Selective removal of dental caries in permanent teeth: an 18-month clinical trial.

OBJECTIVES: This study aimed to evaluate the clinical efficacy of restorations after selective removal of carious tissue (SRCT) in permanent teeth. MATERIAL AND METHODS: A randomized, controlled, and blinded clinical trial was conducted on 74 teeth to compare SRCT with two types of non-selective removal (nSRCT), which served as control groups. For medium-depth lesions (D2) that reached the middle third of the dentin, SRCT was compared with non-selective removal to hard dentine (nSRCT-HD). For deep lesions (D3) extending to the inner third of the dentin, SRCT was compared with stepwise removal

Dentine hypersensitivity--guidelines for the management of a common oral health problem.

UNLABELLED: Dentine hypersensitivity (DHS) remains a worldwide under-reported and under-managed problem, despite making some dental treatments more stressful than necessary and having a negative impact on the patient's quality of life. This article is designed to build dental professionals' confidence and remove any confusion regarding the diagnosis, prevention and treatment of sensitive teeth caused by dentine hypersensitivity in those patients known to be at risk. There is a need for simple guidelines, which can be readily applied in general practice. However, it is also obvious that one str

The pulp capping procedure in primary teeth "revisited".

The purpose of this review is to "revisit" an earlier paper (1992) on the subject of direct pulp capping in primary teeth and bring some new considerations for the procedure by the use of dentin bonding adhesives. It has come to be recognized that the customary employment of calcium hydroxide for this therapy has some shortcomings that reduce the prognosis for a favorable outcome. For at least a decade, many investigations have found that postoperative sensitivity, thermal stimuli, pulp inflammation and pathosis can be attributed not to the composition of various dental materials and their ins

First-In-Human Study of Safety, Tolerability, Efficacy, and Pharmacokinetics of CPNE7-Derived Peptide (Selcopintide) for Dentin Hypersensitivity.

Dentin hypersensitivity is characterized by transient, sharp pain resulting from dentin exposure due to enamel or cementum loss. Current treatments, which primarily focus on superficial tubule occlusion or nerve desensitization, provide only temporary relief. Copine7 (CPNE7) induces odontoblast differentiation and physiologic dentin regeneration, enabling biological dentin sealing and presenting a novel therapeutic approach. This study reports the first-in-human results of a randomized, double-blind, dose-escalation phase 1/2a clinical trial evaluating the safety, tolerability, preliminary eff

Biomimetic oligopeptide formed enamel-like tissue and dentin tubule occlusion via mineralization for dentin hypersensitivity treatment.

OBJECTIVE: Dentin hypersensitivity (DH) is a common oral disease with approximately 41.9% prevalence. Reconstruction of dental hard tissues is the preferred treatment for relieving DH. Here, we applied biomineralization method using oligopeptide simulating cementum protein 1 (CEMP1) to regenerate hard tissues on demineralized dentin. METHODS: The self-assembly and biomineralization property of the oligopeptide were detected by scanning electron microscopy (SEM), circular dichroism spectroscopy, and transmission electron microscopy. Oligopeptide's binding capacity to demineralized dentin was e

Non-surgical management of tooth hypersensitivity.

Tooth sensitivity is a common complaint of patients in dental practices. Studies have demonstrated dentinal hypersensitivity to affect 10-30% of the population. There are various potential causes of tooth sensitivity and a variety of available treatment options. This narrative review will discuss the possible aetiology of this condition, as well as the treatment modalities available. A tailor-made treatment plan that starts with the most non-invasive treatment options and escalates only when those options have proven insufficient in alleviating symptoms should be provided for each patient. Onl

Samarium-doped hydroxyapatite: An effective approach for enhancing biomineralization in dental caries management.

BACKGROUND: Dental caries is a widespread non-communicable disease caused by interactions among acidogenic bacteria, fermentable carbohydrates, and host factors, leading to tooth demineralization. In dentin caries, this process exposes dentinal tubules, causing sensitivity and structural degradation. Despite available agents, effective dentin remineralization remains a challenge. Recent studies highlight samarium-doped hydroxyapatite (Sm-HAp) as a potential biomaterial for promoting remineralization. AIM: To synthesize and characterize the samarium doped hydroxyapatite (Sm-HAp) as a potential

Reducing the risk of sensitivity and pulpal complications after the placement of crowns and fixed partial dentures.

Sensitivity after cementation of a crown with glass-ionomer cement is often attributed to an adverse effect on the pulp by the luting agent. Most permanent restorative materials in common use today do not tend to irritate the pulp; the main cause of pulpal damage is infection, the bacteria originating in the smear layer or deep in the dental tubules, inaccessible to caries-excavating procedures. A poorly fitting provisional crown may expose cut dentin to the oral fluids, and mechanical trauma caused by frictional heat during preparation may also damage the pulp. The following precautions are r

Elastin-like polypeptide induced biomimetic mineralization for dentin hypersensitivity.

Dental hypersensitivity, a prevalent oral health issue, is closely associated with the exposure of dentinal tubules. Biomimetic mineralization has emerged as a promising approach to occlude these tubules and mitigate sensitivity. This study explores the potential of elastin-like polypeptide (ELP), a synthetic polypeptide, in dentinal tubule occlusion and its efficacy in reducing dental hypersensitivity. Building on previous findings that ELP can promote enamel regeneration, we extended its application to dentin repair. The structure of ELP was verified by time-of-flight mass spectrometry (TOF-
