A 56-year-old male with a history of non-adherence to HIV anti-retroviral therapy (ART) presented with Elsberg syndrome - varicella reactivation causing fever, painful dermatomal rash, weakness of bilateral lower extremities, and urinary and bowel dysfunction. On the third day of hospitalization, the patient developed altered mental status. An investigation for encephalitis and myelitis revealed a CD4 count of 150 cells/uL, viral load of about 150,000 copies/mL, and MRI of the lumbar spine demonstrating thickening of the cauda equina. Cerebrospinal fluid (CSF) from lumbar puncture confirmed th

Rift valley fever virus (RVFV) is among the WHO list of priority diseases, yet no effective vaccine or treatment is currently available. Microbial pigments (MPs) represent a promising small-molecules library which can be exploited for the drug discovery of anti-RVFV compound. In this study, thirteen MPs were in silico screened to identify candidates with acceptable drug-likeness and possible ability to cross the blood brain barrier. Next, the binding interaction of the filtered molecules were compared against key RVFV proteins for the selection of the optimum inhibitor. Molecular dynamics simu

Severe fever with thrombocytopenia syndrome (SFTS) is an emerging tick-borne infectious disease caused by SFTS virus (SFTSV), which is a novel bunyavirus. SFTSV was first isolated from patients who presented with fever, thrombocytopenia, leukocytopenia, and multiorgan dysfunction in China. Subsequently, it was found to be widely distributed in Southeast Asia (Korea, Japan, and Vietnam). SFTSV can be transmitted not only from ticks but also from domestic animals, companion animals, and humans. Because the case fatality rate of SFTS is high (6-30%), development of specific and effective treatmen

We describe two patients who developed granulocytopenia (granulocyte count < 1.0 x 10(9)/1) after treatment with antithyroid drugs. Although, perhaps, an already known side-effect of these drugs, we present these cases in order to stay alert for this serious complication. The first patient developed granulocytopenia upon reinstitution of propylthiouracil after an uncomplicated first course of ATD-treatment. In the second case we report the use of granulocyte colony-stimulating factor (G-CSF) for treatment of a patient with methimazole-induced granulocytopenia. Furthermore, we discuss current c

Severe fever with thrombocytopenia syndrome (SFTS), caused by the SFTS virus (SFTSV), has emerged as a significant global public health threat. Infected patients may present with gastrointestinal, neurological, and cardiovascular ribavirin and favipiravir are currently used in clinical practice, their efficacy remains controversial, and treatment primarily relies on symptomatic and supportive care. To date, there is no standard treatment regimen for SFTSV infection, nor are there any approved vaccines. However, recent advances in SFTSV research and the application of novel technologies have op

Dengue virus represents a global public health threat, being prevalent in tropical and subtropical regions, with an increasing geographical distribution and rising incidence worldwide. This mosquito-borne viral agent causes a wide range of clinical manifestations, from mild febrile illness to severe cases and potentially fatal outcomes due to hemorrhage and shock syndrome. The etiological agent, dengue virus (DENV), has four distinct serotypes, each capable of inducing severe clinical outcomes. The current therapeutic landscape remains limited, with management strategies mainly focused on supp

Brucellosis, a zoonotic ailment induced by the Brucella and some patients may present with joint involvement. This report describes a pediatric patient diagnosed with Brucella arthritis, presenting with swelling and pain in the right knee. The patient had a reoccurrence of fever due to sulfamethoxazole-trimethoprim allergy during treatment. Symptoms improved after adjusting the antimicrobial regimen to ceftriaxone and rifampicin. This case emphasizes the importance of the need for brucellosis as a differential diagnosis for arthralgia and fever in brucellosis- endemic areas. Furthermore, it em

A 55-year-old Japanese man was hospitalized with the novel coronavirus disease 2019 (COVID-19). On the 14th day after the start of favipiravir administration, the patient developed a fever with a temperature of 38.1°C. His pulse rate also became elevated to 128 bpm, so relative bradycardia was not suspected. Since he was in good overall health and no concomitant symptoms and signs were apparent, we considered it to be drug fever due to favipiravir. After the completion of favipiravir treatment, the patient's temperature normalized within 24 hours. We herein report this case of drug fever cause

Rift Valley fever virus (RVFV) and Crimean-Congo hemorrhagic fever virus (CCHFV) are designated by the World Health Organization as priority pathogens due to their epidemic potential, zoonotic transmission, and the absence of licensed therapeutics or vaccines. The development of effective antivirals critically relies on robust in vitro and in vivo models; however, progress is limited by the requirement for high-containment facilities. In this review, we provide a comprehensive overview of the experimental models currently available for RVFV and CCHFV, ranging from cell-based assays to animal m

INTRODUCTION: Drug-induced fever is a common but often under-recognized adverse reaction that can complicate clinical management.
AIM: This study aimed to systematically evaluate the risk of drug-induced fever using real-world pharmacovigilance data and to characterize its temporal onset patterns and clinical features.
METHOD: A pharmacovigilance study was conducted utilizing data from the FAERS, covering reports from the first quarter of 2004 to the fourth quarter of 2024. The detection of adverse drug event signals was conducted using disproportionality analysis. The robustness of identifi

Adverse drug reactions to first-line antituberculosis (anti-TB) therapy are common and may necessitate treatment interruption. While drug desensitization is increasingly used for delayed hypersensitivity, evidence supporting outpatient rapid oral desensitization in mild delayed reactions such as drug-induced fever is limited. We report a 63-year-old man with active pulmonary tuberculosis who developed drug-induced fever after commencing standard anti-TB therapy. Sequential and additive drug challenge identified rifampicin as the offending agent. The patient subsequently underwent successful ra

Tropical infectious diseases cause millions of deaths every year in developing countries, with about half of the world population living at risk. Mayaro virus (MAYV) is an emerging arbovirus that causes Mayaro fever, which is characterized by fever, headache, diarrhea, arthralgia, and rash. These symptoms can be clinically indistinguishable from other arboviruses, such as Dengue, Zika, and Chikungunya, which makes the diagnosis and treatment of the disease more difficult. Though, the Mayaro virus is a potential candidate to cause large-scale epidemics on the scale of ZIKV and CHIKV. Despite th

BACKGROUND: Antimicrobial resistance (AMR) is an increasing threat to global health. There are > 14 million cases of enteric fever every year and > 135,000 deaths. The disease is primarily controlled by antimicrobial treatment, but this is becoming increasingly difficult due to AMR. Our objectives were to assess the prevalence and geographic distribution of AMR in Salmonella enterica serovars Typhi and Paratyphi A infections globally, to evaluate the extent of the problem, and to facilitate the creation of geospatial maps of AMR prevalence to help targeted public health intervention.

Yellow fever virus (YFV) is responsible for devastating outbreaks of Yellow fever (YF) in humans and is associated with high mortality rates. Recent large epidemics and epizootics and exponential increases in the numbers of YF cases in humans and non-human primates highlight the increasing threat YFV poses, despite the availability of an effective YFV vaccine. YFV is the first human virus discovered, but the structures of several of the viral proteins remain poorly understood. Here we report the structure of the full-length NS5 protein, a key enzyme for the replication of flaviviruses that con

There is paucity of data regarding the geographical distribution, incidence, and phylogenetics of multi-drug resistant (MDR) Salmonella Typhi in sub-Saharan Africa. Here we present a phylogenetic reconstruction of whole genome sequenced 249 contemporaneous S. Typhi isolated between 2008-2015 in 11 sub-Saharan African countries, in context of the 2,057 global S. Typhi genomic framework. Despite the broad genetic diversity, the majority of organisms (225/249; 90%) belong to only three genotypes, 4.3.1 (H58) (99/249; 40%), 3.1.1 (97/249; 39%), and 2.3.2 (29/249; 12%).
