The importance of autopsy examination in major disasters.

It is appreciated that there are national and jurisdictional variations in regard to post-mortem dissections following accidental death. The purpose of this paper is to plead that, since there will be nationals of many states involved in an aircraft disaster, there should be a uniform system of forensic investigation on a world wide basis. The justification for this recommendation is illustrated through the investigation of accident causes and through the solution of the many problems relating to the settlement of probate which arise in accidental death. The causes of accidents which may be di

The Great East-Japan Earthquake and devastating tsunami: an update and lessons from the past Great Earthquakes in Japan since 1923.

Japan has a long history of fighting against great earthquakes that cause structural damage/collapses, fires and/or tsunami. On March 11, 2011 at 14:46 (Friday), the Great East-Japan Earthquake (magnitude 9.0) attacked the Tohoku region (northeastern Japan), which includes Sendai City. The earthquake generated a devastating tsunami, leading to unprecedented disasters (~18,500 victims) in coastal areas of Iwate, Miyagi and Fukushima prefectures, despite the fact that people living in the Tohoku region are well trained for tsunami-evacuation procedures, with the mindset of "Tsunami, ten-den-ko."

Obstetrical care and women's health in the aftermath of disasters: the first 14 days after the 2010 Haitian earthquake.

OBJECTIVE: Natural disasters disproportionately injure women and children. Disaster teams need intensive training in the management of obstetrics and women's healthcare at the disaster site. DESIGN: This article summarizes the obstetrical experience for the International Medical Surgical Response Team (IMSuRT) stationed at Gheskio in Port Au Prince during the first 2 weeks after the 2010 Haitian earthquake. The world's literature on the impact of disasters on women is reviewed. SETTING: Sixty-three members of the IMSuRT and Disaster Medical Assistance Team set up a mobile surgical field hosp

Managing indirect disaster-related deaths after the 2025 Myanmar earthquake: A public health proposal.

The 2025 Myanmar earthquake caused extensive destruction. Beyond 3,800 direct disaster-related deaths (direct DRDs) and 5,100 injuries, indirect disaster-related deaths (indirect DRDs) remain a persistent concern. Indirect DRDs are deaths caused by secondary health effects after a disaster, rather than by the direct physical impact of the hazard itself. They may arise through disrupted healthcare delivery, deteriorating living and evacuation environments, interruptions to chronic-disease management, and psychosocial stress. Evidence from Japan, including the 2011 Great East Japan Earthquake an

Earthquake-Related Orthopedic Injuries in Adult Population: A Systematic Review.

UNLABELLED: Introduction Earthquake-related trauma results in crush injuries and bony- and soft-tissue trauma. There are no systematic reviews analyzing the typical injury patterns and treatments in "Mega-Mass-Casualty" earthquakes. The characterization of an injury pattern specific to disaster type, be it natural or manmade, is imperative to build an effective disaster preparedness and response system. METHODS: The systematic review was conducted in accordance with Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA). A comprehensive search strategy was developed to id

Disaster hypertension: experience from the great East Japan earthquake of 2011.

After a major disaster, such as the East Japan Earthquake with ensuing tsunami and nuclear accident in March 2011, there is typically a spike in cardiovascular disease onset due to increased blood pressure and hypercoagulability. The risk of cardiovascular disease after an earthquake can be reduced by understanding the characteristics of these risks and taking appropriate preventive and remedial measures. During the East Japan Earthquake disaster, Disaster Cardiovascular Prevention (DCAP) Risk Scores (0-6 points; goal ≤4 points)/Prevention Scores (0-8 points; goal ≥ 6 points) were used to iden

Year in review 2009: Critical Care--cardiac arrest, trauma and disasters.

During 2009, Critical Care published nine papers on various aspects of resuscitation, prehospital medicine, trauma care and disaster response. One article demonstrated that children as young as 9 years of age can learn cardiopulmonary resuscitation (CPR) effectively, although, depending on their size, some may have difficulty performing it. Another paper showed that while there was a trend toward mild therapeutic hypothermia reducing S-100 levels, there was no statistically significant change. Another predictor study also showed a strong link between acute kidney injury and neurologic outcome

[Orthopedics and trauma surgery in national and collective defence : Resilient emergency and palliative care in crises and armed conflict situations].

BACKGROUND: The current European political security environment is very tense due to the return of conventional and hybrid warfare. National and collective defence have again become key political tasks. In this context, the resilience of the healthcare system, understood as critical infrastructures contributing to social cohesion, institutional trust and democratic stability, have moved to center stage. Orthopedics and trauma surgery (O&U) are central to injury care, while palliative medicine represents an essential component of humane and functional healthcare provision in crises and armed co

Natural disasters and communicable diseases in the Americas: contribution of veterinary public health.

The consequences of natural disasters on the people living in the Americas are often amplified by socio-economic conditions. This risk may be increased by climate-related changes. The public health consequences of natural disasters include fatalities as well as an increased risk of communicable diseases. Many of these diseases are zoonotic and foodborne diseases. The aim of this article is to provide an overview of the importance of natural disasters for the Americas and to emphasise the contribution of veterinary public health (VPH) to the management of zoonotic and foodborne disease risks. Meals ready to eat: a brief history and clinical vignette with discussion on civilian applications.

Meals ready to eat (MRE) have undergone many revisions of their origins in the trench ration from World War I. The MRE was implemented in 1980. Its design allows extended storage and easy, safe meal preparation. MRE sodium content varies by meal and may range from 1.6 g/meal to 2.3 g/meal. The average MRE contains 1,200 kcal. When consumed as intended, MREs are adequate for maintaining a soldier's physical parameters without undesirable consequences. The average soldier has a healthy cardiovascular system, has the ability to excrete high sodium loads, and has high insensible losses. The Americ

The penumbra of thalidomide, the litigation culture and the licensing of pharmaceuticals.

Fifty years ago several thousand children were born with severe limb defects after their mothers had been given thalidomide in pregnancy. This tragedy caused procedures for licensing new medicines to become much stricter. Where, nevertheless, significant side effects were found it became common to sue for damages. These consequences have caused possibly an even greater disaster damaging many more people and threatening ruin to health services everywhere. The huge increase in both time and cost in bringing medicines to market is increasing their price to unsupportable levels; and only wealthy c

Climate Change and Emergency Medicine: A Scoping Review Across Emergency Medicine Subspecialties.

INTRODUCTION: Climate change is reshaping emergency medicine (EM) practice through rising temperatures, extreme weather events, and deteriorating air quality. Emergency medicine serves as a critical frontline indicator for climate-sensitive health conditions, yet evidence describing climate impacts across EM subspecialties remains fragmented. This scoping review synthesizes existing literature at the intersection of climate change and EM to identify key findings, knowledge gaps, and priorities for building climate-resilient emergency care systems. METHODS: We conducted a scoping review with r

Sleep Dynamic Therapy for Cerro Grande Fire evacuees with posttraumatic stress symptoms: a preliminary report.

BACKGROUND: Sleep disturbance is common among disaster survivors with posttraumatic stress symptoms but is rarely addressed as a primary therapeutic target. Sleep Dynamic Therapy (SDT), an integrated program of primarily evidence-based, nonpharmacologic sleep medicine therapies coupled with standard clinical sleep medicine instructions, was administered to a large group of fire evacuees to treat posttraumatic insomnia and nightmares and determine effects on posttraumatic stress severity. METHOD: The trial was an uncontrolled, prospective pilot study of SDT for 66 adult men and women, 10 month

Follow-up after the Hanshin-Awaji earthquake: diverse influences on pneumonia, bronchial asthma, peptic ulcer and diabetes mellitus.

We evaluated the influence of the Hanshin-Awaji earthquake on the patients in our internal medicine department. After the initial rush of patients with injury, the number of respiratory diseases, largely pneumonia, increased within one month. This same event, however, seemed to decrease attacks among asthma patients. During the following three months, the number of peptic ulcer patients increased: 39.5% had a giant gastric ulcer and 34.8% had bleeding complications. Diabetic control of outpatients became worse after the earthquake. It is important to recognize that various disorders involving

Post-earthquake life and health experiences of 3-6-year-old children through the eyes of mothers: a qualitative descriptive study.

BACKGROUND: Earthquakes are among the most devastating disasters, profoundly affecting children's physical, psychological, and social well-being. Preschool children aged 3-6 are particularly vulnerable due to their developmental characteristics. This study aimed to explore the life and health experiences of children aged 3-6 years following the 2023 Kahramanmaraş earthquakes. METHODS: A qualitative descriptive exploratory study was conducted with 21 mothers who had personally experienced the earthquakes, were still residing in the affected region, and were recruited through snowball sampling.

PTSD as the second tsunami of the SARS-Cov-2 pandemic.

Since the first cases, the coronavirus disease (COVID-19) rapidly spread around the world, with hundred-thousand cases and thousands of deaths. Post-traumatic stress disorder (PTSD) is a common consequence of major disasters. Exceptional epidemic situations also promoted PTSD in the past. Considering that humanity is undergoing the most severe pandemic since Spanish Influenza, the actual pandemic of COVID-19 is very likely to promote PTSD. Moreover, COVID-19 was renamed severe acute respiratory syndrome coronavirus 2 (SARS-Cov-2). With a poor understanding of viruses and spreading mechanisms,

World Trade Center psychological exposures and trauma related disorders: PTSD and adjustment disorders.

The relationship between exposure to the World Trade Center (WTC) disaster and elevated rates of trauma related psychiatric illnesses in 9/11 responders and survivors has been well documented. This paper is part of a series to promote the practice of evidence-based medicine when managing persons with WTC-related conditions and focuses on "Trauma and Stressor Related Disorders," a diagnostic category that includes posttraumatic stress disorder (PTSD) and adjustment disorder. It offers background on 9/11-related trauma exposure, a summary of research findings from this cohort, and is followed by

[Expert consensus on rabies post-exposure prophylaxis in children (2025 edition)].

Children constitute a high-risk population for rabies exposure due to their physiological and psychological characteristics and limited self-protection abilities, making them particularly vulnerable to severe bites on the head, face, or multiple body parts, which significantly increases the risk of disease onset. Furthermore, rabies post-exposure prophylaxis in children exhibits distinct features in wound management, rabies vaccination, and the use of passive immunizing agents. To address the current challenges of inconsistent awareness and suboptimal practices in rabies exposure prevention fo
