European journal of orthopaedic surgery & traumatology : orthopedie traumatologie2014eng
INTRODUCTION: The shoulder pain has been reported as the third most common site after low back pain and knee pain, mostly caused by impingement and rotator cuff syndromes. Surgery is indicated when non-operative treatment fails and can be open and/or arthroscopic. Are Neer's acromioplasty results still good? Are they modified when it is a professional disease (PD)? If yes is there any explanation?
MATERIALS AND METHODS: It is a retrospective hospital-based study, including all cases of Neer's acromioplasty done from 2006 to 2011, by the same senior surgeon for subacromial impingement syndrome

PURPOSE: The purposes of this study was to better understand the utilization of anterior acromioplasty over time, in the absence of rotator cuff repair; to examine the relation to patient characteristics (age, sex) and types of rotator cuff pathology (inflammation or fibrosis, partial-thickness tearing, full-thickness tearing undergoing debridement); and to assess the utilization of arthroscopy in this procedure.
METHODS: Using the resources of the Rochester Epidemiology Project, cataloging medical records of residents in Olmsted County, Minnesota, we identified 246 patients who underwent ant

BACKGROUND: The shoulder's subacromial space is of significant clinical interest due to its association with rotator cuff disease. Previous studies have estimated the subacromial space width to be 2-17 mm, but no study has measured in vivo subacromial space width during shoulder motion. The purpose of this study was to measure the in vivo subacromial space width during shoulder elevation in patients following rotator cuff repair.
METHODS: Biplane X-ray images were collected during shoulder elevation of 11 patients who had undergone rotator cuff repair. Glenohumeral joint motion was measured f

PURPOSE: Shoulder lesions are usually examined with the joint in only one or two positions. We examined the shoulder with the joint in a variety of positions. We also assessed the application of cine-MR to the detection of instability and impingement.
MATERIAL AND METHODS: The cine-MR examinations were performed in 30 patients and 15 healthy volunteers. We used an open 0.2 T system and a closed 1.0 T system. Spoiled gradient echo 2D T1-weighted images and turbo spin-echo T1- and T2-weighted images were obtained with a field of view of 180 mm. The examinations were videotaped and evaluated lat

PURPOSE: To test previously defined ultrasound (US) criteria for identification of partial-thickness tears of the rotator cuff.
MATERIALS AND METHODS: Before shoulder arthroscopy, 52 patients with shoulder pain for more than 3 months were examined with a 7.5-MHz commercially available linear-array transducer and a standardized study protocol. The criteria used to detect partial-thickness tears were (a) a mixed hyper- and hypoechoic focus in the crucial zone of the supraspinatus tendon and (b) a hypoechoic lesion visualized in two orthogonal imaging planes with either articular or bursal exten

A modified Neer acromioplasty, subacromial decompression, and débridement of massive, irreparable lesions of the supraspinatus and infraspinatus tendons was performed in fifty-seven patients. Fifty patients (fifty-three shoulders) were followed for an average of six and one-half years. The average age of the patients was sixty years (range, thirty-eight to seventy-four years). The results, as rated on the basis of pain, function, range of motion, strength, and satisfaction of the patient, were satisfactory in forty-four shoulders (83 per cent) and unsatisfactory in nine (17 per cent). A favora

Proximal migration of the humeral head is thought to indicate fatty infiltration of the rotator cuff muscles or rotator cuff tears. We sought to evaluate the influence of these rotator cuff abnormalities on the subacromial space. Using anteroposterior radiographs, ultrasound, and computed tomography, we analyzed 54 shoulders in 29 patients with rheumatoid arthritis. The upward migration index was defined as proximal migration of the humeral head relative to its size. The mean muscle density from computed tomography images was used to indicate fatty infiltration. Fatty infiltration of the infra

Twenty-four consecutive patients undergoing shoulder acromioplasty were given postoperative analgesia with a new method in which a patient-controlled continuous infusion of lidocaine infiltrated the subacromial space. Seventeen of the acromioplasties were done with arthroscopy, whereas 7 were performed with an open procedure. A 2% solution of lidocaine without epinephrine was used for both a continuous dose of 2 cc/h and patient-controlled interval doses of 1 cc administered at 15-minute intervals. The catheter was left in place for 72 hours. We prospectively studied complications, the patient

The objective of this paper is to present a review of the role of magnetic resonance imaging in the evaluation of shoulder impingement syndrome and rotator cuff tendon tears. This imaging modality is effective in demonstrating both the soft tissue and bony abnormalities associated with structural shoulder impingement which include subacromial-subdeltoid bursitis, supraspinatus tendinopathy, rotator cuff tendon tears, subacromial osteophytic spurs and acromioclavicular joint capsular hypertrophy and osteophytosis. The detection of a subacromial osteophytic spur is considered specific for should

BACKGROUND: The pathomechanics of primary frozen shoulders are not yet fully understood. There is ongoing uncertainty regarding the optimal extent of surgical release, with concerns about both over- and under-release of the joint capsule. In the frozen shoulder, different ranges of motion of the shoulder joint experience varying areas of stress, contributing to stiffness and limited movement. The ultimate goal of treatment is to restore the full range of motion while carefully addressing the specific areas of pathology. The purpose of this study was to analyze the kinematics of the glenohumera

The impingement syndrome is a common disorder of the shoulder girdle. The causes for this syndrome may be anatomic changes in the coracoacromial arch, also within the ultrastructural regions, on the one hand, or changes in the biomechanics which have developed for various reasons, on the other. Diagnosis is based on roentgenograms using the appropriate technique. In large-scale-studies, sonography has proved to be an extremely sensitive screening method for differential diagnosis of rupture of the rotator cuff. Magnetic resonance imaging might gain in value in the diagnosis of impingement as r

INTRODUCTION: The purpose of this study is to investigate the incidence of os acromiale in patients who had reverse shoulder arthroplasty (RSA) for rotator cuff tear arthropathy and the effect of presence of os acromiale on the functional results.
HYPOTHESIS: We hypothesize than in the presence of os acromiale, the contraction strength of the deltoid would decrease due to the dynamic downward depression of the bony fragment leading to less favorable clinical results.
MATERIAL AND METHOD: A total 46 patients with a mean age of 70.8 who had RSA and a minimum follow-up of 24 months were include

PURPOSE: The factors affecting the anatomical and functional outcomes of arthroscopic superior capsular reconstruction (ASCR) were investigated in this study. Continuity between the posterior remnant tissue and graft, as well as medial-to-lateral anatomical graft continuity, might play a vital role in shoulder stability and functional recovery, which could be correlated with postoperative factors such as the acromiohumeral distance (AHD).
METHODS: Thirty-two patients (36 shoulders) who underwent ASCR were included. The follow-up period was 24.8 ± 6.9 months. The relationship between graft con

BACKGROUND: Revision of unstable reverse shoulder arthroplasty (RSA) remains a significant challenge. The purpose of this study was to determine the reliability of a new treatment-guiding classification for instability after RSA, to describe the clinical outcomes of patients stabilized operatively, and to identify those with higher risk of recurrence.
METHODS: All patients undergoing revision for instability after RSA were identified at our institution. Demographic, clinical, radiographic, and intraoperative data were collected. A classification was developed using all identified causes of in

In forensic anthropology, identification begins by determining the sex, age, ancestry and stature of the individuals. Asymptomatic variations present on the skeleton, known as discrete traits, can be useful to identify individuals, or at least contribute to complete their biological profile. We decided to focus our work on the upper part of the skeleton, from the first vertebra to the pelvic girdle, and we chose to present 8 discrete traits (spina bifida occulta, butterfly vertebra, supraclavicular nerve foramen, coracoclavicular joint, os acromiale, suprascapular foramen, manubrium foramen an

BACKGROUND: The optimal treatment for symptomatic, nontraumatic rotator cuff tear is unknown. The purpose of this trial was to compare the effectiveness of physiotherapy, acromioplasty, and rotator cuff repair for this condition. We hypothesized that rotator cuff repair yields superior results compared with other treatment modalities.
METHODS: One hundred and eighty shoulders with symptomatic, nontraumatic, supraspinatus tears were randomized into one of three cumulatively designed intervention groups: the physiotherapy-only group (denoted as Group 1), the acromioplasty and physiotherapy grou

PURPOSE: We present a computer-assisted planning solution "ArthroPlanner" for acromioplasty based on 3D anatomical models, computed tomography and joint kinematic simulations.
METHODS: In addition to a standard static clinical evaluation (anamnesis, radiological examination), the software provides a dynamic assessment of the shoulder joint by computing in real time the joint kinematics from a database of activities of daily living. During motion, the precise bone resection (location and amount) is computed based on detected subacromial impingements, providing surgeons with precise information

The EuroSID-2re (ES-2re) Anthropomorphic Test Device (ATD) commonly known as the crash test dummy is also used in the military domain to assess the risk of injury of armored vehicles occupants from lateral impact. The loading conditions range from low velocity - long duration impacts (4 m/s - 50 ms) similar to the automotive domain, to high velocity - short duration impacts (28 m/s - 3 ms) corresponding to cases where the panel deforms under an explosion. The human shoulder response to lateral impact was investigated at bounds of the loading condition spectrum previously mentioned, and also at

We prospectively studied the results of distal release, performed between February 1989 and December 1994 for the treatment of a contracture of the deltoid muscle, in forty patients (forty-nine shoulders). Forty-seven contractures (in thirty-eight patients) developed after multiple intramuscular injections of various medications. The two remaining contractures (in two patients) were congenital. The average age at the onset of the symptoms was thirty-two years (range, birth to fifty-eight years). The primary symptoms included pain around the neck and the shoulder girdle, dimpling of the skin, a
