Effects of hyperbaric oxygen pretreatment on brain antioxidant capacity in rats with decompression sickness.

OBJECTIVE: Decompression sickness (DCS) causes serious brain hypoxic-ischemic injury. This experiment was designed to observe whether hyperbaric oxygen (HBO2) pretreatment played a neuroprotective effect in decompression sickness rat models and to explore the mechanism of protective effects. METHODS: Sprague-Dawley (SD) male rats were pretreated with HBO2 and then underwent decompression to establish the DCS rat model. Antioxidant capacities were evaluated by detecting peroxides (GPx), superoxide dismutase (SOD), catalase (CAT) activity and malondialdehyde (MDA) content in brains. The levels

Gas in Joints After Diving: Computed Tomography May Be Useful for Diagnosing Decompression Sickness.

A 26-y-old experienced scotoma scintillans after 59 min of scuba diving at a maximum depth of 26 m. After the patient smoked a cigarette, the scotoma scintillans ceased. However, he then developed a headache, general fatigue, and shoulder and elbow pain. He therefore called an ambulance. Based on the rules of the medical cooperative system for decompression sickness in Izu Peninsula, the fire department called a physician-staffed helicopter. After a physician checked the patient, his complaints remained aside from a low-grade fever. A portable ultrasound revealed bubbles in his inferior vena c

The effect of intravenous perfluorocarbon emulsions on whole-body oxygenation after severe decompression sickness.

INTRODUCTION: Decompression sickness (DCS) results from a decrease in ambient pressure leading to supersaturation of tissues with inert gas and bubble formation. Perfluorocarbons (PFCs) are able to dissolve vast amounts of non-polar gases. Intravenous (IV) PFC emulsions reduce both morbidity and mortality associated with DCS, but the mechanism of this protective effect has not yet been demonstrated. METHODS: Juvenile Dorper-cross sheep (n = 31) were anaesthetised and instrumented for physiological monitoring, IV fluid administration and blood sampling. Animals were compressed in air in a hype

Intramedullary Gas Detected on Computed Tomography in Cases of Decompression Sickness: A Case Series.

Decompression sickness (DCS) is caused by nitrogen gas bubbles in the blood that lead to bubble embolization and tissue compression. Intramedullary gas bubbles may also be observed; however, their clinical significance remains unclear. Herein, we describe our experiences with three cases that experienced DCS in which intramedullary gas bubbles were incidentally discovered on computed tomography (CT) performed before treatment. Case 1 involved a 28-year-old male who had repeatedly dived to 30 m and surfaced. After diving, the patient experienced pain in the left lower extremity. CT revealed bub

Severe neurological decompression sickness associated with right ventricular dilatation and a persistent foramen ovale.

We present the case of a 28-year-old female diver who performed a scuba air dive with significant omitted decompression obligation. She developed constitutional and neurological symptoms. Brain magnetic resonance imaging post treatment demonstrated multifocal embolic infarcts and transthoracic echocardiogram with bubble contrast on day three revealed a persistent foramen ovale (PFO) and severe right ventricular (RV) dilatation. We postulate that the high venous bubble load from the provocative decompression caused an increase in pulmonary artery pressure, leading to RV dilatation and increased

Vestibular rehabilitation and recovery in divers with inner ear decompression sickness: a case series.

INTRODUCTION: The mechanism of injury and recovery of divers with inner ear decompression sickness (IEDCS) is not well understood and there is no consensus regarding management following recompression treatment. Given the rare occurrence, divers are not routinely offered the standard therapies that patients with other acute vestibular disorders may be offered such as vestibular rehabilitation. METHODS: This is an observational case series of 13 divers presenting acutely with IEDCS to DDRC Healthcare in Plymouth, UK between July 2021 and January 2024. Vestibular and balance tests were undertak

Delayed recognition of Type II decompression sickness in a diver with chronic atrial fibrillation.

INTRODUCTION: This case report describes an initially overlooked Type II decompression sickness (DCS) occurrence that was confused with a cerebral vascular accident in a patient with chronic atrial fibrillation (AF). The purpose of this case report is to reinforce the maxim that DCS needs to be suspected anytime a scuba diver experiences signs or symptoms compatible with DCS after completing a scuba dive. METHODS: A 71-year-old scuba diver with a history of AF and who was taking warfarin made four dives, all with maximum depths less than 60 fsw (20 msw) over a 10-hour interval. Shoulder pain

Inner ear barotrauma and inner ear decompression sickness: a systematic review on differential diagnostics.

INTRODUCTION: Inner ear barotrauma (IEBt) and inner ear decompression sickness (IEDCS) are the two dysbaric inner ear injuries associated with diving. Both conditions manifest as cochleovestibular symptoms, causing difficulties in differential diagnosis and possibly delaying (or leading to inappropriate) treatment. METHODS: This was a systematic review of IEBt and IEDCS cases aiming to define diving and clinical variables that help differentiate these conditions. The search strategy consisted of a preliminary search, followed by a systematic search covering three databases (PubMed, Medline, S

Gas in coronary artery: A case of fatal decompression sickness evaluated by computed tomography.

A 54-year-old man suffered a leg cramp while diving in the ocean at a depth of 20 meters. He began to surface, with his ascent based on a decompression table. He lost consciousness at the surface and was rescued by a nearby boat. The boat staff judged him to be in cardiac arrest, so they performed chest compressions. When the boat reached port where an ambulance was waiting, emergency medical technicians confirmed that the patient was in cardiac arrest; his initial rhythm was asystole. Treated with basic life support, the patient was then transported to a rendezvous point, where a physician-st

Cerebellar infarction presenting as inner ear decompression sickness following scuba diving: a case report.

INTRODUCTION: Inner ear decompression sickness following scuba diving is not uncommon and the characteristic features of this disorder are acute peripheral vestibular syndrome, sometimes associated with cochlear signs, requiring urgent hyperbaric oxygen therapy. Cerebellar infarction can also mimic isolated peripheral vestibulopathy. CASE REPORT: The authors report the case of a 47-year-old man in good general health admitted with acute left vestibular dysfunction suggestive of inner ear decompression sickness 6 hours after scuba diving. Normal videonystagmography and delayed onset of occipit

Recurrent cutaneous decompression sickness in a hyperbaric chamber attendant with a large persistent foramen ovale.

A 41-year-old female nurse had cutaneous decompression sickness on two occasions after acting as an inside chamber attendant for patients receiving hyperbaric oxygen. She breathed air during the treatments at pressures equivalent to 14 and 18 metres of seawater, but each time she decompressed whilst breathing oxygen. Latency was 2.5 hours and one hour. She was found to have an 11 mm diameter persistent foramen ovale. It was closed and she returned to work without recurrence of decompression sickness. Review of the literature suggests that shunt mediated decompression sickness is an important o

Screening and Risk Stratification Strategy Reduced Decompression Sickness Occurrence in Divers With Patent Foramen Ovale.

OBJECTIVES: This paper sought to evaluate the occurrence of decompression sickness (DCS) after the application of a patent foramen ovale (PFO) screening and risk stratification strategy. BACKGROUND: PFO is associated with an increased risk of DCS. Recently, transcatheter closure was reported to reduce DCS occurrence in divers with a high-grade shunt. However, to date, there are no data regarding the effectiveness of any PFO screening and risk stratification strategy for divers. METHODS: A total of 829 consecutive divers (age 35.4 ± 10.0 years, 81.5% men) were screened for PFO by means of tra

Patent Foramen Ovale (PFO), Personality Traits, and Iterative Decompression Sickness. Retrospective Analysis of 209 Cases.

Introduction: There is a need to evaluate the influence of risk factors such as patency of foramen ovale (PFO) or "daredevil" psychological profile on contra-indication policy after a decompression sickness (DCS). Methods: By crossing information obtained from Belgian Hyperbaric Centers, DAN Emergency Hotline, the press, and Internet diving forums, it was possible to be accountable for the majority if not all DCS, which have occurred in Belgium from January 1993 to June 2013. From the available 594 records we excluded all cases with tentative diagnosis, medullary DCS or unreliability of report

Assessment of spinal cord trauma using evoked potentials in a rat model of decompression sickness.

A rapid quantitative technique for assessing spinal cord trauma in a rat model of decompression sickness is described. Evoked potentials are measured from the lower limbs of rats before and after dives with compressed air in a hyperbaric chamber. Under chloral hydrate anesthesia, the sciatic nerve is stimulated at the sciatic notch with needle electrodes and platinum/iridium electrodes are used to record the action potentials from the plantar muscles. Analysis showed that the sensory reflex response was markedly depressed in the rats soon after diving and did not recover for up to 5 days. The

Utilization of a central auditory processing test battery in evaluating residual effects of decompression sickness.

Six professional divers were examined as patients who presented peripheral auditory, vestibular, or central symptoms attributed to incidents which occurred while diving. Each diver received a central auditory processing (CAP) test battery consisting of: (1) a dichotic sentence listening task, (2) a monotic filtered word task, (3) a binaural fusion task, and (4) an alternating speech task. The CAP test results, in all cases substantiated other neurological/vestibular test battery results which we have reported in detail previously.

Adaptive evolution of endothelial nitric oxide synthase (NOS3) may reduce cetacean susceptibility to decompression sickness.

Cetaceans tolerate repeated diving bouts. While the extent to which cetaceans experience decompression sickness remains debated, this group of fully aquatic mammals must have evolved a tolerance to damaging nitrogen (N2) gas bubbles that can form during overly rapid ascents or after prolonged dives. Here, we present the first in-depth molecular evolutionary analysis of the nitric oxide synthase gene family across cetaceans and identify cetacean-specific amino acid substitutions in NOS3 (encoding endothelial nitric oxide synthase) that likely arose in a stem cetacean ancestor, coincident with t

Case Report: Successful treatment of severe type II decompression sickness characterized by multiple gas emboli.

BACKGROUND: Decompression sickness (DCS) is usually caused by inadequate decompression. Although adherence to decompression protocols can significantly reduce the incidence of DCS, it still cannot prevent all cases from occurring. If a large number of gas bubbles enter the right heart and pulmonary arterial system, patients may present with symptoms such as cough, tachypnea, chest pain, dyspnea, or even shock. The presence of numerous bubbles in the abdominal cavity and portal venous system may also lead to liver dysfunction or abdominal pain. Theoretically, DCS occurring after dives that foll
