The use of the reverse shoulder prosthesis for the treatment of failed hemiarthroplasty for proximal humeral fracture.

BACKGROUND: Humeral hemiarthroplasty is an established treatment for patients with selected fractures of the proximal part of the humerus. However, a subset of patients have development of glenoid arthritis and rotator cuff deficiency due to tuberosity failure. To date, there has been no reliable salvage procedure for this problem. METHODS: Over a period of five years, twenty-nine patients (twenty-five women and four men) with a mean age of sixty-nine years (range, forty-two to eighty years) were managed with removal of a hemiarthroplasty prosthesis and revision with a Reverse Shoulder Prosth

Prosthetic limb use in Australia 1981-1985 under the Free Limb Scheme.

A study has been made of data on 19,421 prosthetic limbs prescribed for 12,143 Australians under the Free Limb Scheme in the years 1981-1985. These prostheses consisted of 18,119 legs and 1,302 arms. The mean age of the lower limb prosthesis user was 52.8 and the upper limb prosthesis user 31.3 years. Males outnumbered females by 3 to 1 in the upper limb prosthesis users, and 2.8 to 1 with lower limb prosthesis users. Below-knee prostheses, patellar-tendon-bearing and thigh-lacing prostheses, made up 58.7% of all prostheses prescribed in the time span. Below-elbow prostheses were the commonest

Comparison of bipolar and monopolar radial head prostheses in elbow fracture-dislocation.

INTRODUCTION: The role of bipolar radial head prostheses (RHP) in elbow fracture-dislocation is controversial, with some reports of poorer stabilization than with monopolar designs. The aim of the present study was to compare mono- versus bi-polar RHPs in elbow fracture-dislocation. The study hypothesis was that mono- and bi-polar RHPs do not differ in clinical and radiological results, complications or revision rates. MATERIAL AND METHODS: A single-center retrospective study included 58 patients, with a mean age of 55 years (range, 21-84 years). All received RHP for elbow dislocation with as

[A bipolar radial head prosthesis after comminuted radial head fractures: indications, treatment and outcome after 5 years].

The treatment of comminuted fractures of the radial head with concomitant injuries of the ulnar complex by resection of the radial head usually does not provide satisfactory long-term results. Other than joint instability in the elbow and a limited range of motion, radius proximalisation in the sense of ulnocarpal impingement, osteoarthritis and pain in the elbow have been described. Between 1995 and 1997, 11 radial head prostheses were implanted in ten patients who had sustained a comminuted fracture of the radial head with concomitant injury to the ulnar complex. A follow-up survey was condu

Lengthening of congenital below-elbow amputation stumps by the Ilizarov technique.

Patients with short congenital amputations below the elbow often function as if they have had a disarticulation of the elbow. We have reviewed the results in six patients who had lengthening of such stumps by the Ilizarov technique to improve the fitting of prostheses. The mean lengthening was 5.6 cm (3.4 to 8.4), and in two patients flexion contractures of the elbows were corrected simultaneously. Additional lateral distraction was used in one patient to provide a better surface on the stump. There were no major complications. All six patients were able to use their prosthesis at the latest f

Treatment of comminuted radial head fractures with personalized radial head prosthesis produced with 3-dimensional printing technology.

BACKGROUND: The radial head is the forearm's pivot point for supination-pronation. Although radial head resection can be performed in comminuted radial head fractures, radial head prosthesis has gained popularity in order to prevent possible complications such as progressive valgus instability in the elbow and secondary ulnocarpal impaction that may develop due to proximal displacement of the radius. As for radial head prostheses, standard prosthetic designs do not provide the same results in everyone, and the alignment of the radial head and radial neck angle cannot be fully achieved. We beli

Clinical and radiologic outcomes of pyrocarbon radial head prosthesis: midterm results.

BACKGROUND: The modular pyrocarbon (MoPyC) radial head prosthesis (Tornier, Saint-Ismier, France) is a monoblock modular radial head prosthesis. This study assessed midterm outcomes after implantation of the prosthesis. MATERIALS: A retrospective study was conducted of a consecutive cohort of 65 patients who underwent radial head replacement with the MoPyC prosthesis from January 2006 to April 2013. Indications were fractures, early or late failures from orthopedic or fixation treatments, and revisions after another implant. Patients were observed for >2 years for range of motion, pain, and s

Displaced, unstable fractures of the radial head: fixation vs. replacement--what is the evidence?

Unstable, displaced fractures of the radial head nearly always occur in association with other fractures or ligament injuries. Radiocapitellar contact is important to elbow and forearm stability and should be restored in the context of such injuries. Open reduction and internal fixation of the radial head is associated with early failure, nonunion, and poor forearm motion. Replacement of the radial head with a metal prosthesis may be preferable. The major pitfall of a metal prosthesis is insertion of a prosthesis that is too large in longitudinal length, which can lead to subluxation, capitell

Customized 3D-Printed Resurfacing Arthroplasty for the Treatment of Severely Malunited Distal Humerus Associated With Posttraumatic Elbow Stiffness: A Case Report and Review of the Literature.

BACKGROUND: Posttraumatic elbow stiffness together with severe bony deformity of the distal humerus poses a great challenge for elbow surgery specialists. For young and high-demand patients suffering from grievous bony deformity but with no/mild joint degeneration, total elbow arthroplasty is not a suitable option, and hemiarthroplasty often results in a significant amount of bone stock loss, making it difficult to perform a revision procedure. Resurfacing arthroplasty has been applied in the shoulder, hip, and knee joints, as well as the radiocapitellar joint. However, resurfacing arthroplast

Novel Coronoid Reconstruction Using a Patient-Specific 3-Dimensional Metal-Printed Prosthesis and Guide for Comminuted Coronoid Process Fractures.

BACKGROUND: The coronoid process is essential for elbow stability, and its severe comminuted fractures may require coronoid reconstruction. While coronoid reconstruction using autologous bone grafts has shown positive clinical outcomes, it has inherent limitations that create uncertainty in its outcomes. To our knowledge, while the potential for coronoid prostheses has emerged, there are currently no commercially available options, and clinical results remain insufficient. Thus, this study aimed to develop a new patient-specific coronoid prosthesis using 3-dimensional (3D) metal printing techn

Salvage of elbow function in chronic complex elbow fracture dislocation with total elbow arthroplasty: a case report.

In patients with an elbow fracture dislocation the incidence of radial head fracture is 36%, where as coronoid process fractures occur in 13%, and olecranon fractures in 4% of patients. Combination of all these fractures with a 'terrible triad' is rarely reported in the literature. We describe a 40 year old lady involved in a polytrauma who had head injury, pnuemothorax and an open fracture dislocation of the left elbow. The Injury Severity Score initially on admission was 44. She presented with chronic elbow instability with pain 1 year later. A semi constrained total elbow arthroplasty (TEA)

Acute disassembly of a bipolar radial head arthroplasty.

The GUEPAR(®) implant is a metallic bipolar radial head prosthesis designed to treat comminuted radial head fractures when anatomic realignment of the articular surface of the radiocapitellar joint is not possible. We report herein the rare case of an acute complete disassembly of this implant, discuss the reason for this occurrence and review the literature. In the presented case, the complete removal of the prosthesis provided a satisfactory outcome with an excellent Mayo Elbow Performance Score at 12months follow-up.

Radial head replacement with the MoPyC pyrocarbon prosthesis.

BACKGROUND: Radial head fractures often pose therapeutic dilemmas. We present the early results of patients who underwent radial head replacement with the MoPyC prosthesis (Bioprofile, Tornier, Saint-Ismier, France). MATERIALS AND METHODS: We re-evaluated patients who underwent post-traumatic radial head resection and implantation of the MoPyC prosthesis due to pain and motion restriction. All patients underwent radiographic evaluation. Clinical evaluation was performed using the Broberg-Morrey and the Mayo Elbow Performance Score (MEPS) scales. RESULTS: Thirty-two patients (20 men, 12 women

Instability of the elbow treated with semiconstrained total elbow arthroplasty.

The results of nineteen semiconstrained modified Coonrad-Morrey total elbow arthroplasties performed in nineteen patients to treat instability were evaluated at an average of seventy-two months (range, twenty-five to 128 months) postoperatively. Preoperatively, all patients had either a flail elbow or gross instability of the elbow that prevented useful function of the extremity. The instability of sixteen elbows was the result of a traumatic injury or of the treatment of such an injury. The most recent result was satisfactory for sixteen elbows and unsatisfactory for three. The average overal
