Postoperative epidural analgesia versus systemic analgesia for thoraco-lumbar spine surgery in children.

BACKGROUND: Spine surgery may be associated with severe acute postoperative pain. Compared with systemic analgesia alone, epidural analgesia may offer better pain control. However, epidural analgesia has sometimes been associated with rare but serious complications. Therefore, it is critical to quantify the real benefits of epidural analgesia over other modes of pain treatment. OBJECTIVES: To assess the effectiveness and safety of epidural analgesia compared with systemic analgesia for acute postoperative pain control after thoraco-lumbar spine surgery in children. SEARCH METHODS: We searche

Reverse-engineering placebo analgesia.

Placebo analgesia is a widely observed clinical phenomenon. Establishing a robust mouse model of placebo analgesia is needed for careful dissection of the underpinning circuit mechanisms. However, previous studies failed to observe consistent placebo effects in rodent models of chronic pain. We wondered whether strong placebo analgesia can be reverse engineered using general-anesthesia-activated neurons in the central amygdala (CeAGA) that can potently suppress pain. Indeed, in both acute and chronic pain models, pairing a context with CeAGA-mediated pain relief produced robust context-depende

Rethinking Thoracotomy Analgesia: Paravertebral Block Versus Thoracic Epidural Analgesia in Real-World Practice.

Background Open thoracotomy causes severe dynamic pain that compromises ventilation and cough, with downstream risk of pulmonary complications and delayed recovery. Enhanced Recovery After Surgery (ERAS) pathways emphasize multimodal, opioid-sparing analgesia, but the relative real-world performance of thoracic paravertebral block (PVB) versus thoracic epidural analgesia (TEA) in open thoracotomy remains uncertain and requires appraisal of both effectiveness and safety within ERAS priorities. Objective This study aims to compare the analgesic effectiveness and safety of PVB versus TEA after op

Patient-controlled epidural analgesia versus continuous infusion for labour analgesia: a meta-analysis.

BACKGROUND: Patient-controlled epidural analgesia (PCEA) is a relatively new method of maintaining labour analgesia. There have been many studies performed that have compared the efficacy of PCEA with continuous epidural infusion (CEI). The purpose of this systematic review is to compare the efficacy and safety of PCEA and CEI. METHODS: All randomized controlled trials that compared PCEA, without background infusion, with CEI were sought from the literature. These were rated for quality using a validated, five-point scale. The primary outcome was the number of patients who received anaestheti

[Continuous peridural analgesia vs patient - controlled intravenous analgesia for pain therapy after thoracotomy].

OBJECTIVES: Continuous epidural analgesia (EA) and patient-controlled intravenous analgesia (PCA) are widely used for postoperative pain control. Studies indicate that both analgesic regimens provide good analgesia after major surgery. However, because of the following reasons it is still unclear whether one of the two modes of application is superior. First, there are conflicting data regarding the differences in pain relief and drug use between epidural and intravenous administration of opioids. Second, in many studies epidural analgesia is performed by a combination of local anaesthetics an

Epidural Analgesia in a Parturient for Labor Analgesia After Surgical Correction of Spina Bifida.

This case report details the successful management of labor pain using epidural analgesia in a 22-year-old primigravid patient with a history of spina bifida and extensive surgical repair. Spina bifida, a congenital neural tube defect, presents unique challenges for neuraxial anesthesia due to altered spinal anatomy and potential neurological deficits. Despite these complexities, effective pain relief was achieved through careful patient counseling, comprehensive pre-procedural planning, and meticulous technique. Upon admission for labor induction due to preeclampsia, a physical examination re

Analgesia from morphine and ketamine. A comparison of infusions of morphine and ketamine for postoperative analgesia.

Ketamine 4 micrograms/kg/minute produced pain relief similar to that from morphine 33 micrograms/minute in a double-blind study that compared analgesia from constant-rate intravenous infusions of the two drugs in 60 patients. The analgesic efficacy of the infusions, as assessed by pain scores and the requirement for supplementary self-administered morphine, was poor. Ventilatory depression, the most significant side effect, occurred only in patients who received morphine infusion. The low dose ketamine infusion did not provide clinically useful analgesia even though adequate plasma concentrati

Spinal analgesia with continuous local wound infusion vs thoracic epidural analgesia after open pancreaticoduodenectomy.

BACKGROUND: The current gold standard for postoperative analgesia following a pancreaticoduodenectomy (PD) is a thoracic epidural analgesia (TEA). Spinal analgesia with continuous wound infusion (CWI) of local anaesthetic is an emerging alternative modality. This non-inferiority study aimed to compare CWI with spinal analgesia to TEA and assess its impact on clinical outcomes. METHODS: A retrospective observational analysis of patients undergoing open pancreatoduodenectomy through a midline laparotomy. A total of 74 patients were included in the study forming two groups: CWI (n = 33) and TEA

From Patient-Controlled Analgesia to Artificial Intelligence-Assisted Patient-Controlled Analgesia: Practices and Perspectives.

Pain relief is a major concern for patients who have undergone surgery, and it is an eternal pursuit for anesthesiologists. However, postoperative pain management is far from satisfactory, though the past decades have witnessed great progress in the development of novel analgesics and analgesic techniques. A Cochrane systematic review showed that patient-controlled analgesia (PCA) achieved better pain relief and greater patient satisfaction than traditional "on-demand" parenteral analgesia, suggesting that it might be the manner of analgesia implementation that matters for effective postoperat

Patient-controlled intravenous analgesia remifentanil for labor analgesia: time to stop, think and reconsider.

PURPOSE OF REVIEW: Remifentanil is a relatively new but commonly used opioid alternative in the management of labor pain relief. The present article evaluates efficacy and safety of patient-controlled intravenous analgesia (PCIA) with remifentanil in the management of labor pain relief. RECENT FINDINGS: Remifentanil PCIA provides analgesia that is superior to nitrous oxide or other opioids such as meperidine, but is less effective when compared with epidural analgesia. Recent evidence suggests it produces significant respiratory depression making routine use as first line analgesia strategy v

Is multimodal analgesia as effective as postoperative patient-controlled analgesia following upper extremity surgery?

INTRODUCTION: The present prospective study compared the clinical outcomes between a multimodal analgesia group and a patient-controlled analgesia (PCA) group for postoperative pain control in upper extremities surgery. HYPOTHESIS: Multimodal analgesia including pre-emptive analgesic can provide similar or superior analgesic effects and a lower incidence of adverse reactions than PCA following upper extremity surgery. PATIENTS AND METHODS: Sixty-one patients undergoing upper extremity surgery were randomized to 2 perioperative analgesic groups (multimodal analgesia and PCA). We compared the

[Obstetric analgesia using nitralgin inhalation and lumbal peridural anesthesia (a model for obstetric analgesia)].

Authors report the systems of anesthesia used at their ward for delivery in the last eight years. Nitralgin inhalation is used since 1981, lumbar peridural anesthesia is used since 1983. Of 13,458 deliveries in case of 3893 parturiants Nitralgin analgesia was used, while LEDA was used for 2300 parturients. By means of Nitralgin inhalation system it could be assured in the delivery room to apply the pain relief gas mixture (Nitralgin) simultaneously for several parturients. With direction of anesthesiologists employed for the ward the LEDA was attained by gynecologists knowing the use and appli

Opioid-Sparing Multimodal Analgesia versus Standard Analgesia in Hip Fracture Surgery: A Prospective Study.

INTRODUCTION: Hip fracture surgery is frequently associated with severe post-operative pain, traditionally managed using opioid-based regimens. In elderly patients, opioids are linked to adverse effects such as delirium, respiratory depression, and delayed mobilization. Opioid-sparing multimodal analgesia, incorporating non-opioid medications and regional anesthesia techniques, has emerged as a promising alternative. This study aimed to compare the efficacy and safety of an opioid-sparing multimodal analgesia regimen with standard opioid-based analgesia in patients undergoing hip fracture surg

Patient controlled analgesia with remifentanil versus epidural analgesia in labour: randomised multicentre equivalence trial.

OBJECTIVE: To determine women's satisfaction with pain relief using patient controlled analgesia with remifentanil compared with epidural analgesia during labour. DESIGN: Multicentre randomised controlled equivalence trial. SETTING: 15 hospitals in the Netherlands. PARTICIPANTS: Women with an intermediate to high obstetric risk with an intention to deliver vaginally. To exclude a clinically relevant difference in satisfaction with pain relief of more than 10%, we needed to include 1136 women. Because of missing values for satisfaction this number was increased to 1400 before any analysis. W
