A History of US Medical Device Regulation and Current Regulatory Framework.

» The Food and Drug Administration (FDA) was created in the wake of public outcry generated by Upton Sinclair's The Jungle. The FDA gained further regulatory authority following successive public health setbacks in the 20th century. » A full review for a new device is referred to as a premarket approval (PMA), while an application for a device based on a predicate is referred to as a premarket notification (PMN) better known as a 510k. » Both for PMNs and PMAs the costs and timelines typically far exceed the statutory limits imposed by congress, running into years of review and many millions o

Recalls of Arthroscopic Equipment After Food and Drug Administration 510(k) Approval: A Twenty-Year Analysis of Causes, Trends, and Time to Recall (2004 to 2024).

BACKGROUND: Current reliance on the expedited 510(k) approval pathway has driven rapid commercial availability of novel arthroscopic devices. Despite the low complication rates of arthroscopic procedures, products from this pathway are suspected to increase the rate of recalls and device malfunctions. PURPOSE: This study aimed to characterize arthroscopic device recalls, analyze trends in recall incidence, and identify predictors of time to recall. METHODS: A 20-year, retrospective, cross-sectional study was conducted using the Food and Drug Administration (FDA) Recalls database. Recalled de

Cardiac assist devices for end-stage heart failure.

Cardiac assist devices have become an important therapy for patients with end-stage cardiac failure. These devices continue to evolve. Most current devices provide temporary, left-sided support for patients with cardiogenic shock from postcardiotomy syndrome or for patients awaiting cardiac transplantation. The two most frequently used left ventricular assist devices as a bridge to transplant are the Thermocardiosystems (HeartMate I) and the Novacor devices. The selection criteria for the use of these devices and treatment of these patients will be reviewed. Additionally, other currently avail

Early postmarket results with PulseRider for treatment of wide-necked intracranial aneurysms: a multicenter experience.

OBJECTIVE: Traditionally, stent-assisted coiling and balloon remodeling have been the primary endovascular treatments for wide-necked intracranial aneurysms with complex morphologies. PulseRider is an aneurysm neck reconstruction device that provides parent vessel protection for aneurysm coiling. The objective of this study was to report early postmarket results with the PulseRider device. METHODS: This study was a prospective registry of patients treated with PulseRider at 13 American neurointerventional centers following FDA approval of this device. Data collected included clinical presenta

Progressive vertebral body osteolysis after cervical disc arthroplasty.

STUDY DESIGN: Case report. OBJECTIVE: To review the management of a patient with progressive osteolysis of the vertebral body after undergoing cervical arthroplasty for management of a refractory radiculopathy. SUMMARY OF BACKGROUND DATA: Since the Food and Drug Administration's (FDA) approval of cervical arthroplasty devices in 2007, many surgeons have incorporated this technology into clinical practice. As arthroplasty becomes more widespread, complications unique to this technology are inevitable. To date, only a limited number of complications have been reported in the literature suggest

Stimulating Patient Engagement in Medical Device Development in Kidney Disease: A Report of a Kidney Health Initiative Workshop.

New technologies challenge current dialysis treatment paradigms as devices become smaller, more portable, and increasingly used outside the dialysis clinic. It is unclear how patients will view this care transition, and it will be important to consider patient and care partner perspectives during all aspects of development for novel dialysis therapies, from design and clinical trials to regulatory approval. To gain insight into this area, the Kidney Health Initiative, a public-private partnership between the American Society of Nephrology, the US Food and Drug Administration, and nearly 80 mem

Controversies in utilization of transvaginal mesh.

PURPOSE OF REVIEW: Due to technological advancements, transvaginal mesh use for prolapse and incontinence has exploded over the last decade with mixed results. Recent governmental guidelines have further increased the controversy regarding the risks and benefits of mesh use forcing clinicians to critically review the available data regarding transvaginal mesh use. RECENT FINDINGS: With the success of the transvaginal tape procedure introduced in 1995 and re-popularization of the abdominal sacrocolpopexy using synthetic mesh, pelvic surgeons began to feel more comfortable using mesh in urogyne

Management of a large atrial septal occluder embolized to the left ventricular outflow tract without the use of cardiac surgery.

Transcatheter closure of secundum-type atrial septal defects (ASDs) using the AMPLATZER™ Septal Occluder (ASO) has been in use for more than a decade since its US Food and Drug Administration approval in 2001. Device embolization remains an uncommon complication, which can sometimes occur after the initial deployment. Previous reports of ASO devices embolized to the left ventricle have primarily been managed by open-heart surgical retrieval. We present a case of an ASO device embolized to the left ventricular outflow tract (LVOT) 18 hr after initial implantation, which was successfully retriev

Barrier to Entry: How Health Canada's Medical Device Single Audit Program Limits Patient Access to New Technology in Canada.

In this article, we look at the approval process for medical devices in Canada, and how the Medical Device Single Audit Program (MDSAP) has made it unfeasible for small manufactures to enter the Canadian market. Specifically, we explore how MDSAP has been a barrier in Canada for the entry of hypoglossal nerve stimulators (HGNS), a proven technology with a strong track record in other markets for the treatment of obstructive sleep apnea. This is ultimately preventing Canadian patients access to life-saving medical devices. We hope this commentary will shed light on this barrier and prompt Healt

The CAPTURE registry: predictors of outcomes in carotid artery stenting with embolic protection for high surgical risk patients in the early post-approval setting.

BACKGROUND: The use of carotid artery stenting with embolic protection has been practiced for over a decade in the United States, and increasingly so since carotid stenting received FDA approval in 2004. While there have been attempts at establishing predictors of outcomes in carotid artery stenting, they have generally been limited to single center experiences and/or multicenter retrospective surveys. This report examines predictors of outcomes in carotid stenting in the earliest and largest prospective multicenter neurologist-adjudicated experience in the United States post device approval.

Diaphragmatic pacing to facilitate ventilator weaning in neuromyelitis optica-associated respiratory failure.

Patients with neuromyelitis optica may develop neurogenic respiratory failure in the context of severe upper cervical myelitis, which can require prolonged or indefinite mechanical ventilation. Diaphragmatic pacing has FDA approval under a humanitarian device exemption (HDE) to facilitate weaning from mechanical ventilation in the context of cervical spinal cord injury. We describe the use of diaphragmatic pacing in a patient with acute respiratory failure due to a severe attack of neuromyelitis optica on the cervical spinal cord. The device resulted in successful early ventilator weaning.

Over-the-counter light therapy for acne: a cross-sectional retrospective analysis.

Acne vulgaris affects a large portion of the population and drives many patients to seek over-the-counter (OTC) treatments. Light-emitting diode (LED) therapy has recently emerged as a potential therapeutic option for inflammatory acne. We used the U.S. Food and Drug Administration (FDA) 510(k) premarket submission database to assess the growth in OTC LED therapy devices for treatment of acne. We also used Google Trends data in searches for "acne light therapy mask" to characterize growth in consumer interest in these devices. Overall, 35 LED devices received pre-market approval from 2000 to 2

Summary and Comparison of Atrial Septal Defect Closure Devices.

A large number of atrial septal defect (ASD) closure devices have been investigated in an attempt to develop a nonsurgical, transvenous method of occlusion of ASD. Some of the devices have been discontinued and several are in clinical trials at this time, but none are approved for general clinical use. There are no prospective, randomized clinical trials to compare the available devices. Based on separate clinical trials, the feasibility, safety, and effectiveness of all devices appear similar. Considerations pertaining to the size of the device delivery sheath, ease of implantation, cost and

The First 4 Years of Postmarketing Safety Surveillance Related to the MitraClip Device: A United States Food and Drug Administration MAUDE Experience.

OBJECTIVE: The MitraClip (Abbott) is a commercially available device to perform percutaneous transcatheter mitral valve repair (TMVR) for patients with symptomatic mitral regurgitation (MR). Recent data support its role in appropriately selected patients with functional MR, and its use is poised to increase. However, limited safety data in "real-world" practice are available after market introduction. METHODS: We queried all available adverse event reports from the publicly available Manufacturer and User Facility Device Experience (MAUDE) database including "injuries" and "deaths" from Octob

Off-label uses of the Pipeline embolization device: a review of the literature.

The Pipeline embolization device (PED) is the most widely used flow diverter in endovascular neurosurgery. In 2011, the device received FDA approval for the treatment of large and giant aneurysms in the internal carotid artery extending from the petrous to the superior hypophyseal segments. However, as popularity of the device grew and neurosurgeons gained more experience, its use has extended to several other indications. Some of these off-label uses include previously treated aneurysms, acutely ruptured aneurysms, small aneurysms, distal circulation aneurysms, posterior circulation aneurysms
