Effects of bed rest and immobilization on intramuscular and intermuscular adipose tissue: A systematic review.

Muscle disuse caused by bed rest and immobilization is associated with muscle atrophy and insulin resistance, which might be related to increases in intramuscular fat (IntraMAT) and intermuscular fat (InterMAT) accumulation. In this systematic review, we compiled evidence on the effects of bed rest and unilateral lower-limb immobilization on IntraMAT and InterMAT in healthy adults. Following PRISMA guidelines, we searched PubMed, Scopus, Web of Science, and Cochrane Library from inception until July 2025. Included studies were clinical trials involving healthy adults (≥18 years old) undergoing

Apical hypercontractility mitigates impaired diastolic filling and lower intraventricular haemodynamic forces in human bed rest.

Prolonged physical inactivity alters the cardiovascular system, including the heart. Long-term bed rest is known to decrease left ventricular volume and cause diastolic dysfunction; however, the interplay between these changes and their effect on cardiac contractility have been understudied. Here we used novel non-invasive imaging techniques to longitudinally assess changes in regional cardiac contractility and function that can occur prior to the development of overt cardiac changes. Specifically, we performed cardiac magnetic resonance imaging on 24 healthy participants before, during, and a

Effects of 15-Day -6° Head-Down Bed Rest and HD-tDCS on Cognitive Functions.

A favorable cognitive state, efficient attention, and sharp alertness are essential for successful space missions. To explore the effects of microgravity on working memory, attention, and alertness and to find effective approaches to improve them, we recruited 17 healthy participants (control: 9, active: 8) to be involved in a 15-day -6° head-down bed rest (HDBR). A 30 min HD-tDCS was applied to the active group every other day during HDBR. We adopted three types of tasks: N-back, SCWT, and PVT to evaluate the effects, which were completed before, during, and after HDBR. The results showed tha

Peripheral nerve adaptations to 10 days of horizontal bed rest in healthy young adult males.

Space analogs, such as bed rest, are used to reproduce microgravity-induced morphological and physiological changes and can be used as clinical models of prolonged inactivity. Nevertheless, nonuniform decreases in muscle mass and function have been frequently reported, and peripheral nerve adaptations have been poorly studied, although some of these mechanisms may be explained. Ten young healthy males (18-33 yr) underwent 10 days of horizontal bed rest. Peripheral neurophysiological assessments were performed bilaterally for the dominant (DL) and nondominant upper and lower limbs (N-DL) on the

Effect of Chinese herbal medicine on vascular functions during 60-day head-down bed rest.

PURPOSE: Chinese herbal medicine is a promising countermeasure against cardiovascular dysfunction associated with a sedentary lifestyle. We examined the impact of the Chinese herb, Taikong Yangxin, on the micro- and macrovascular dysfunction associated with a 60-day bed rest. METHODS: Fourteen healthy men were randomly divided into two groups: those given herbal supplement, and the control group; the two groups underwent a 60-day bed rest. The macrovasculature was assessed by sonography. Skin microvascular functions were assessed with laser Doppler. The plasma level of endothelial micropartic

Resistive vibration exercise retards bone loss in weight-bearing skeletons during 60 days bed rest.

UNLABELLED: Countermeasures are desirable to retard bone loss during long-term space flight. We evaluated the effect of an intervention protocol on bed rest-induced bone loss. INTRODUCTION: We developed a resistive vibration exercise (RVE) platform to test if an intervention RVE protocol would be effective to protect bed rest-induced bone loss. METHODS: Fourteen male subjects were assigned randomly to either the RVE group (n = 7) that performed daily supervised resistive vibration exercise or to the no any exercise control (CON) group (n = 7). Both dual-energy X-ray absorptiometry and periph

Effect of prolonged standardized bed rest on cystatin C and other markers of cardiovascular risk.

BACKGROUND: Sedentary lifestyle is associated with coronary artery disease but even shorter periods of physical inactivity may increase cardiovascular risk. Cystatin C is independently associated with cardiovascular disease and our objective was to investigate the relation between this novel biomarker and standardized bed rest. Research of immobilization physiology in humans is challenging because good biological models are in short supply. From the Women International Space simulation for Exploration study (WISE) we studied markers of atherosclerosis and kidney function, including cystatin C,

Effects of vibration training on bone metabolism: results from a short-term bed rest study.

The absence of mechanical loading leads to a prompt increase in bone resorption measured by bone resorption markers. There is high potential that vibration training can positively influence bone metabolism in immobilized subjects, reduce the increase in osteoclastic activity and increase bone formation processes. We investigated whether vibration training at 20 Hz with an amplitude of 2-4 mm influences bone metabolism during immobilization. Eight male subjects (26.4 ± 4.9 years; 78.1 ± 9.5 kg) performed a 14 day bed rest in 6°-head down tilt (HDT). Subjects received vibration training for 2 ×

Resistive simulated weightbearing exercise with whole body vibration reduces lumbar spine deconditioning in bed-rest.

STUDY DESIGN: Randomized controlled trial. OBJECTIVE: Determine the effectiveness a resistive exercise countermeasure with whole-body vibration in relation to lumbo-pelvic muscle and spinal morphology changes during simulated spaceflight (bed-rest). SUMMARY OF BACKGROUND DATA: Spinal lengthening, flattening of the spinal curves, increases in disc size, and muscle atrophy are commonly seen in spaceflight simulation. This may represent a risk for low back injury. Consideration of exercise countermeasures against these changes is critical for success of long-term spaceflight missions. METHODS:

Arterial baroreflex control of sympathetic vasoconstrictor traffic and orthostatic intolerance after head-down bed rest.

The purpose of the present study is to examine the changes in the arterial baroreflex control of muscle sympathetic nerve activity (MSNA) after head-down bed rest (HDBR), in relation to orthostatic hypotension after HDBR. Therefore, we performed 60 degrees head-up tilt (HUT) tests before and after 14 days of HDBR, with monitoring MSNA, heart rate and blood pressure. We calculated the gain of the arterial baroreflex control of MSNA, and compared the gains between the subjects who did (defined as the fainters) and those who did not (defined as the nonfainters) become presyncopal in HUT tests aft

Orthostatic intolerance during a 13-day bed rest does not result from increased leg compliance.

Increased leg compliance (LC) has been proposed as a mechanism for orthostatic intolerance after spaceflight or bed rest. Using venous occlusion plethysmography with mercury-in-Silastic strain gauge, we evaluated LC before, during, and after a 13-day head-down (-6 degrees) bed rest in 10 men. LC was measured by the relationship between the increased calf areas (in cm2) at thigh cuff occlusions of 20, 30, 50, 70, and 80 mmHg. Orthostatic tolerance was evaluated by a presyncopal-limited lower body negative pressure test (PSL-LBNP) before and after bed rest. The 10 subjects were divided into TOL

Psychological and supportive needs during postoperative bed rest following free flap reconstruction of the lower extremity.

BACKGROUND: Postoperative bed rest following lower extremity free flap reconstruction represents a standard of care required to ensure microsurgical success and it also poses substantial physical and psychological challenges to the patient. We investigated patients' subjective experiences, unmet needs, and support preferences during immobilization. METHODS: A cross-sectional survey was conducted among 93 patients who underwent lower limb free flap surgery. A questionnaire was used to assess pain, emotional symptoms, perceived support, and desired interventions. Descriptive and inferential sta

Simulated weightlessness procedure, head-down bed rest has reversible effects on the metabolism of rhesus macaque.

It is a consensus in the international manned space field that factors such as microgravity during the space flight can cause anxiety, depression and other important brain function abnormalities in astronauts. However, the neural mechanism at the molecular level is still unclear. Due to the limitations of research conditions, studies of biological changes in the primate brain have been comparatively few. We took advantage of -6° head-down bed rest (HDBR), one of the most implemented space analogues on the ground, to investigate the effects of simulated weightlessness on non-human primate brain

Optic Disc Edema and Choroidal Engorgement in Astronauts During Spaceflight and Individuals Exposed to Bed Rest.

IMPORTANCE: Optic disc edema develops in astronauts during long-duration spaceflight and is a risk for all future astronauts during spaceflight. Having a ground-based analogue of weightlessness that reproduces critical features of spaceflight-associated neuro-ocular syndrome will facilitate understanding, preventing, and/or treating this syndrome. OBJECTIVE: To determine whether the ocular changes in individuals exposed to an analogue of weightlessness are similar to the ocular changes in astronauts exposed to a duration of spaceflight comparable to this analogue of weightlessness. DESIGN, S

Cardiac adaptations to 60 day head-down-tilt bed rest deconditioning. Findings from the AGBRESA study.

AIMS: Reduced physical activity increases the risk of heart failure; however, non-invasive methodologies detecting subclinical changes in myocardial function are not available. We hypothesized that myocardial, left ventricular, systolic strain measurements could capture subtle abnormalities in myocardial function secondary to physical inactivity. METHODS AND RESULTS: In the AGBRESA study, which assessed artificial gravity through centrifugation as potential countermeasure for space travel, 24 healthy persons (eight women) were submitted to 60 day strict -6° head-down-tilt bed rest. The effect of 8 days of strict bed rest on the incretin effect in healthy volunteers.

Bed rest and physical inactivity are the consequences of hospital admission for many patients. Physical inactivity induces changes in glucose metabolism, but its effect on the incretin effect, which is reduced in, e.g., Type 2 diabetes, is unknown. To investigate how 8 days of strict bed rest affects the incretin effect, 10 healthy nonobese male volunteers underwent 8 days of strict bed rest. Before and after the intervention, all volunteers underwent an oral glucose tolerance test (OGTT) followed by an intravenous glucose infusion (IVGI) on the following day to mimic the blood glucose profile
