Using a serosal trough for fashioning a continent catheterizable stoma: technique and outcomes.

UNLABELLED: WHAT'S KNOWN ON THE SUBJECT? AND WHAT DOES THE STUDY ADD?: Continent urinary diversion with bladder augmentation is an established method of providing urinary continence in children with bladder exstrophy, who are not suitable candidates or have a failed bladder neck reconstruction. Sub-mucosal implantation of the tubularized catheterizable stoma (usually the appendix) into the reservoir, with backing typically provided by either the bladder musculature or colonic taenia, is safe and highly effective in these children. In some cases of classic bladder exstrophy and in the majority

[Bladder stones and lithotomy: a vanished ailment as the basis of urology].

Urinary bladder stones have played a role in medicine for ages. Many examples of this disease can be found in Dutch history. The need to cure this ailment led to the development of extremely painful interventions, for which self-appointed specialists very soon came forward and were then employed in this capacity in the cities. The various interventions were: perineal lithotomy using either the 'apparatus minor' or the 'apparatus major', lateral lithotomy, cutting for stone 'in two trips', suprapubic cystotomy, and ultimately lithotripsy. Bladder stones have largely disappeared from contemporar

[Hormone disorders and HPV infection. Risk factors for tumors of the urethra?].

Overall, epithelial growths in the urethra are rare, and present as benign or malignant lesions at different periods of life; their incidence varies between the sexes. Among the benign urethral growths, condylomata in younger men, and urethral caruncles in elderly women are relatively common. In contrast, cancer of the urethra is relatively rare and shows a clear predilection for female sex (4:1). In patients with persistent urethral and urination problems, with or without macrohematuria, the rare urethral carcinoma should be included in the differential diagnostic considerations, in particula

Controversies in management of urethral trauma after pelvic fracture in men.

Whenever trauma of the urethra is suspected a "test" catheterisation is potentially bad and useless. Retrograde urethrography should be performed before use of any catheter. Doctors in urgency department should be trained to do this. Suprapubic diversion under imaging guidance is the best solution when trauma is diagnosed. In complete rupture without extreme displacement of both ends of the urethra, reconstruction is foreseen at day 7 to 10 after trauma. Bleeding is stopped at that moment and elasticity of the tissues is still sufficient. A second urethrogram the day before intervention is adv

[Kidney transplantation in patients with anomalies of the lower urinary tract].

In the transplant centre in Erlangen-Nuremberg, 1242 patients underwent renal transplantation between 1966 and 1993. In 4.5% of cases (56 patients) the renal end stage disease had been caused by severe abnormalities or functional disturbance of the lower urinary tract. Despite long-standing defunctionalization, urinary diversions and multiple operative procedures, it was possible to use the original bladder for ureterocystoneostomy in all patients. Only 1 patient needed an enterocystoplasty after transplantation. Altogether 72 transplantations have been performed in 56 patients ranging from 10

Reoperation for Channel Complications in Children With Continent Cutaneous Catheterizable Channels: The Test of Time.

OBJECTIVES: To examine the durability of continent cutaneous catheterizable urinary channels (CCCC) in children and assess whether channel complications continue to arise with extended follow-up. Previous studies demonstrated that complications of CCCC cluster in the early years following surgery. METHODS: The database of a tertiary center was queried for patients≤21 years who underwent CCCC. Patients with <6 years of follow-up were excluded. Patients were invited for follow-up to assess continence. Clinic visits and hospital admissions were reviewed for channel complications requiring reoper

Our initial experience with the technique of complete primary repair for bladder exstrophy.

OBJECTIVE: We reviewed our initial results with complete primary repair of exstrophy in regard to continence status and the need for subsequent continence procedures. PATIENTS AND METHODS: We performed a retrospective review of our surgical records from 1996 to 2008 to identify all patients with bladder exstrophy managed at our center. RESULTS: Sixteen children were closed successfully. Six patients (37.5%) experienced complications: umbilical hernias in two, transient penopubic fistula in three, and subcoronal fistula due to meatal stenosis in one. Of the 12 males, seven (58.3%) were left w

Application of the Mitrofanoff principle for intermittent self-catheterization in quadriplegic patients.

OBJECTIVES: Patients with cervical spinal cord lesions have impaired hand function, which often prohibits clean intermittent self-catheterization (CIC), especially in female patients. Enabling these patients to perform CIC gives them control of their bladder management and can improve their quality of life considerably. We have used an appendicovesicostomy to provide easy access for CIC in such patients. METHODS: Five patients (4 women, 1 man) with a mean age of 31 years (range 20 to 52) and a mean duration of the cervical cord lesion of 22 months (range 7 to 37) underwent appendicovesicostom

[History of surgical instruments. 9. Surgical instruments and development of surgical technique of lithotomy incision].

Lithotomy, i.e. the surgical method of cutting for stone in the bladder, belongs to the oldest and, due to the high risk, most notorious operating techniques. Records of stone-cuttings date as far back as Ancient Greece. The first detailed description of the procedure and instrumentation of lithotomy is to be found by Celsus (1. century AD). The patient in the lithotomy position, the neck of the bladder is approached by a median incision of the perineum. Celsus is also the first to mention special stone-hooking instruments ("uncus") to aid in extracting the bladder-stones. Medico-historical de

Infection of the pancreatic duct following pancreas transplantation with bladder drainage.

Combined kidney-pancreas transplantation represents a widely accepted therapy for endstage diabetic nephropathy. Drainage of the pancreatic juice into the urinary bladder is the preferred surgical technique in most centers. Between January 1987 and December 1994 a total of 85 pancreas transplantations with pancreatocystostomy were performed at the Innsbruck University Hospital. In all cases a polyvinylcatheter was placed into the pancreatic duct and drained transvesically to the exterior. During several weeks after transplantation pure pancreatic juice was harvested for immunological and micro

[Transperineal surgery for pelvic fracture-induced posterior urethral atresia: Selection and application of surgical modality].

OBJECTIVE: To investigate the selection of the modality of transperineal surgery and its effect in the treatment of pelvic fracture-induced posterior urethral atresia (PUA). METHODS: This study included 116 cases of PUA caused by pelvic fractures treated in our hospital from January 2012 to December 2021. We used transperineal resection of the urethral stricture and end-to-end urethral anastomosis (EEUA) as the first choice in the treatment of 79 cases. For the patients with the urethral atresia segment longer than 3 cm and evident urethral involution tension, we incised the penile septum, se

Surgical excision of acquired urethral diverticulum and single-stage urethroplasty for a 32-year-old male. Case report and literature review.

INTRODUCTION: Urethral diverticulum (UD) is a saccular dilatation of the urethral wall, continuous with the true urethral lumen. It is categorized etiologically into congenital and acquired. The etiology of an acquired urethral diverticulum is thought to be secondary to trauma. The gold standard imaging modalities for diagnosis of UD are retrograde urethrogram (RGU) and micturating cystourethrogram (MCU). Management options include: nonoperative treatment, minimally invasive and open surgeries. Open surgeries comprise a primary anastomosis or, Substitution urethroplasty after UD excision, with
