Synchronized nasal intermittent positive pressure ventilation (SNIPPV) decreases work of breathing (WOB) in premature infants with respiratory distress syndrome (RDS) compared to nasal continuous positive airway pressure (NCPAP).

Synchronized nasal intermittent positive pressure ventilation (SNIPPV) is non-invasive respiratory support that delivers ventilator breaths via the nasal prongs. We hypothesized that SNIPPV is more effective than nasal continuous positive airway pressure (NCPAP) in premature neonates due to decreased work of breathing (WOB). Fifteen infants (BW: 1,367 +/- 325 g, GA: 29.5 +/- 2.4 weeks) were studied on (a) NCPAP at 5 cmH(2)O (NCPAP5) and (b) three increasing SNIPPV settings achieved by NCPAP5 with additional delivered peak inspiratory pressures (PIP) of 10, 12, and 14 cmH(2)O. Tidal volumes and

Work of Breathing into Snow in the Presence versus Absence of an Artificial Air Pocket Affects Hypoxia and Hypercapnia of a Victim Covered with Avalanche Snow: A Randomized Double Blind Crossover Study.

Presence of an air pocket and its size play an important role in survival of victims buried in the avalanche snow. Even small air pockets facilitate breathing. We hypothesize that the size of the air pocket significantly affects the airflow resistance and work of breathing. The aims of the study are (1) to investigate the effect of the presence of an air pocket on gas exchange and work of breathing in subjects breathing into the simulated avalanche snow and (2) to test whether it is possible to breathe with no air pocket. The prospective interventional double-blinded study involved 12 male vol

Log in and breathe out: efficacy and cost-effectiveness of an online sleep training for teachers affected by work-related strain--study protocol for a randomized controlled trial.

BACKGROUND: Insomnia and work-related stress often co-occur. Both are associated with personal distress and diminished general functioning, as well as substantial socio-economic costs due to, for example, reduced productivity at the work place and absenteeism. Insomnia complaints by people experiencing work-related stress are correlated with a deficient cognitive detachment from work. Diffuse boundaries between work and private life can additionally complicate the use of recreational activities that facilitate cognitive detachment.Cognitive behavioral therapy for insomnia is effective but rare

Challenges and perspectives in obstructive sleep apnoea: Report by an ad hoc working group of the Sleep Disordered Breathing Group of the European Respiratory Society and the European Sleep Research Society.

Obstructive sleep apnoea (OSA) is a major challenge for physicians and healthcare systems throughout the world. The high prevalence and the impact on daily life of OSA oblige clinicians to offer effective and acceptable treatment options. However, recent evidence has raised questions about the benefits of positive airway pressure therapy in ameliorating comorbidities. An international expert group considered the current state of knowledge based on the most relevant publications in the previous 5 years, discussed the current challenges in the field, and proposed topics for future research on epi

When Helping Babies Breathe Is Not Enough: Designing a Novel, Mid-Level Neonatal Resuscitation Algorithm for Médecins Sans Frontières Field Teams Working in Low-Resource Hospital Settings.

BACKGROUND: Neonatal resuscitation (NR) combines a set of life-saving interventions in order to stabilize compromised newborns at birth or when critically ill. Médecins Sans Frontières/Doctors Without Borders (MSF), as an international medical-humanitarian organization working particularly in low-resource settings (LRS), assisted over 250,000 births in obstetric and newborn care aid projects in 2016 and provides thousands of newborn resuscitations annually. The Helping Babies Breathe (HBB) program has been used as formal guidance for basic resuscitation since 2012. However, in some MSF project

[Problems of regulation of breathing].

Knowledge about the regulation of breathing has pronounced clinical consequences. In the review the concepts of control and regulation of breathing are defined. Regulation based on the chemical feed back is well documented. Apart from that, attention is paid to unproven, hypothetical regulations. Among them, the regulation of the breathing pattern and that of functional residual lung capacity are of especial interest. Not less important is the cortical or volitional regulation of breathing, which is so far almost unexplored. An important problem represents abnormalities of the regulation of br

Control of extrathoracic airway dynamics.

The ventilatory controlling factors associated with oral augmentation of nasal breathing were investigated in 25 (14 women, 11 men) healthy adults during an incrementally graded bicycle exercise test. Ventilatory variables were measured by separate oral and nasal pneumotachometers integrated with a valveless oral-nasal face mask and a flexible oral catheter. Inspired and expired breath length, nasal flows, nasal ventilation, transnasal pressures, nasal work of breathing, nasal powers, and nasal resistances were measured simultaneously on a breath-by-breath basis and averaged over the 30-s inte

How to breathe? Respiratory mechanics and breathing pattern.

On theoretical grounds any given level of pulmonary or alveolar ventilation can be obtained at various absolute lung volumes and through many combinations of tidal volume, breathing frequency and inspiratory and expiratory timing. However, inspection of specific cases of newborn and adult mammals at rest indicates that the breathing pattern reflects a principle of economy oriented toward minimal respiratory work. The mechanisms that permit optimization of respiratory cost are poorly understood; yet, it is their efficiency and coordination that permits pulmonary ventilation at rest to require o

Action of the isolated canine diaphragm on the lower ribs at high lung volumes.

The normal diaphragm has an inspiratory action on the lower ribs, but subjects with chronic obstructive pulmonary disease commonly have an inward displacement of the lateral portions of the lower rib cage during inspiration. This paradoxical displacement, conventionally called 'Hoover's sign', has traditionally been attributed to the direct action of radially oriented diaphragmatic muscle fibres. In the present study, the inspiratory intercostal muscles in all interspaces in anaesthetized dogs were severed so that the diaphragm was the only muscle active during inspiration. The displacements o

Potential advantages of patient-ventilator synchrony.

During conventional mechanical ventilation, fixed set pressure, flow, and tidal volume result in a mismatch between patient and ventilator inspiratory time and in a patient's inability to adapt to changing ventilatory demand. Synchrony between the patient and ventilator improves neuromuscular coupling and the ability to adapt to increased ventilatory demand or loading. The sensation of dyspnea prevents ineffective inspiratory efforts and attenuates periodic breathing during sleep.

Strategies to minimize breathing effort during mechanical ventilation.

A primary objective of mechanical ventilation is to alleviate the intolerable effort of breathing while allowing the patient to perform enough work to prevent atrophy. By assuming the workload associated with breathing, mechanical support averts ventilatory failure, prevents respiratory arrest, assures CO2 removal and pH homeostasis, while permitting the overtaxed respiratory muscles to replenish energy reserves as the primary process is addressed. Skillful manipulation of the breathing workload can often facilitate the ventilator withdrawal process. The objectives of this article are to chara

The switching point from nasal to oronasal breathing.

The switching point from nasal to oronasal breathing during incrementally graded submaximal exercise was determined in 30 (14 M, 16 F) healthy adult volunteers. Nasal airflow was measured by a pneumotachograph attached to a nasal mask. Oral airflow was determined as the difference between nasal airflow and total pulmonary airflow, the latter being measured by a head-out exercise body plethysmograph. The airflow and pressure signals were sampled every 20 msec by a micropressor, which calculated respiratory volumes and nasal work of breathing, and produced an on-line print-out. Twenty of the 30

Expiratory muscle dysfunction in critically ill patients: towards improved understanding.

INTRODUCTION: This narrative review summarizes current knowledge on the physiology and pathophysiology of expiratory muscle function in ICU patients, as shared by academic professionals from multidisciplinary, multinational backgrounds, who include clinicians, clinical physiologists and basic physiologists. RESULTS: The expiratory muscles, which include the abdominal wall muscles and some of the rib cage muscles, are an important component of the respiratory muscle pump and are recruited in the presence of high respiratory load or low inspiratory muscle capacity. Recruitment of the expiratory

A Novel Pharyngeal Clearance Maneuver for Initial Tracheostomy Tube Cuff Deflation in High Cervical Tetraplegia.

Mechanical insufflation-exsufflation, or "cough assist," is a commonly used method of clearing tracheal and pulmonary secretions in patients with respiratory insufficiency secondary to spinal cord injury. This report presents a novel technique termed the pharyngeal clearance maneuver, which uses a modified application of the mechanical insufflation-exsufflation device to mobilize "secretion burden" at the portion of the trachea above the tracheostomy cuff during cuff deflation. Utilization of this strategy may reduce the risk of aspiration, infection, and respiratory compromise for patients wi

Effects of long-hose breathing.

The need for aircrew members to use long breathing hoses between regulator and mask arises in the design of new transport aircraft and in several cargo operations requiring an open rear door. In this study, induced work of breathing through long hoses, the resulting changes in oxygen consumption, and comfort were studied in relation to altitude, hose length, and exercise level. Conclusions were that, above 18,000 ft (5.5 km), a 30-ft (9.1-m) hose is acceptable for all reasonable work levels; for altitudes between 13,000 and 18,000 ft (4.0 and 5.5 km), the hose length should preferably be 18 ft
