Removal of Massive Left Atrial Appendage Closure Device-Related Thrombus With Complete Embolic Protection: Step-by-Step Technique.

OBJECTIVE: Device-related thrombus (DRT) is a life-threatening complication of left atrial appendage closure devices. Transcatheter vacuum-assisted thrombectomy has recently been described as a novel strategy for removal of large DRT, but it is associated with increased risk of systemic embolism. We describe a step-by-step technique of a complete embolic protection strategy in patients undergoing vacuum-assisted thrombectomy. KEY STEPS: First, bilateral cerebral embolic protection devices are deployed. Next, systemic embolic protection is achieved by positioning an ONO retrieval device (Onoco

Cerebral embolic protection device utilization and outcomes in transcatheter aortic valve replacement: A nationally representative propensity matched analysis.

INTRODUCTION: In patients undergoing transcatheter aortic valve replacement (TAVR), cerebral embolic protection devices (CEPD) are used to possibly diminish the risk of periprocedural stroke. Trends and outcomes of CEPD usage in TAVR are not well characterized. METHODS: National readmission databases (NRD) 2017-2019 was used to identify hospital admissions for TAVR using ICD-10 codes, with versus without Sentinel CEPD. Primary outcomes of the study were in-hospital and 30-day stroke. Secondary outcomes include in-hospital mortality, 30-day mortality, 30-day readmission rate, and other procedu

Transcranial Doppler monitoring during stenting of the carotid bifurcation: evaluation of two different distal protection devices in preventing embolization.

PURPOSE: To compare the efficacy of 2 emboli protection devices in preventing embolization during carotid artery stenting (CAS). METHODS: The GuardWire distal occlusion system (n=19) and the distal FilterWire EX (n=12) were compared in 31 consecutive patients (24 men; mean age 71+/-10 years) monitored with transcranial Doppler for microembolic signals before, during, and after CAS. The choice of the protection device was based on availability and on the patency of the contralateral carotid artery. RESULTS: The baseline characteristics were similar in the patients treated under protection fro

A randomized, controlled trial of saphenous vein graft intervention with a filter-based distal embolic protection device: TRAP trial.

The purpose of this prospective, multicenter trial was to evaluate the safety and effectiveness of the TRAP Vascular Filtration System (VFS) to reduce embolic complications during stenting of diseased saphenous vein grafts (SVGs). Patients with SVG lesions were randomly assigned to undergo stenting with or without the TRAP device. The trial was designed to enroll 752 randomized patients. However, the sponsor terminated the study after a total of 467 patients (358 randomized) were enrolled because of poor recruitment once another distal protection device was approved for clinical use. The prima

Randomized trial of a distal embolic protection device during percutaneous intervention of saphenous vein aorto-coronary bypass grafts.

BACKGROUND: Stents provide effective treatment for stenotic saphenous venous aorto-coronary bypass grafts, but their placement carries a 20% incidence of procedure-related complications, which potentially are related to the distal embolization of atherosclerotic debris. We report the first multicenter randomized trial to evaluate use of a distal embolic protection device during stenting of such lesions. METHODS AND RESULTS: Of 801 eligible patients, 406 were randomly assigned to stent placement over the shaft of the distal protection device, and 395 were assigned to stent placement over a con

Association of Transcatheter Aortic Valve Replacement Reimbursement, New Technology Add-on Payment, and Procedure Volumes With Embolic Protection Device Use.

IMPORTANCE: In the setting of uncertain efficacy and additional, unreimbursed cost, use of an embolic protection device (EPD) during transcatheter aortic valve replacement (TAVR) has had variable uptake. The Centers for Medicare & Medicaid Services (CMS) instituted a new technology add-on payment to cover EPD use in October 2018. OBJECTIVE: To evaluate the association between CMS TAVR reimbursement rates and EPD use. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used the Society for Thoracic Surgeons/American College of Cardiology Transcatheter Valve Therapy registry to identify patie

Safety and Efficacy of Carotid Stenting Without Embolic Protection Device in Delayed Window Period: Experience From A Tertiary Care Center.

BACKGROUND AND OBJECTIVES: Cerebrovascular disease is the fifth leading cause of death in the United States. Ischemic stroke, often due to atherosclerotic carotid artery stenosis, carries a high risk of recurrence. Preventive options for stroke include medical management, carotid endarterectomy (CEA), and carotid artery stenting (CAS). CAS offers comparable safety and efficacy to CEA, but the role of embolic protection devices (EPDs) is still unclear. The study aims to evaluate the safety and efficacy of CAS without EPDs in symptomatic carotid artery stenosis, focusing on periprocedural compli

Guide Extension and Balloon-Assisted Retrieval of an Entrapped Embolic Protection Device Within a Coronary Stent: A Case Report and In Vitro Analysis.

The entrapment of an embolic protection device (EPD) within a coronary stent is a rare but serious complication for which established bailout strategies are lacking. Herein, we report a case of stent deformation caused by EPD entrapment at the distal stent edge. We conducted an in vitro bench study to elucidate the mechanism of entrapment and present the novel, mechanically optimized bailout strategy we used to treat our patient. A 65-year-old man underwent percutaneous coronary intervention for attenuated plaque in the right coronary artery. EPD entrapment and subsequent stent deformation occ

Embolic protection during saphenous vein graft intervention using a second-generation balloon protection device: results from the combined US and European pilot study of the TriActiv Balloon Protected Flush Extraction System.

BACKGROUND: Stent placement in saphenous vein bypass grafts is associated with a high incidence of myonecrosis usually resulting from embolization of thrombus and friable atheroma. Embolic protection devices reduce the incidence of adverse events after vein graft stenting. However, first-generation balloon occlusion systems are still associated with a 10% incidence of periprocedural adverse events. We report the first experience with a new second-generation balloon occlusion embolic protection device, the TriActiv Balloon Protected Flush Extraction System. METHODS: Ninety-six lesions in 78 sa

An Updated Meta-Analysis on Cerebral Embolic Protection in Patients Undergoing Transcatheter Aortic Valve Intervention Stratified by Baseline Surgical Risk and Device Type.

BACKGROUND: Transcatheter aortic valve intervention (TAVI) can lead to the embolization of debris. Capturing the debris by cerebral embolic protection (CEP) devices may reduce the risk of stroke. New evidence has allowed us to examine the effects of CEP in patients undergoing TAVI. We aimed to assess the effects of CEP overall and stratified by the device used (SENTINEL or TriGuard) and the surgical risk of the patients. METHODS: We selected randomized controlled trials using electronic databases through September 17, 2022. We estimated random-effects risk ratios (RR) with (95% confidence int
