A Geometric Classification of Jaw Deformities.

In the United States, the most widely used classification system for jaw deformities is the one provided by the International Classification of Diseases, Clinical Modification (ICD-CM), a taxonomy scheme that is based on the World Health Organization's International Classification of Diseases (ICD). The last iteration of ICD-CM, version 10, sorts jaw deformities according to geometry, into 3 groups: anomalies of jaw size, anomalies of jaw-cranial base relationship, or unspecified. Yet these deformities can affect 6 different geometric attributes: size, position, orientation, shape, symmetry, a

Are distinct etiologies of upper airway obstruction in mouth-breathing children associated with different cephalometric patterns?

OBJECTIVE: To test the null hypothesis that mouth-breathing (MB) children by distinct obstructive tissues present a similar cephalometric pattern. METHODS: The sample included 226 prepubescent children (113 MB and 113 nasal breathing (NB) controls). An ENT clinical examination, including flexible nasal endoscopy, orthodontic clinical and cephalometric examinations, was performed on the MB population. MB children were grouped into three categories, according to the obstructive tissues: 1) adenoid group (AG), 2) tonsillar group (TG), and 3) adenotonsillar group (ATG). The NB controls were match

Are There Mandibular Morphological Differences in the Various Facial Asymmetry Etiologies? A Tomographic Three-Dimensional Reconstruction Study.

PURPOSE: Facial asymmetries (FAs) have been classified according to the mandibular morphological differences to obtain better diagnostic and treatment decisions. The purpose of the present study was to establish diagnostic differentiation among FAs using computed tomography (CT) and 3-dimensional (3D) reconstruction. MATERIALS AND METHODS: We performed a cross-sectional study of patients with a diagnosis of FA, who had been evaluated by CT and 3D reconstruction in the same clinical center from 2015 to 2018. The following mandibular anatomic characteristics were compared between the 2 sides (d

Surgical mandibular setback and changes in uvuloglossopharyngeal morphology and head posture: a short- and long-term cephalometric study in males.

A detailed cephalometric analysis was conducted on a sample of 31 adult males who underwent correction of mandibular prognathism by mandibular setback osteotomy (BSRO) with rigid fixation to evaluate the changes in uvuloglossopharyngeal morphology, hyoid bone position and head posture. Lateral cephalograms were obtained 1-3 days prior to the operation and at standardized 6 months and 3 years post-operative follow-up. Statistical evaluation was performed by paired Student's t-test and Pearson product moment correlation analysis. Inferior position of the hyoid bone (AH perpendicular FH, AH perpe

The use of patient-specific implants in orthognathic surgery: A series of 32 maxillary osteotomy patients.

The use of virtual surgery, patient-specific saw and drill guides, and custom-made osteosynthesis plates is rapidly spreading from deformity surgery to orthognathic surgery. Most of the commercially available systems are using computer-aided design/computer-aided manufacture (CAD/CAM) wafers to produce patient-specific saw guides. However, most plate systems provided are still the conventional "in stock" mini plates that can be individually designed by pre-bending according to the stereolithographic model of the patient. Custom made three-dimensional (3D) printed implants have earlier been dem

Unique canal communicating between both maxillary sinuses through the palatine recess with hypertrophy of sinuses.

The result of an inappropriate growth of the jaws is in many cases related to abnormalities in the rest of the facial skeleton. We present a 21-year-old patient with a possibly unique anomaly of the maxillary sinus in skeletal class III dentofacial deformity. This anatomical variant has not to our knowledge been previously reported and all clinicians should be aware of it. It presented not only as hypertrophy of the sinuses but also as a communication between the two maxillary sinuses, and might be associated with a severe III class deformity with an extremely narrow maxilla. We describe this

Changes in airway and hyoid position in response to mandibular protrusion in subjects with obstructive sleep apnoea (OSA).

This prospective clinical study examined the alterations in airway and hyoid position in response to mandibular advancement in subjects with mild and moderate obstructive sleep apnoea (OSA). Pairs of supine lateral skull radiographs were obtained for 13 female and 45 male, dentate Caucasians. In the first film, the teeth were in maximal intercuspation, while in the second the mandible was postured forwards into a position of maximum comfortable protrusion. Radiographs were traced and digitized, and the alterations in the pharyngeal airway and position of the hyoid were examined. Males and fema

Morphometry of the midfacial complex in subjects with class III malocclusions: Procrustes, Euclidean, and cephalometric analyses.

The purpose of this study was to determine whether the morphology of the midface differed in subjects with a retrognathic midfacial appearance (Class III malocclusions) using a combination of morphometric and cephalometric analyses. After obtaining appropriate consent, lateral cephalographs of 133 children of European-American descent, ages 5-11 years, were compared: 73 had Class III malocclusion, 60 had normal (Class I) occlusion. The cephalographs were traced and subdivided into seven age- and sex-matched groups. Average geometries based upon seven nodes (pterygoid point, PTS; rhinion, RO; p

[Architectural and structural craniofacial analysis (lateral view). Theoretical principles. Some example of its use in maxillofacial surgery (author's transl)].

Based upon the study of mutual equilibrium of the various bony structures of the skull and face, cranio facial architectural and structural analysis studies successively the skull (vault and base), then the face in relation to the skull and to the cranio-spinal joint. Its value lies in avoiding statistical averages rather relying upon individual measurements, influenced by the special features of each skeleton. It places the dentition within its cephalic context, thereby making it possible to demonstrate certain pathogenic factors of dento-facial dysmorphoses, not demonstrated by traditional t

Orthodontic Management of a Growing Patient With a Skeletal Class III Malocclusion and an Anterior Open Bite: A Case Report.

Class III malocclusion presents many challenges due to its varying elements of imbalance in skeletal, dental, and soft tissues. This necessitates a comprehensive treatment plan, including growth modification during the pre-pubertal growth phase, and long-term retention to reduce the chances of orthognathic surgery later. The components of Class III malocclusion include maxillary retrognathism, mandibular prognathism, or, in some cases, a combination of both. This article presents a case report of a pre-pubertal female patient, 11.5 years old, with skeletal Class III malocclusion, who sought or

Stress distribution and displacement in the maxillofacial complex during intrusion and distalization of the maxillary arch using miniplates versus mini-implants: a 3-dimensional finite element study.

OBJECTIVES: To three-dimensionally analyse the stress distribution and displacement pattern in the maxillofacial complex following intrusion and distalization of the maxillary arch using finite element analysis in skeletal class II malocclusion with prognathic maxilla and vertical maxillary excess using miniplates and mini-implants. MATERIALS AND METHODS: Finite Element models of a skull, Y-shaped stainless steel miniplate, mini-implant and a posted arch were generated. Three force levels (1) 200 g (2) 300 g and (3) 500 g per side were applied to the assembly. The models were pre-processed an

Upper Airway Changes and OSAS Risk in Patients after Mandibular Setback Surgery to Treat III Class Skeletal Malocclusion.

INTRODUCTION: Mandibular setback surgery (MSS) is one of the treatment options to resolve mandibular prognathism in patients suffering from skeletal class III malocclusion, which cannot be treated with simple orthodontic treatment. The mandibular setback surgical operation can involve changes in the pharyngeal morphology, resulting in a narrowing of the posterior airway space (PAS). This aspect is associated with an increase in airflow resistance, which increases the risk of developing snoring or obstructive sleep apnea syndrome (OSAS). The aim of this study is to evaluate the medium- and long
