Favorable neuromuscular and cardiovascular responses to 7 days of exercise with an eccentric overload in elderly women.

The metabolic, cardiovascular, and neural cost of eccentric muscle contraction is less than that of concentric contraction, but the strength and neural adaptations in eccentric contractions are significantly greater following resistive exercise. We thus compared the short-term effects of exercise with an eccentric overload (n = 10) with those of exercise with a standard load distribution (n = 10) in ostensibly healthy sedentary elderly women (mean age 71.4). Subjects were tested for concentric and eccentric three-repetition maximum, maximal isokinetic eccentric and concentric and isometric for

Sex-based differences in central and cerebral hemodynamic responses to lower body negative pressure (LBNP): an experimental study.

PURPOSE: The effectiveness of lower body negative pressure (LBNP) as an in-flight countermeasure against cardiovascular deconditioning is still debated, with significant gaps in understanding cerebral hemodynamics and sex-related differences. This study investigates acute central, peripheral, and cerebral responses to graded LBNP during exposure to -6° head-down tilt posture (HDT) in 30 subjects (15 females, 15 males). METHODS: Central and peripheral hemodynamic variables (heart rate, blood pressure, cardiac output, and total peripheral resistance) were continuously and non-invasively measure

Jumping on the moon as a potential exercise countermeasure.

The Moon's gravitational field strength (17% Earth's gravity) may facilitate the use of bodyweight jumping as an exercise countermeasure against musculoskeletal and cardiovascular deconditioning in reduced gravity settings. The present study characterised the acute physiological and kinetic responses to bodyweight jumping in simulated Lunar gravity. Nineteen healthy adults (age: 25 ± 7 years, weight: 73 ± 11 kg; height: 1.81 ± 0.05 m, V ̇ O 2 max : 50 ± 11 mL kg-1 min-1) performed an incremental jumping test in simulated Lunar gravity (9.5° head-up tilt suspension) comprising 4-min stages o

Evaluation of unexplained dyspnea in a young athletic male with pectus excavatum.

Pectus excavatum (PE) is a relatively common congenital deformity of the anterior chest wall associated with reduced exercise capacity. Uncertainty exists over the nature of physiologic impairment in PE. Evidence suggests that myocardial compression exerted by the displaced sternum on the right heart chambers, disables the ability of the heart to augment stroke volume during exercise. This case study describes the evaluation of an athletic 20 year old Caucasian male, lifelong non-smoker, with severe pectus deformity and previous fixation procedure to repair a sternal fracture. The patient perf

Cardiorespiratory fitness in children with surgically corrected congenital heart disease: Exercise testing profiles and biventricular function.

AIMS: Children with surgically corrected congenital heart disease (CHD) often exhibit reduced cardiorespiratory fitness, reflected by impaired maximal oxygen uptake (̇VO₂max). Although exercise limitation is multifactorial, the relative contributions of cardiovascular, ventilatory, and gas exchange abnormalities remain unclear. This study aims to characterize exercise impairments in children with CHD using cardiopulmonary exercise testing (CPET) and cardiac imaging. METHODS: Children with surgically corrected biventricular CHD were prospectively recruited. CPET parameters were expressed as pe

Is heart rate in post-hematopoietic stem cell transplant patients clinically relevant?

Patients with hematological malignancies undergoing hematopoietic stem cell transplantation (HSCT) have become a critical treatment option in the treatment of many hematological malignancies, myeloproliferative disorders, and some solid tumors.Although HSCT has the potential of offering either a cure or minimizing disease burden while improving overall survival, HSCT is associated with some morbidity and mortality, particularly hypertension, diabetes, dyslipidemia, and renal disease with an increased cumulative incidence of cardiovascular (CVD) complications.Aside from the usual heart failure

Sex-specific cardiometabolic responses during microgravity simulation in European Space Agency VIVALDI dry immersion.

BACKGROUND: Exposure to microgravity is marked by enhanced physical inactivity, reduced gravitational loading and fluid shifts. These characteristics provide a unique model for medical research. Dry immersion is used as a ground-based model to simulate and investigate some of the physiological effects of microgravity experienced by astronauts (body fluid shifts, lack of mechanical support, unloading of the musculoskeletal system and hypokinesia). To date, there is limited knowledge about the effect of sex on cardiovascular and metabolic responses to dry immersion. METHODS: This study integrat

Echocardiographic evaluation of posterior left ventricular wall motion in muscular dystrophy.

Maximal systolic endocardial velocity (SEVM) and maximal diastolic endocardial velocity (DEVM) were determined echocardiographically in patients with muscular dystrophy (MD). The SEVM of the muscular dystrophy patients was 5.5 +/- 0.9 cm/sec and the DEVM was 13 +/- 3 cm/sec. The SEVM in MD was significantly less than that seen in age-matched normals (P less than 0.05), persons with myotonia congenita (P less than 0.02), deconditioned patients (P less than 0.001), or older normal persons (P less than 0.05). The Sevm of the MD patients was not significantly different from persons with spinal mus

Light at the end of the tunnel? Follow-up of cardiopulmonary function in children with post-COVID-19.

UNLABELLED: Few studies have examined post-COVID-19 sequelae in children, particularly regarding cardiopulmonary capacity. Longitudinal data are especially scarce. This study aimed to retest pediatric patients previously assessed in a cross-sectional design. In this longitudinal study, children meeting post-COVID-19 criteria and an age- and sex-matched control group underwent cardiopulmonary exercise testing at baseline and after 6 months. Thirteen of 20 post-COVID-19 children (mean age: 13.6 ± 2.6 years, 48% female) and 23 of 28 controls (mean age: 11.9 ± 3.1 years, 62% female) completed foll

Probing heart rate and blood pressure control mechanisms during graded levels of lower body negative pressure (LBNP).

Following space flight or head down tilt bed rest, cardiovascular deconditioning is often observed as a failure to maintain arterial blood pressure with symptoms of presyncope or syncope. LBNP can be used as a stressor of the cardiovascular system to observe the regulatory process. We have recently developed a new method to study cardiovascular control. Coarse graining spectral analysis (CGSA), allows simultaneous extraction of the harmonic components to evaluate sympathetic and parasympathetic nervous activities, and of the underlying complexity of the physiological response as given by the s

Chronic Resistance Exercise Improves Functioning and Reduces Toll-Like Receptor Signaling in Elderly Patients With Postoperative Deconditioning.

OBJECTIVE: Elderly patients continue to experience low levels of mobility during and following postoperative hospitalization that lead to persistent physical decline. Therefore, here we compared chronic resistance (CR) exercise against chronic aerobic (CA) exercise in ameliorating postoperative functioning and reducing proinflammatory muscular Toll-like receptor (TLR)-associated signaling in elderly postoperative patients. METHODS: We conducted a prospective, randomized trial comparing the effects of 3 exercise programs (CR, CA, and CR + CA) in 66 elderly patients recovering from recent hip,

Reduced spontaneous baroreflex response slope during lower body negative pressure after 28 days of head-down bed rest.

Effects of 28 days of continuous 6 degrees head-down tilt bed rest on spontaneous vagally mediated baroreflex response slope were evaluated from beat-by-beat relationships between R-R interval and systolic arterial blood pressure. Twelve healthy men (age 27-42 yr) were assigned to either countermeasure (CM) or no-countermeasure (no-CM) groups. CM consisted of strenuous short-term exercise once per day 6 days/wk from days 7 to 28 and lower body negative pressure (LBNP) for 15 min on days 16, 18, 20, and 22-28. Spontaneous baroreflex slope was evaluated by application of linear regression to seq

Biomedical results of the Skylab Program.

Skylab, the fourth in a logical sequence of USA manned space flight projects following Mercury, Gemini and Apollo, presented life scientists with their first opportunity for an in-depth study of man's response to the space environment. Extensive medical investigations were undertaken to increase our understanding of man's adaptation to the space environment and his readaptation to gravity upon return to earth. The flight durations of the three Skylab missions were progressively increased from 28 days to 59 days and, finally, 84 days. The results of these investigations of the various body syst

Cardiovascular Exercise Physiology Under Hypoxia, Microgravity, and Heat Stress: A Review with Public Health Implications.

Aerobic exercise capacity, best quantified by maximal oxygen uptake (VO2max), varies between individuals and is dependent on cardiac output (CO) and oxygen uptake in the periphery (a-vO2 diff). Environmental stressors like hypoxia, microgravity, and heat negatively impact these parameters, thereby reducing aerobic exercise capacity. However, in response to acute and chronic exposures to these environments, compensatory processes serve to counteract reductions in VO2max. In hypoxic environments, reduced oxygen partial pressure (PO2) leads to hypoxic pulmonary vasoconstriction (HPV) and a diffus

Systematic Brain Natriuretic Peptide Measurement for Cardiac Screening in Patients with Severe Obesity: The OLECOEUR Study.

INTRODUCTION: Heart failure (HF) is difficult to diagnose in obese patients because of cardiovascular and pulmonary comorbidities associated with physical deconditioning, all of which lead to dyspnea. METHODS: The OLECOEUR study is a prospective screening for HF using systematic brain natriuretic peptide (BNP) measurement in ambulatory patients with obesity from a department of Nutrition (Paris, France). Clinical, biological, and echocardiographic data were extracted from electronic medical records. RESULTS: We included 1,506 patients middle-aged (mean age: 47.2 ± 14.6 years old) with severe

Mobilization of intensive care cardiac surgery patients on mechanical circulatory support.

Historically, intensive care cardiac surgery patients remained on bed rest for several days postoperatively to prevent complications and promote rest and healing. Over time, the cardiac surgery discipline has acknowledged the benefits of early mobility, including reduced risk of venous thromboembolism and pulmonary emboli, improved pulmonary toilet, prevention of pneumonia, decreased length of stay, reduced deconditioning, and need for rehabilitation, among others advantages. These benefits have changed clinical practice, with emphasis on early extubation, progressive mobility, and reduced len

Discriminating Circulatory Problems From Deconditioning: Echocardiographic and Cardiopulmonary Exercise Test Analysis.

BACKGROUND: Discriminating circulatory problems with reduced stroke volume (SV) from deconditioning, in which the muscles cannot consume oxygen normally, by gas exchange parameters is difficult. METHODS: We performed combined stress echocardiography (SE) and cardiopulmonary exercise tests (CPET) in 110 patients (20 with normal effort capacity, 54 with attenuated SV response, and 36 with deconditioning) to evaluate multiple hemodynamic parameters and oxygen content difference (A-V.o2 Diff) in four predefined activity levels to assess which of the gas measures may help in the discrimination. Vascular dysfunctions following spinal cord injury.

The aim of this article is to analyze the vascular dysfunctions occurring after spinal cord injury (SCI). Vascular dysfunctions are common complications of SCI. Cardiovascular disturbances are the leading causes of morbidity and mortality in both acute and chronic stages of SCI. Neuroanatomy and physiology of autonomic nervous system, sympathetic and parasympathetic, is reviewed. SCI implies disruption of descendent pathways from central centers to spinal sympathetic neurons, originating in intermediolateral nuclei of T1-L2 cord segments. Loss of supraspinal control over sympathetic nervous sy
