Catheter-Based Fetal Cardiac Interventions.

Fetal cardiac intervention (FCI) is an emerging and rapidly advancing group of interventions designed to improve outcomes for fetuses with cardiovascular disease. Currently, FCI is comprised of pharmacologic therapies (e.g., trans-placental antiarrhythmics for fetal arrhythmia), open surgical procedures (e.g., surgical resection of pericardial teratoma), and catheter-based procedures (e.g., fetal aortic valvuloplasty for aortic stenosis). This review focuses on the rationale, criteria for inclusion, technical details, and current outcomes of the three most frequently performed catheter-based F

Cardiovascular Vulnerability, Including Heart Failure Risk, in Breast Cancer Surgery: The Role of Operative Technique, Frailty, and Postoperative Complications.

Background and Objectives: Breast cancer surgery is increasingly performed in older patients with multimorbidity, in whom cardiovascular disease and frailty may substantially modify perioperative risk, including vulnerability to heart failure decompensation and other major medical complications. However, most available studies report global perioperative complication rates and composite medical endpoints, with heart failure events only rarely captured as dedicated outcomes, and operative technique, cardiovascular comorbidity, and frailty are often treated as separate domains rather than compon

Health Care Quality Measures: Methodological Considerations and the Role of the Consensus-Based Entity (Battelle Partnership for Quality Measurement).

This article examines the methodological foundations and evolving frameworks of health care quality measurement, with a focus on cardiothoracic surgery. It highlights the importance of rigorous measure development, including appropriate population selection, endpoint definition, and data sourcing. Central to provider performance evaluation is risk adjustment to account for case-mix variability. The article critiques inconsistencies in existing quality metrics and emphasizes the need for reliability and validity in measurement models. It also describes the role of the Consensus-Based Entity (Ba

Association between the Duke Activity Status Index and complications after noncardiac surgery: A systematic review.

BACKGROUND: Individuals with poor functional capacity are at increased risk of perioperative complications. The Duke Activity Status Index (DASI) can ascertain the maximum physical activity achievable. However, the accuracy of the DASI score in identifying high-risk individuals is unclear. The objective of this systematic review was to determine the association between the DASI score and postoperative complications. METHODS: Studies conducted in adults undergoing elective or emergency noncardiac surgery were eligible. The search strategy used MEDLINE, EMBASE, EMCARE and Cochrane CENTRAL, from

The new world of cardiac interventions: a brief review of the recent advances in non-coronary percutaneous interventions.

Advances in cardiovascular interventional techniques have allowed percutaneous treatment of conditions that either previously required open operations or have not been amenable to treatment. Conditions such as atrial and ventricular septal defects and patent foramen ovale are now amenable to percutaneous closure using implantable devices. A number of strategies have evolved to reduce the risk of stroke such as percutaneous occlusion of the left atrial appendage for patients in atrial fibrillation. Valvular heart disease and complications of its repair are approachable via the percutaneous rout

Left-sided ICD implantation via right-sided venous access in a patient with left hemodialysis fistula.

A sufficient patency of the central vein ipsilateral to atriovenous fistula is the lifeline for hemodialysis patients. We describe a case with left dialysis fistula who underwent left-sided pectoral implantation of an implantable cardioverter defibrillator via right subclavicular venous access in order to avoid the possibility of development of left subclavian stenosis or occlusion and to achieve lower defibrillation thresholds. This approach may serve as an alternative choice in patients unsuitable for left-sided venous access and with higher defibrillation thresholds on right-positioned impl

Potential Value of Novel Lipid-Derived Indicator as a Biomarker for Nonarteritic Anterior Ischemic Optic Neuropathy.

IMPORTANCE: Overuse of surgical procedures is increasing around the world and harms both individuals and health care systems by using resources that could otherwise be allocated to addressing the underuse of effective health care interventions. In low- and middle-income countries (LMICs), there is some limited country-specific evidence showing that overuse of surgical procedures is increasing, at least for certain procedures. OBJECTIVES: To assess factors associated with, extent and consequences of, and potential solutions for low-value surgical procedures in LMICs. EVIDENCE REVIEW: We searc

Prediction Models for Postoperative Delirium After Cardiovascular Surgery: A Systematic Review and Critical Appraisal.

BACKGROUND: Postoperative delirium of cardiovascular surgery (PODOCVS) is an acute brain dysfunction, which will lead to increased postoperative complications and high mortality rates. Using prediction models for early identification and intervention could improve outcomes and resource utilisation. However, the quality and applicability of existing models remain unclear, and healthcare professionals are often unsure which model should be recommended to cardiovascular surgery (CS) patients in a specific setting. AIM: To systematically review and critically evaluate the development, performance

Beyond the Heart: The Significance of Depression in Cardiac Surgery.

OBJECTIVES: Preoperative depression is common among patients with cardiovascular disease and a significant risk factor for worse outcomes after cardiac surgery. This review summarizes the current evidence on depression as a risk factor and possible treatment options in patients undergoing cardiac surgery, and highlights future perspectives for clinical research. METHODS: This narrative review was based on a selection of key papers, identified through collegial expert discussions rather than a systematic literature search. RESULTS: Depression is often underdiagnosed and undertreated in patien

Understanding Electrocardiography in Adult Patients With Congenital Heart Disease: A Review.

IMPORTANCE: Congenital heart disease in adults is still a relatively new concept for many cardiologists, and the complexity as well as diversity of cardiac phenotypes encountered necessitate that systematic, practical information be available for the nonspecialist. The analysis of the 12-lead electrocardiogram is an invaluable cornerstone in the clinical appraisal of these patients. OBSERVATIONS: Consideration of the main anatomic and pathophysiological aspects of the various congenital heart conditions can shed light on their distinctive electrocardiogram patterns, which are an electrical re

[Different surgical approaches to Scimitar syndrome].

Scimitar syndrome is a rare congenital heart defect characterized by the combination of vascular, bronchial, and parenchymal malformations. This syndrome includes anomalous right pulmonary venous drainage to the inferior caval vein, hypoplastic right pulmonary artery, right lung hypoplasia and the presence of aortopulmonary collaterals to the right lung. In this study, we evaluate the different surgical approaches of 3 cases with Scimitar syndrome who was corrected successfully.

Radiographic and Clinical Brain Infarcts in Cardiac and Diagnostic Procedures: A Systematic Review and Meta-Analysis.

BACKGROUND AND PURPOSE: The incidence of periprocedural brain infarcts varies among cardiovascular procedures. In a systematic review, we compared the ratio of radiographic brain infarcts (RBI) to strokes and transient ischemic attacks across cardiac and vascular procedures. METHODS: We searched MEDLINE and 5 other databases for brain infarcts in aortic valve replacement, coronary artery bypass grafting, cardiac catheterization, and cerebral angiogram through September 2015. We followed the PRISMA (preferred reporting items for systematic reviews and meta-analyses) recommendations. We defined

Role of Computational Modelling in Planning and Executing Interventional Procedures for Congenital Heart Disease.

Increasingly, computational modelling and numerical simulations are used to help plan complex surgical and interventional cardiovascular procedures in children and young adults with congenital heart disease. From its origins more than 30 years ago, surgical planning with analysis of flow hemodynamics and energy loss/efficiency has helped design and implement many modifications to existing techniques. On the basis of patient-specific medical imaging, surgical planning allows accurate model production that can then be manipulated in a virtual surgical environment, with the proposed solutions fin

Intracardiac echocardiography: evolving use in interventional cardiology.

Intracardiac echocardiography (ICE) uses a catheter-based steerable ultrasound probe that is passed into the right heart chambers to image intracardiac structures. The transducer can be variably positioned for optimal imaging: in the inferior vena cava to visualize the abdominal aorta; in the right atrium for the interatrial septum, aortic, mitral, and tricuspid valves, and pulmonary veins; or in the right ventricle for the left ventricular function, outflow tract, or pulmonary artery. Intracardiac echocardiography is primarily used for imaging during an invasive cardiac procedure using consci

An interrupted suture technique for ascending aortic replacement concomitant to aortic valve repair.

While running suture anastomoses represent the mainstay of thoracic aortic surgery, advantages may exist for interrupted techniques in selected cases, especially with ascending aortic replacement and concomitant aortic valve repair. Interrupted suture anastomoses precisely reconstruct the sinotubular geometry in dissections, and allow the proximal suture line to be placed close to the coronary arteries and aortic valve posts. With aneurysm extension into the aortic arch, interrupted suture techniques allow hemi-arch replacement without the need for aortic clamp removal and circulatory arrest.

Technical modification of the neoaortic reconstruction in Norwood procedure: sparing of anterior wall of the main pulmonary artery.

The growth potential of the neoaorta after the stage I Norwood procedure could be limited when the patient has a small ascending aorta. Patients with hypoplastic left heart syndrome usually have a large main pulmonary artery (MPA) and from its posterior wall both pulmonary arteries arise. We describe a modified technique that spares a large anterior wall of the MPA in the neoaortic reconstruction of Norwood procedure to enhance the growth potential of the neoaorta.

Scimitar syndrome.

Scimitar syndrome is a rare congenital anomaly consisting in part of right pulmonary venous return to the inferior vena cava. There is a clear bimodal presentation of this syndrome with either an infantile manifestation or a pediatric/adult form. The infantile variant is marked by a higher incidence and severity of associated defects, heart failure, pulmonary hypertension, and significant mortality. The patient with the pediatric/adult form is less severely affected and may be asymptomatic on diagnosis. In this article, we review the historical aspects, presentation, and pathophysiology of Sci
