Monkeypox Case Investigation - Cook County Jail, Chicago, Illinois, July-August 2022.

Knowledge about monkeypox transmission risk in congregate settings is limited. In July 2022, the Chicago Department of Public Health (CDPH) confirmed a case of monkeypox in a person detained in Cook County Jail (CCJ) in Chicago, Illinois. This case was the first identified in a correctional setting in the United States and reported to CDC during the 2022 multinational monkeypox outbreak. CDPH collaborated with CCJ, the Illinois Department of Public Health (IDPH), and CDC to evaluate transmission risk within the facility. Fifty-seven residents were classified as having intermediate-risk exposur

Ixodes scapularis (Acari: Ixodidae) distribution surveys in the Chicago metropolitan region.

Considering recent studies confirming an increased risk of contracting Lyme disease near metropolitan Chicago, we surveyed a more comprehensive area to assess whether the geographical distribution and establishment of Ixodes scapularis (Say) populations across northeast Illinois are widespread or limited in occurrence. From May through October 2008 and from April through October 2009, 602 I. scapularis ticks of all three life stages (larva, nymph, adult) were collected from sites in Cook, DuPage, Lake, and McHenry counties in northeast Illinois. The surveys were conducted by drag sampling vege

HIV risk practices among needle exchange users and nonusers in Chicago.

OBJECTIVES: To assess associations between needle exchange program (NEP) use and drug injection risk practices. METHODS: Between 1997-2000, injecting drug users (IDUs) in Chicago were recruited from NEPs and an area with no NEP, interviewed about risk practices, and counseled and tested for HIV. The risk practices of "regular NEP users"--those who obtained at least half of their needles from an NEP (n = 558)--were compared with those of IDUs who did not use an NEP (n = 175). RESULTS: In multivariate analysis, regular NEP users, compared with NEP nonusers, were less likely to receptively shar

Excess hospital admissions during the July 1995 heat wave in Chicago.

INTRODUCTION: This study describes medical conditions treated in all 47 non-VA hospitals in Cook County, IL during the 1995 heat wave. We characterize the underlying diseases of the susceptible population, with the goal of tailoring prevention efforts. METHODS: Primary and secondary discharge diagnoses made during the heat wave and comparison periods were obtained from computerized inpatient hospital discharge data to determine reasons for hospitalization, and comorbid conditions, respectively. RESULTS: During the week of the heat wave, there were 1072 (11%) more hospital admissions than ave

Heat-related deaths during the July 1995 heat wave in Chicago.

BACKGROUND: During a record-setting heat wave in Chicago in July 1995, there were at least 700 excess deaths, most of which were classified as heat-related. We sought to determine who was at greatest risk for heat-related death. METHODS: We conducted a case-control study in Chicago to identify risk factors associated with heat-related death and death from cardiovascular causes from July 14 through July 17, 1995. Beginning on July 21, we interviewed 339 relatives, neighbors, or friends of those who died and 339 controls matched to the case subjects according to neighborhood and age. RESULTS:

Wave-driven resuspension as the dominant environmental predictor of enterococci at Chicago beaches.

Predicting fecal indicator bacteria (FIB) to protect public health at Chicago beaches remains a significant challenge due to complex, site-specific environmental dynamics. This study presents a machine learning pipeline utilizing site-specific random forest regressor models to predict log-transformed enterococci concentrations. The models integrate meteorological, hydrodynamic, and temporal-lag features. A hybrid evaluation framework is implemented; models are trained for regression but evaluated on their ability to classify bacterial exceedances at a 320 CCE/100 mL threshold, chosen as a cons

From Liverpool to Chicago: The Legacy of Mitchell-Banks and Ferguson in Pediatric Herniotomy.

BACKGROUND: Pediatric inguinal herniotomy remains one of the most commonly performed surgical procedures in children. Two historical techniques-those of William Mitchell-Banks and Alexander Hugh Ferguson-laid the foundation for age-adapted surgical strategies in hernia repair. AIM: To examine the historical, anatomical, and surgical foundations of the Mitchell-Banks and Ferguson techniques in pediatric inguinal herniotomy, to distinguish their technical differences, and to advocate for the pedagogical recovery of their eponyms in contemporary surgical training. METHODS: This historical revie

[Diagnosis of esophageal motility disorders - Significance of the new Chicago classification v4.0].

Esophageal motility disorders are diseases in which there are malfunctions of the act of swallowing due to a change in neuromuscular structures. The main symptom is therefore dysphagia for solid and/or liquid foods, often accompanied by symptoms such as chest pain, regurgitation, heartburn, and weight loss. Esophageal manometry is the gold standard in diagnostics. Endoscopy and radiology serve to exclude inflammatory or malignant changes. With the introduction of high-resolution esophageal manometry (HRM), the diagnosis of esophageal motility disorders has improved and led to a new classificat

Association Of Neighborhood Disadvantage With Racial Disparities In COVID-19 Positivity In Chicago.

Racial health inequities exemplified during the COVID-19 crisis have awakened a sense of urgency among public health and policy experts to examine contributing factors. One potential factor includes the socioeconomic disadvantage of racially segregated neighborhoods. This study quantified associations of neighborhood socioeconomic disadvantage in Chicago, Illinois, as measured by the Area Deprivation Index (ADI), with racial disparities in COVID-19 positivity. A retrospective cohort included 16,684 patients tested for COVID-19 at an academic medical center and five community-based testing site

Case files of the toxikon medical toxicology fellowship in Chicago: the poisoned anesthesiologist.

Suicide attempts by health care workers are very serious because these patients have the knowledge of and access to potentially lethal substances. Our patient presented with hypotension and hemodynamic instablility. He was treated with multiple-dose activated charcoal and 2 rounds of hemodialysis. While there was rapid toxicokinetic improvement and extubation on day 2 of his hospital stay, he had prolonged altered mental status, illustrating the long-acting effects of phenobarbital. It remains unclear exactly how our medical treatment regimens alter the clinical course of phenobarbital poisoni

Epidemiology of respiratory syncytial virus infection among infants and children in Chicago.

From January 1, 1967 to December 1971, the temporal pattern of respiratory syncytial virus infection was investigated in infants and children younger than 18 months hospitalized for acute lower respiratory tract disease. Of 4696 infants and children with acute lower respiratory tract disease admitted to the Cook County Hospital, 2530 were tested for virus infection by virus isolation or serologic procedures or both. Overall, respiratory syncytial virus infections were detected in 12% and parainfluenza 3 virus in 10.8% of individuals tested. Other respiratory viruses were less commonly identifi

How Healthy Are Community Health Workers: Findings from a Chicago-Area Survey.

Community health workers (CHWs) are a growing and essential component of the U.S. healthcare workforce, yet little is known about their own health and well-being. The CHWs Our Work + Our Health Pilot Study administered a cross-sectional survey to a convenience sample of CHWs living and/or working in the Chicago metropolitan area. The survey assessed general health, chronic conditions, health behaviors, and mental health using validated, standardized measures. Prevalence estimates were compared to Chicago population data using two-proportion z-tests. The final analytic sample included 179 CHWs,

[Chicago Classification ver. 4.0: Diagnosis of Achalasia and Esophagogastric Junction Outflow Obstruction].

Achalasia is a common esophageal motility disorder characterized by inappropriate relaxation of the lower esophageal sphincter and a loss of normal peristalsis in the esophageal body. The newly suggested Chicago Classification ver. 4.0 recommends conclusive diagnoses of types 1, 2, and 3. A conclusive diagnosis of type 3 achalasia requires no appreciable peristalsis compared to the previously used ver. 3.0. In the case of inconclusive diagnosis of achalasia, additional tests, including timed barium esophagogram (TBE) or functional lumen imaging probe (FLIP), are strongly recommended in ver. 4.
