BACKGROUND: Cold- and heat-induced urticaria are chronic physical urticaria conditions in which wheals, angioedema or both are evoked by skin exposure to cold and heat respectively. The diagnostic work up of both conditions should include skin provocation tests and accurate determination of critical temperature thresholds (CTT) for producing symptoms in order to be able to predict the potential risk that each individual patient faces and how this may be ameliorated by therapy.
OBJECTIVE: To develop and validate TempTest(®) 4, a simple and relatively inexpensive instrument for the accurate det

BACKGROUND: While a number of the consequences of mast cell degranulation within tissues have been documented including tissue-specific changes such as bronchospasm and the subsequent cellular infiltrate, there is little known about the immediate effects of mast cell degranulation on the associated vasculature, critical to understanding the evolution of mast cell dependent inflammation.
OBJECTIVE: To characterize the microcirculatory events that follow mast cell degranulation.
METHODOLOGY/PRINCIPAL FINDINGS: Perturbations in dermal blood flow, temperature and skin color were analyzed using l

BACKGROUND: Patients with acquired cold urticaria (ACU) show itchy wheals during cold exposure. This disturbing condition involves histamine and platelet-activating factor in its pathogenesis. Rupatadine is a dual antagonist of both histamine and platelet-activating factor.
OBJECTIVE: To assess rupatadine efficacy in preventing reactions to cold challenge in patients with ACU.
METHODS: A crossover, randomized, double-blind, placebo-controlled study in which 21 patients with ACU received rupatadine, 20 mg/d, or placebo for 1 week each is presented. The main outcome was the critical stimulatio

BACKGROUND: Acquired cold-induced urticaria (ACU) has not been well evaluated in pediatrics.
OBJECTIVE: To further evaluate the presentation of ACU in children and associated risk of anaphylaxis.
METHODS: A retrospective chart review was performed in children 18 years or younger diagnosed with ACU at Boston Children's Hospital (US, Northeast) from 1996 to 2017.
RESULTS: A total of 415 patients with ACU were identified, aged 4 months to 18.3 years at the time of diagnosis, with similar male:female distribution. Most patients had a history of atopic disease (78.3%), and 25.8% had other urtica

BACKGROUND: Increased dosing of nonsedating antihistamines is recommended by the current European Academy of Allergology and Clinical Immunology/Global Allergy and Asthma European Network/European Dermatology Forum guidelines on patients with acquired cold urticaria (ACU) who do not respond satisfactorily to the standard dose. Prospective data supporting this recommendation are scant.
OBJECTIVE: We sought to assess the effects of 5 and 20 mg of desloratadine and placebo on cold-induced urticarial reactions in patients with ACU.
METHODS: In this prospective, randomized, double-blind, 3-way cr

This study aimed to determine the current prospective strategies for diagnosing and treating cold urticaria based on the history of its research since the 19th century. The study analysed relevant scientific sources, and the main historical milestones of cold urticaria research were identified. Based on the results, a chronology of scientific events was established that formed the modern understanding of the pathogenesis of cold urticaria and contributed to the development of diagnostic and treatment methods used in contemporary clinical practice. Throughout the 19th century, scientists formul

Cold contact urticaria is the second most common subtype of physical urticaria. Cold stimulation standardized tests are mandatory to confirm the diagnosis. The aim of this study is to define the utility of determining thresholds (critical time and temperature) in assessment of the clinical course of typical acquired cold contact urticaria. Nineteen adult patients (10 women and 9 men; mean age 45 years) were included in the study and the diagnosis was confirmed with the ice-cube test and TempTest 3.0. Patients were treated continuously for 1 year with 20 mg/day rupatadine (anti-H1). Thresholds

The histopathology, immunofluorescence and ultrastructure of skin in idiopathic cold-contact urticaria have been studied over the 24 h following the application of a cold stimulus sufficient to provoke a confluent weal on the anterior thigh. Biopsies were taken 10 min, 2 h and 24 h after ice removal. Considerable epidermal and dermal oedema was present. Type I and Type II mast-cell degranulation was noted but was not universal. Lymphatics and capillaries were dilated and endothelial cells showed an increase in micropinocytotic activity, without evident vasculitis. In two cases packed platelets

BACKGROUND: Phospholipase Cγ2 (PLCγ2) is an important signaling molecule that receives and transmits signals from various cell surface receptors in most hematopoietic lineages. Variants of PLCG2 cause PLCγ2-associated immune dysregulation (PLAID), a family of conditions that are classified by mutational effect. PLAID with cold urticaria (CU-PLAID) is caused by in-frame deletions of PLCG2 that are dominant negative at physiologic temperatures but become spontaneously active at sub-physiologic temperatures.
OBJECTIVE: To identify genetic lesions that cause PLAID by combining RNA sequencing of f

Serum immunoglobulins, complement and alpha 1-antitrypsin were assayed in forty-eight patients with chronic urticaria. Thirteen cases had chronic cold urticaria and thirty-two had chronic idiopathic urticaria. Elevated mean serum IgM was found in chronic cold urticaria. Seven patients had partial immunoglobulin deficiencies. IgE was elevated in sixteen cases of chronic idiopathic and in two with chronic cold urticaria. Eight patients had depressed serum total haemolytic complement activity. Low C3 and normal C4 serum protein concentrations in four cases suggested alternative complement pathway

Chronic inducible urticaria (CIndU) is a group of chronic urticarias characterized by the appearance of recurrent wheals, recurrent angioedema or both, as a response to specific triggers. CIndU includes both physical (symptomatic dermographism, cold and heat urticaria, delayed pressure urticaria, solar urticaria, and vibratory urticaria) and nonphysical urticarias (cholinergic urticaria, contact and aquagenic urticaria). Here, we review the different forms of CIndU with an emphasis on symptomatic dermographism, cold urticaria, cholinergic urticaria, and delayed pressure urticaria. We discuss t

Ten percent of chronic urticarias are physical urticarias. Patients suffering from physical urticaria all have a suggestive history with specific eliciting stimuli (cold, heat, water, sun.) and wheals in the areas where the stimulus acts. The involved pathomechanisms are not well known. An unknown allergen (related to a cold or a heat injury, a polar molecule contained in the stratum corneum and soluble into water, a photoallergen) could induce a mast cell mediator release, followed by an infiltration by eosinophil then neutrophils polymorphonuclears. T-cells are not highly involved. Dermograp

Psychosomatic investigations were made in 18 patients with dermographism, 17 with cold urticaria, and 13 with cholinergic urticaria, and 13 with cholinergic urticaria. The Maudsley Personality Inventory was used and electroencephalograms were recorded from most patients. In the majority of cases of dermographism first symptoms coincided with frustrating situations and psychic stress-intensified manifestations. 81.8% of the patients with dermographism had abnormal electroencephalograms. In cold urticaria the role of psychic factors could not be demonstrated, whereas in cholinergic urticaria 'br

In urticaria, adverse reactions to food are only a frequent finding in the subset of patients with chronic continuous urticaria. Mostly these reactions are of pseudoallergic nature, directed against artificial additives as well as naturally occurring aromatic components. IgE-mediated allergic reactions are a rare cause in acute urticaria as well as in recurrent chronic urticaria. In other types of urticaria, e.g. physical urticaria, food plays hardly any role as an eliciting agent with the exception of ice-cold drinks in cold urticaria. By contrast, exercise-induced anaphylaxis is frequently f

Urticaria and physical angioedema frequency is hard to evaluate in children. In this series, we keep 53/1000. Physical urticaria is found in 36% of the cases and concerns cholinergic urticaria, urticaria to cold, idiopathic dermographism and mastocytosis, and delayed urticaria to pressure. The authors insist on the interest of the etiologic research in these forms of infantile urticaria.

A military-centric case study of a 29-year-old woman with no significant medical history presented with urticaria induced by cold. She was clinically diagnosed with primary acquired cold urticaria (ACU). This case is a catalyst for the discussion of a workup for ACU, the differential diagnosis of ACU, and treatment of ACU.

Chronic spontaneous urticaria (CSU) is a clinical condition characterized by spontaneous or inducible recurrent wheals. This condition may significantly affect quality of life of patients and of their families. Etiology is not identified in 25-85% of cases that are indicated as 'idiopathic', because all diagnostic tests are negative. Autoimmune processes may be present in 30-50% of patients, although a definite etiological diagnosis is seldom possible. Some patients, in fact, have autoantibodies against the high-affinity IgE receptor FcεR1 or IgE. These patients show an increased incidence of

Inducible urticaria is a heterogeneous subgroup of chronic urticarias caused by a wide variety of environmental stimuli, such as exercise, cold, heat, pressure, sunlight, vibration, and water. A new term, follicular traction urticaria, was suggested as an unusual form of inducible urticarias. We report a patient who was diagnosed with follicular traction urticaria.
