PURPOSE: Individuals who wear Ankle Foot Orthoses (AFOs) express a desire for adaptability and improved stair ambulation. This exploratory study examined how the stiffness and neutral angle of a robotic AFO influence stair ambulation and preference in spastic cerebral palsy (CP) and compared robotic AFOs, solid AFOs, and shoes alone.
METHODS: Seven individuals with spastic CP completed modified Timed-Up-and-Down-Stairs (TUDS) tests under nine robotic AFO stiffness-neutral angle configurations, solid AFOs, and shoes alone. Outcomes included TUDS time, ankle kinematics, soleus and vastus latera

BACKGROUND: We evaluated treatment of osteomyelitis in the foot in the presence of Charcot neuroarthropathy, a devastating condition with progressive degeneration and joint destruction. We hypothesized that there was a difference in (1) amputation rate, (2) amputation level, (3) duration of antibiotic therapy, and (4) duration of immobilization for treatment of osteomyelitis within versus outside the Charcot zone.
METHODS: Forty patients (43 ft) diagnosed with Charcot neuroarthropathy and osteomyelitis of the same foot were retrospectively analyzed. Some patients were successfully treated for

BACKGROUND: Diabetic peripheral neuropathy is now well recognized as the most common cause of Charcot arthropathy of the foot and ankle, but it may be associated with other peripheral neuropathies. While not well known, it is well documented that rheumatoid arthritis is correlated with peripheral neuropathy. However, despite rheumatoid neuropathy, Charcot arthropathy has never been associated with rheumatoid arthritis. We report a series of Charcot arthropathy patients with concomitant rheumatoid arthritis.
METHODS: The medical records of patients treated between 1986 and 2009 with Charcot ar

BACKGROUND: The Ponseti method for treating clubfoot consists of initial treatment with serial casting accompanied by achillotenotomy if needed, followed by the maintenance phase including treatment with a foot abduction orthosis (FAO) for at least four years. This study aimed to examine the duration, course, and outcome of orthotic treatment in children with clubfoot.
METHODS: 321 children with clubfoot, born between 2015 and 2017, registered in the Swedish Pediatric Orthopedic Quality Register (SPOQ), were included in this prospective cohort study. Data on deformity characteristics and orth

BACKGROUND: Foot abduction orthoses (FAO) are believed to decrease recurrence following treatment of congenital talipes equinovarus (CTEV) as described by Ponseti. The purpose of this project is to examine the outcomes of FAO bracing following treatment by the Ponseti method in a cohort of idiopathic CTEV patients.
METHODS: After IRB approval, a cohort of patients aged 3 to 46 days with idiopathic CTEV was identified in a previous prospective study of brace compliance by family report and sensor. Dimeglio score and family demographic information were collected. Initial treatment was by the Po

BACKGROUND: Ankle-foot orthoses (AFOs) are commonly prescribed after stroke, yet their immediate effects on bilateral hip-knee coordination and joint-specific contributors to functional gains are unclear.
RESEARCH QUESTION: What are the immediate effects of AFO use on bilateral coordination and joint-level kinematics, and which changes relate to performance gains?
METHODS: Forty-eight individuals with subacute stroke (mean age 57.9 ± 13.9 years; 28 men/20 women; FAC median 3.0) performed overground 10-m trials under pre-AFO and immediate post-AFO conditions (no acclimatization); eight had tw

BACKGROUND: We evaluated patients who were followed after acutely developing single-sided footdrop and improving with conservative management or spontaneously.
METHODS: In 2019 and 2020, ten patients were retrospectively evaluated for unilateral weakness of the lower extremity in the form of absent dorsiflexion at the ankle joint and were given a diagnosis of footdrop without etiologic cause. Patients were followed for 18 months. Patients were evaluated for acute footdrop of the affected extremity with electromyography, lumbar spine magnetic resonance imaging (MRI), knee MRI, peripheral MRI n

BACKGROUND: The use of an unloader brace is a non-surgical treatment option for patients with medial osteoarthritis (OA). However, many patients do not adhere to brace treatment, because of skin irritation due to the pads at the level of the joint space and bad fit. A new concept to unload the medial compartment of the knee is a foot ankle brace with a lever arm pressing the thigh in valgus. The aim of this prospective randomized trial was to examine the outcomes of patients with medial OA after treatment with a conventional knee unloader brace (Unloader One®) and the new foot ankle orthosis (

We present a case of fibular insufficiency (stress) fracture in a patient with pre-existing asymptomatic advanced tibialis posterior dysfunction. The patient had severe planovalgus (flat foot) deformity but no pain before the injury; hence had never sought any treatment. She was first treated conservatively for the fracture which failed to control the deformity and had ongoing symptoms. She eventually required tibio-talo-calcaneal fusion. To the best of our knowledge, this is the first case report of this condition in the English literature. In patients with severe tibialis posterior dysfuncti

BACKGROUND: Ponseti serial casting is the most commonly used method in North America to treat children with clubfeet. Despite initial correction, recurrence is common. tibialis anterior tendon transfer (TATT) is commonly used to treat recurrent clubfeet. Recurrence can occur after TATT, and patients at risk of recurrence may benefit from closer monitoring. We studied the rate of second recurrence (recurrence after TATT) and studied the predictive factors for this recurrence.
METHODS: Retrospective chart review of patients who have undergone TATT for recurrent clubfeet between 2002 and 2010 at

BACKGROUND: The study aimed at finding whether there are any nail changes specific to treatment in clubfoot.
METHODS: Sixty new, 26 undergoing serial corrective casting, and 247 clubfoot patients using foot abduction braces were prospectively studied. The casted and braced group formed the basis of the study to observe nail changes, if any. The new patients and opposite normal foot (in unilateral casted cases) were taken as controls.
OBSERVATIONS: Acute paronychia, ingrown toe nail, onychoshizia, onychorrhexis, nail plate concavity, latent onychomadesis, and distal onycholysis were observed

BACKGROUND: Emerging research increasingly supports that epidural spinal cord electrical stimulation (EES) combined with neurorehabilitation can improve motor recovery in spinal cord injury (SCI) subjects. Patients with lesions involving the medullary cone may be challenging to treat with this approach, probably due to potential peripheral nervous system damage, leaving the open question of whether this large population may benefit from EES.
METHODS: A T11-T12 SCI patient, with medullary cone involvement, underwent EES implant in a clinical trial (NCT05926843). During three months of testing,

Post-traumatic osteoarthritis (PTOA) often develops following tibial pilon fractures. Evidence suggesting PTOA development is driven by elevated articular contact stress from residual malreduction has led surgeons to strive for precise articular reduction, typically at the cost of extended operative time. Post-operative bracing using carbon fiber custom dynamic orthoses (CDOs) offers another means to decrease tibiotalar joint reaction force (JRF) and contact stress. The purpose of this cadaveric study was to measure how CDO stiffness influences ankle JRF and contact stress over the stance phas

OBJECTIVES: Perceived risk of falling is an important factor for people with spinal cord injury (SCI). This study investigated the influence of ankle joint motion on postural stability and walking in people with SCI when using an orthosis.
METHODS: Volunteer subjects with SCI (n=5) participated in this study. Each subject was fitted with an advanced reciprocating gait orthosis (ARGO) equipped with either solid or dorsiflexion-assist type ankle-foot orthosis (AFOs) and walked at their self-selected speed along a flat walkway to enable the comparison of walking speed, cadence and endurance. A robotic ankle-foot orthosis (AFO) that provides powered assistance could adjust to varying gait dynamics much better than a rigid AFO. To provide insight into how humans would adapt to a powered AFO, we studied the response of neurologically intact subjects walking with an active dorsiflexion assist orthosis proportionally controlled by tibialis anterior electromyography (EMG). We examined the two mechanical functions of ankle dorsiflexors in gait (power absorption at heel strike and power generation at toe-off) by recruiting two groups of healthy subjects: Group One, called Continuous Contr

BACKGROUND: Clinical functional tests for patellofemoral pain (PFP), such as step-up, step-down and squat, are commonly used in clinic practice, although the extent to which changes in these tests reflect changes in patient-reported outcome measures (PROMs) remains unclear.
OBJECTIVE: Investigate how changes in step-up, step-down, and squat tests are associated with changes in PROMs in individuals with PFP.
METHODS: This secondary analysis used data from a randomized controlled trial (N = 218) comparing 12-week outcomes following either foot orthoses or 4-week hip exercise program. The numbe
