Management of Traumatic Penile Degloving Injury in a Resource-Constrained Setting: A Case Report and Literature Review.

Genital degloving injuries from motorcycle accidents are uncommon, with most reported cases involving complete devascularisation requiring skin grafting. We present a 70-year-old male who presented to a rural health center following a high-speed motorcycle accident with a unique distal pedicure circumferential penile skin avulsion. The penile skin sleeve remained viable through its distal attachment near the corona, while the proximal end was completely separated at the penoscrotal junction, exposing intact underlying corpora with confirmed urethral continuity. Management consisted of thorough

Wrist and hand degloved injuries salvaged with reverse radial forearm flap in the CoVid-19 era.

Reverse radial forearm flap is a quick and reliable flap, providing a well-vascularized tissue for coverage of hand defects. It is based on the reverse flow of the radial artery and the only drawback is the proximal ligation of the vessel. Two cases with complex dorsal hand defects were referred with delay to our Hand Unit, due to the CoVid-19 pandemic restrictions. In both cases deep structures of the wrist and fingers were exposed and the wounds were infected. Surgical debridement, skin coverage and reconstruction were decided in a staged approach: The reverse radial forearm flap was first p

Destructive Arthropathy of the Interphalangeal Joint Following Revascularization After Degloving Hand Injury: A Case Report.

It is difficult to achieve satisfactory results in the treatment of advanced degloving injury, which is one of the most challenging injuries in hand surgery. In this report, we present a case of marked destructive arthropathy of the interphalangeal joint that developed following revascularization after degloving hand injury. A 37-year-old, right-handed female manual worker sustained a degloving injury of her fingers, including the dorsal hand. We performed revascularization and secondary surgery, which included tenolysis and a tendon graft, to obtain functional improvement. These 2 procedures

Treatment of thumb tip degloving injury using the modified first dorsal metacarpal artery flap.

PURPOSE: This study reports repair of a thumb tip degloving injury using the modified first dorsal metacarpal artery (FDMA) flap, including both dorsal branches of the proper digital nerve (DBPDNs). METHODS: From May 2006 to February 2008, the modified FDMA flap was used in 11 thumbs in 11 patients. All patients suffered a degloving injury to the thumb tip, and 4 had associated bone loss ranging from 1 to 3 mm (mean, 2 mm) in length. The size of the soft tissue defects was 2.6 to 4.6 cm (mean, 3.5 cm) in length and 1.8 to 2.2 cm (mean, 2.0 cm) in width. The flaps ranged in size from 2.7 × 2.2

Surgical management and outcomes of open degloving injuries of the extremities: a retrospective cohort study at a Level II trauma center.

BACKGROUND: Open degloving injuries are severe soft-tissue traumas associated with ischemia, infection, and poor functional outcomes. Due to heterogeneous presentations, standardized management protocols remain limited. METHODS: We conducted a retrospective cohort study of 28 consecutive patients with open degloving injuries of the extremities treated at a Level II trauma center between 2020 and 2024. Demographics, injury characteristics, surgical interventions, and outcomes were analyzed using chi-square/Fisher's exact tests and nonparametric methods. RESULTS: Median age was 49 years, and 5

Flap Salvage Using Topical Oxygen Therapy (Natrox) in a Pediatric Foot Degloving Injury: A Case Report.

Background: Foot degloving injuries are associated with extensive soft-tissue disruption, compromised perfusion, and a high risk of flap necrosis. Hyperbaric oxygen therapy (HBOT) is known to enhance tissue oxygenation and support flap survival; however, its application in pediatric patients may be limited due to poor cooperation, intolerance to chamber-based treatment, and limited accessibility. Case Presentation: A 7-year-old girl presented with a crush injury to the left foot after being run over by a vehicle, resulting in severe soft-tissue damage. Evaluation revealed a dorsal foot deglovi

Use of domestic tools to downstage the reconstructive ladder in a patient with severe crush and degloving injury: a case report.

In the COVID-19's crisis, elective surgery and non-emergent cases were postponed; all other procedures have to be minimized. A 17-year male patient with severe crush and degloving injury over the thigh, gluteal, sacral, and perineum areas was admitted to our Department on the 16th of March 2020. The patient presented soft tissue skin and muscle loss. A double Latissimus Dorsi and Anterolateral Thigh free flaps were indicated. However, due to the particular circumstance of the COVID-19 crisis, we applied domestic negative wound therapy (NPWT) using gauzes and wall suction. We obtained suitable

Case Report: The casting motion to mobilize stiffness technique for rehabilitation after a crush and degloving injury of the hand.

STUDY DESIGN: Case report. INTRODUCTION: This case report describes the use of the casting motion to mobilize stiffness (CMMS) technique in the management of a crush and degloving injury of the hand. The patient was unable to attend multiple hand therapy sessions due to geographic constraints. The CMMS technique involved the application of a nonremovable plaster of paris cast that selectively immobilizes proximal joints in an ideal position while constraining distal joints to direct desired motion over a long period. This uses active motion only. Traditional hand therapy techniques or modalit

Combining tangential hydrodissection, panniculectomy, and negative pressure wound therapy in treating extensive degloving injury of the leg.

Major degloving injuries of the lower limb are daunting lesions because they are relatively rare and always produce larger soft tissue defects than direct visual inspection that could be predicted in the emergency room. Enough body of the medical literature supports the excision of the avulsed tissue and replaces it as a full-thickness skin graft. However, almost paradoxically, there is little support for the use of pristine large full-thickness skin graft in the treatment of these lesions. This article focuses on the use of tangential hydrodissection (VERSAJET Hydrosurgery System, Smith & Nep

Use of a hypogastric flap and split-thickness skin grafting for a degloving injury of the penis and scrotum: A different approach.

Penile and scrotal skin avulsions are not common events and are caused usually by accidents with industrial machines or agricultural machines. We report a case of a 27-year-old newly married thin-built patient with avulsion and traumatic degloving of the penile and scrotal skin, with exposure of the corpora cavernosa and copus spongiosum of penis and testes as his loose clothes got entangled in a paddy harvesting machine accidently. Reconstruction was performed using a hypogastric flap and split skin graft, achieving a satisfactory aesthetic result and sexual functions.

Preservation of function following complete degloving injuries to the hand: use of simultaneous groin flap, random abdominal flap, and partial-thickness skin graft.

Early total wound coverage is effectively achieved in complete degloving injuries to the hand and digits by preliminary skeletal narrowing and shortening, volar partial-thickness skin grafting, and application of a long, axial-pattern attached groin flap to the dorsum of the hand and remaining phalanges. If the thumb has been skeletonized by the initial trauma, simultaneous use of a circumferential random-pattern abdominal flap ensures complete coverage and earlier mobilization. THis combination of techniques is effective in preservation of pinch power and control, hook function of the medial

Devastating genital injury leads to degloving and rerouting of the penis: An unusual case report in an 8-year-old child.

Penetrating penile injuries are rare in children. Thus, the evaluation and management of this type of injury have not been well described. Penile trauma in children varies in severity from trivial to more severe, with devastating functional and psychosocial consequences. Here, we report the case of an 8-year-old boy who suffered a penetrating penile injury as a result of falling from a height onto a sharp object, necessitating rerouting of the penis. After stabilization, the patient was subjected to careful surgical exploration and meticulous reconstructive surgery. The final outcome was excel

Novel Use of a Synthetic Electrospun Wound Matrix in an Extensive Forearm Degloving Injury: Case Report and Review of the Literature.

Wound matrices, such as synthetic electrospun fiber matrices (SEFMs), provide innovative scaffolds to allow for complex wound healing. This case report described a 74-year-old woman with an 80% degloving injury of the left forearm caused by a fall. Due to her medical history, including liver cirrhosis, malnutrition, and previous orthopedic injuries, traditional skin grafting was deemed too risky. Instead, following initial debridement and application of negative pressure wound therapy, an SEFM sheet was applied and later supplemented with 5 applications of particulate SEFM. In subsequent follo

Uncommon degloving intestinal injury accompanied by complete disruption of the rectus muscles in patient with seat belt sign: a case report.

Blunt abdominal trauma rarely causes injury to hollow intraabdominal viscus. Still, in victims with seat belt sign, a high index of suspicion should be raised. Additionally, accompanied injury to the abdominal wall muscles is reported. We present a case of a male patient injured in a high-velocity car accident presented with seat belt sign and peritoneal signs. Laparotomy revealed uncommon degloving injury of the small intestine with additional mesentery lesions and lacerations of the ascending and descending colon. Concomitant transection of both rectus abdominis muscles was also encountered.
