root cause of mechanical failure, the authors created a benchtop mechanical model to mimic mechanical stressors on a shunt system. To test shunt fracture, cyclical loading on the catheter-valve connection site was tested with the shunt catheter held perpendicular to the valve. Standard methods were used to secure the catheter

Splenic laceration: an unusual complication of ventriculoperitoneal shunt.

Ventriculoperitoneal shunt is a commonly performed procedure. Associated complications have been well described in the literature. This is the first case report of a splenic laceration caused by the distal catheter looping around the spleen, requiring a splenectomy. Careful study of pre-operative imaging is recommended in cases where intraabdominal injury is suspected.

Laparoscopic management of ventriculoperitoneal and lumboperitoneal shunt complications.

BACKGROUND AND OBJECTIVES: Minimally invasive approaches for the initial placement of ventriculoperitoneal (VP) and lumboperitoneal (LP) shunts have been well described. A laparoscopic approach has multiple advantages over open techniques, including decreased morbidity, more rapid recovery, and ability to visually assess catheter function. However, few series have addressed the role of laparoscopy in the management of VP and LP shunt complications. METHODS: We present here the largest published series of laparoscopic treatment of VP and LP shunt complications in adults, by retrospectively rev

Ventriculoperitoneal shunt complications in California: 1990 to 2000.

BACKGROUND: Risk factors for ventriculoperitoneal (VP) shunt complications have not been assessed with population cohort data since the advent of modern surgical techniques. We examined demographic factors and VP shunt complications in a population-based retrospective cohort study of all nonfederal California hospital admissions between 1990 and 2000. METHODS: We identified all admissions in a statewide administrative hospital discharge database that indicated a VP shunt insertion. Repeat hospital admissions within California generating a discharge diagnosis of surgical shunt complication, de

Ventriculoperitoneal shunt malfunction presenting with pleuritic chest pain.

BACKGROUND: Ventriculoperitoneal (VP) shunts are widely used for treating hydrocephalus. These devices are prone to malfunction with up to 70% requiring revision. Shunt infection and obstruction comprise the majority of malfunctions and usually present dramatically. However, rare presentations occur. METHODS/RESULTS: We report a rare case of VP shunt malfunction presenting with pleuritic chest pain. A 13-year-old girl with a VP shunt placed at birth for congenital hydrocephalus presented on multiple occasions with pleuritic chest pain, cough, and fever. She was diagnosed with an upper respira

Ventriculoperitoneal shunt dysfunction during rehabilitation: prevalence and countermeasures.

OBJECTIVE: We examined the prevalence of shunt dysfunction (e.g., overdraining or underdraining malfunctions) in patients with a ventriculoperitoneal shunt and elucidated effective countermeasures of a programmable valve shunt system in treatments for shunt dysfunction during rehabilitation therapy. SUBJECTS: Among 114 patients with a ventriculoperitoneal shunt for normal pressure hydrocephalus, underdraining appeared in eight patients during hospitalization for rehabilitation therapy, and seven patients experienced overdraining. RESULTS: We could treat underdraining noninvasively for all si

Intestinal volvulus--a rare complication of ventriculoperitoneal shunt.

Intestinal volvulus causing obstruction is an unusual complication of ventriculoperitoneal shunts. There are no clear guidelines on the management of this problem. In our patient exteriorization of the shunt with subsequent reinsertion at a different site led to an uneventful recovery. The extreme rarity of this complication and its challenging management are discussed.

Ventriculoperitoneal shunt-associated infection due to Haemophilus influenzae.

We report two children with ventriculoperitoneal shunt infections due to Haemophilus influenzae, type b. One child was cured with systemic and intraventricular antimicrobial therapy alone. The other child had persistently positive ventricular fluid cultures, ultimately required shunt removal in addition to antimicrobial therapy, and still had a relapse, which responded to antimicrobial therapy alone. In treating children with this problem we recommend removal of the shunt apparatus unless there is prompt sterilization of ventricular fluid and clinical improvement. Systemic antimicrobial therap

Iatrogenic failure of a ventriculoperitoneal shunt. Case report.

A previous undescribed cause of ventriculoperitoneal shunt malfunction is presented. Limitation of abdominal excursion by a spica cast used to correct congenital hip dislocation reduced cerebrospinal fluid flow through a shunt. This easily correctable cause of "shunt failure" nearly resulted in replacement of a working shunt.

Abdominal Pseudocyst Post Ventriculoperitoneal Shunt: A Systematic Review.

Abdominal pseudocyst (APC) is a rare complication of ventriculoperitoneal shunt (VPS). The purpose of this study is to provide an updated systematic review to provide a comprehensive understanding of this complication and to guide clinical decisions. A comprehensive search was conducted to cover articles published between October 1969 and December 2022. The search was conducted according to PRISMA guidelines and resulted in the identification of 162 eligible papers that met the study criteria. The study included 208 patients with a mean age of 12 years. The majority were pediatric patients (62

Cerebrospinal Fluid Prognostic Index in Ventriculoperitoneal Shunt Infection.

BACKGROUND: Considering the consequences of shunt infection, the importance of early diagnosis and effective treatment becomes clear. However, there is no clear parameter to predict the management of shunt infection. Since we thought that an index including cerebrospinal fluid (CSF) leukocyte, glucose, and protein levels may affect treatment guidance, we aimed to investigate its effect on prognosis using the index we defined as CSF prognostic index. METHODS: Ventriculoperitoneal shunt infection patients' age, gender, etiology of hydrocephalus, serum glucose, leukocytes, white blood cell, C-re

Outcomes of laparoscopic and open ventriculoperitoneal shunt placement.

OBJECTIVE: Ventriculoperitoneal (VP) shunt placement requires a concurrent abdominal procedure. For peritoneal access laparoscopic or open approach may be utilized. Our aim was to compare patient/procedure characteristics and outcomes by peritoneal approach for VP shunts in children. METHODS: NSQIP-Pediatric procedure targeted cerebral spinal fluid shunt Participant Use Data Files from 2016 to 2020 were queried. Patients were grouped into laparoscopic vs open abdominal approach. Patient demographics, procedure characteristics and 30-day outcomes were compared. RESULTS: 7742 NSQIP-Pediatric p
