[Percutaneous extraction of intravascular foreign bodies: a series of 38 cases].

BACKGROUND AND OBJECTIVES: Retained foreign bodies or embolization in cardiac chambers is a relatively common clinical problem. As experience was acquired, it became apparent that failure to remove such fragments could result in morbility or mortality. In the past, surgical removal was the only method available. We present our experience of percutaneous extractions of embolized foreign bodies. The technical aspects are discussed. PATIENTS AND METHODS: Twenty eight cases (age between 1 and 80 years) of cardiac and intravascular foreign bodies seen in two hospitals are reported. Seventeen were

[Percutaneous left atrial appendage closure after resolution of left atrial appendage thrombi with dabigatran].

The "gold standard" of the prevention of atrial fibrillation related thromboembolic events is anticoagulation therapy with oral vitamin K antagonists. A certain proportion of high-risk patients with atrial fibrillation are not receiving effective antithrombotic therapy because of problems associated with its use. Resolution of subsequent left atrial appendage thrombi is quite a great challenge in patients who are not tolerating "standard" antithrombotic drugs. According to the knowledge of the authors, this is the first report of a patient with non-valvular persistent atrial fibrillation and h

Disparities in Sex-Based Health Outcomes of Endovascular Intervention for the Treatment of Symptomatic Lower Extremity Peripheral Artery Disease: A Scoping Review.

Peripheral artery disease (PAD) is a chronic vascular condition that affects millions of individuals worldwide and is a significant burden on public health. Although female subjects may be at greater risk of developing PAD owing to sex-specific risk factors and frequently present with atypical symptoms, diagnosis rates are comparable between women and men. Treatment of PAD commonly relies on endovascular interventions, including angioplasty, stenting, and atherectomy. While these interventions have proven effective, understanding sex-specific differences in outcomes has been insufficiently stu

Predictors of Patency Following Infrainguinal Arterial Interventions Under Intravascular Ultrasound Guidance: Analysis from the iDissection Studies.

BACKGROUND: Intravascular Ultrasound (IVUS) has been shown in smaller studies to improve outcomes when used to guide infrainguinal peripheral arterial interventions. The iDissection series of studies were conducted to determine the presence of dissections with various prepping devices in femoropopliteal and infrapopliteal arteries. The impact of IVUS-directed treatment on the long-term outcomes in these patients remains unclear. METHODS: All patients were enrolled at a single center with the exception of the Auryon BTK study (at 4 US centers). Seven prospective iDissection studies had been pr

Safety and feasibility of transradial infrainguinal peripheral arterial disease interventions.

OBJECTIVE: Transradial access (TRA) has traditionally been favored for coronary interventions. Tools with up to 200 cm length now allow operators to treat infrainguinal peripheral arterial disease (PAD) using TRA. This study aims to assess the safety and feasibility of TRA infrainguinal interventions. METHODS: Patients with infrainguinal PAD who underwent intervention via TRA from July 2013 through June 2019 were retrospectively reviewed. Exclusion criteria included Barbeau D waveform, a radial artery diameter of greater than 2 mm, radial artery occlusion, Raynaud syndrome, or peripheral vasc

Activated endocannabinoid system in coronary artery disease and antiinflammatory effects of cannabinoid 1 receptor blockade on macrophages.

BACKGROUND: Cannabinoid 1 (CB1) receptor blockade with rimonabant represents a clinical therapeutic strategy for obesity. Recently, the role of the endocannabinoid system has been described in peripheral organs. We sought to determine whether the endocannabinoid system could be involved in human atherosclerosis and whether CB1 receptor blockade could modulate proinflammatory activity in macrophages. METHODS AND RESULTS: mRNA expression levels of CB1 receptor in coronary atherectomy samples were significantly higher in patients with unstable angina than in those with stable angina (3.62+/-2.96

Percutaneous intervention for infrainguinal occlusive disease in women: equivalent outcomes despite increased severity of disease compared with men.

OBJECTIVES: Experience with open surgical bypass suggests similar overall outcomes in women compared with men, but significantly increased risk of wound complications. Percutaneous treatment of lower extremity occlusive disease is therefore an attractive alternative in women, although it is not clear whether there is a difference in outcomes between women and men treated with this technique. We sought to determine the results and predictors of failure in women treated by percutaneous intervention. METHODS: Percutaneous infrainguinal revascularization was performed on 309 women between 2001 an

Initial experience with the Endicor X-sizer thrombectomy device in patients with ST segment elevation myocardial infarction.

We investigated 16 patients with ST segment elevation myocardial infarction who had an occluded coronary artery (TIMI 0) at initial angiogram. Instead of balloon angioplasty and stenting, patients were subjected to thrombectomy (Endicor X-sizer) and stenting. In 15/16 patients the occlusion could be crossed by the thrombectomy device resulting in TIMI flow 3 in all of them. Thereafter, stenting was performed. At final angiogram all 15 patients continued to show TIMI flow grade 3. Twelve-lead ECG was performed prior to and post-intervention. ST elevation was measured as the sum of eight leads f

Long-term clinical outcome and predictors of major adverse cardiac events after percutaneous interventions on saphenous vein grafts.

OBJECTIVES: The purpose of this study was to examine the long-term clinical outcome after percutaneous intervention of saphenous vein grafts (SVG) and to identify the predictors of major adverse cardiac events (MACE). BACKGROUND: Percutaneous interventions of SVGs have been associated with more procedural complications and higher restenosis rates compared with interventions on native vessels. METHODS: From 1993 to 1997, 1,062 patients underwent percutaneous intervention on 1,142 SVG lesions. Procedural, in-hospital and long-term clinical outcomes were recorded in a database and analyzed. Revascularization for claudication: Changing the natural history of a benign disease!

OBJECTIVE: The benign natural history of intermittent claudication was first documented in 1960 and has been reconfirmed in several subsequent studies. Excellent outcomes in patients with intermittent claudication can be achieved with exercise therapy and optimal medical management. Professional society guidelines have clearly stated that revascularization procedures should be performed only in patients with incapacitating claudication who have failed conservative therapy. Despite these guidelines, revascularization procedures, primarily percutaneous interventions, have been increasingly utili

Treatment of calcified coronary lesions with intravascular lithotripsy: experience of a regional French centre.

INTRODUCTION: Coronary intravascular lithotripsy (CIVL) has emerged as a promising technique for modifying severe coronary calcification before stent deployment, yet real-world data are still limited. The present study aimed to evaluate the indications, procedural modalities, and clinical outcomes of CIVL in consecutive real-world patients. PATIENTS AND METHODS: We conducted a retrospective analysis of all patients in whom CIVL was attempted between April 2022 and December 2023 at our centre, representing our initial experience with this technique. Demographic, clinical, and procedural data w

Open versus Endovascular Revascularisation for Femoropopliteal Disease in Patients with Chronic Limb Threatening Ischaemia.

OBJECTIVE: Multiple options exist for the treatment of femoropopliteal (FP) disease, including endovascular interventions (ENDOs) and bypass surgery (OPEN). There are limited data directly comparing ENDO and OPEN strategies for FP disease in chronic limb threatening ischaemia (CLTI). METHODS: Outcomes of patients in the Best Endovascular vs. Best Surgical Therapy in Patients with CLTI (BEST-CLI) trial who underwent revascularisation for FP disease were examined. ENDO and OPEN were compared using single segment great saphenous vein (SSGSV) or alternative conduits (ACs). The primary outcome was

Indications and Treatment Outcomes of Below-the-Knee Peripheral Artery Interventions in the XLPAD Registry.

There are unresolved questions regarding indications and outcomes of endovascular below-the-knee (BTK) interventions in patients with symptomatic peripheral artery disease (PAD) in real-world clinical practice. We analyzed 884 patients from the multicenter XLPAD registry between 2006 and 2023 with nonstent BTK PAD interventions. Primary outcome: freedom from major adverse limb events (MALE) at 1 year, a composite of all-cause death, major amputation, or clinically driven revascularization. Majority (62.8%) of the BTK interventions were performed for chronic limb threatening ischemia (CLTI), wh

Single-Cell RNA Sequencing Reveals an Immune Landscape of CD4+ T Cells in Coronary Culprit Plaques With Acute Coronary Syndrome in Humans-Brief Report.

BACKGROUND: Acute coronary syndrome (ACS) involves plaque-related thrombosis, causing primary ischemic cardiomyopathy or lethal arrhythmia. We previously demonstrated a unique immune landscape of myeloid cells in the culprit plaques causing ACS by using single-cell RNA sequencing. Here, we aimed to characterize T cells in a single-cell level, assess clonal expansion of T cells, and find a therapeutic target to prevent ACS. METHODS: We obtained the culprit lesion plaques from 4 patients with chronic coronary syndrome (chronic coronary syndrome plaques) and the culprit lesion plaques from 3 pat
