While endolymphatic hydrops is a characteristic pathologic feature of Menière's disease, there are exceptions to this rule. There is evidence that hydrops develops as a result of malabsorption of endolymph. This implies dysfunction of the endolymphatic sac and duct, which normally absorb endolymph. In approximately 20% of cases of Menière's disease, a specific pathologic condition in the temporal bone can be associated as a cause of endolymphatic hydrops. Syphilis, fractures of the temporal bone, otosclerosis, and preceding chronic otitis media are some of the more commonly encountered patholo

To visualize endolymph, perilymph, and bone separately in a single image after intravenous injection of single-dose gadodiamide, we generated a HYDROPS2 image (subtraction of MR cisternography from heavily T(2)-weighted 3-dimensional fluid-attenuated inversion recovery image) in 12 patients with suspected Ménière's disease. The contrast-to-noise ratio of endo- and perilymph did not differ significantly between the HYDROPS2 and previously reported HYDROPS (hybrid of reversed image of positive endolymph signal and native image of positive perilymph Signal) images, and scan time was 40% shorter f

OBJECTIVES: Three-tesla MRI with gadolinium-based contrast agents is important in diagnosing Ménière's disease. However, contrast agents cannot be used in some patients. By using the compositional difference between the inner ear endolymph and perilymph, we performed basic and clinical research focused on potassium ions and protein to find the optimal parameters for visualizing endolymphatic hydrops on MRI without contrast. We then examined the relationship between severity stage and visualization rate of endolymphatic hydrops.
METHODS: In phantom experiments simulating the endolymph and peri

The pattern of response decline following semicircular canal stimulation corresponds to a mechanical process. This process is the elastic recoil of the hairs of the sensory cells. The response decline is the same, whether the endolymph-cupula system is patent or mechanically maintained in a displaced position. The role of the bending of the entire cupula as the principle factor is excluded for the response and its decline. Repeated endolymph displacements immediately after each decline of response always result in an equal response and its decline. This rules out the depletion of a chemical me

Spontaneous otoacoustic emissions (SOAEs) are indicators of active processes in the inner ear and are found in all classes of land vertebrates. In the Australian bobtail lizard, earlier work showed that otoacoustic emissions are generated by an active motility process in the hair-cell bundle. This is likely to be driven by calcium-sensitive mechanisms implicated in other non-mammalian hair cell systems. If so, it should be fundamentally influenced by the extracellular calcium concentration. In in vitro studies, the rate of force generation in hair cell stereovilli is linked to the extracellula

The angular accelerometer of vertebrates, the semi circular canal, is a pressure gage. The transformation, acceleration to pressure, is due to the inertia of the endolymph. In the 0 to 1 Hz frequency interval, the endolymph-cupula system can be described by a second order, localised parameter model with two real time constants. This mechanical model explains the occurr]nces of the vestibular-ocular reflex during rotatory stimulations. However, some features of the response to caloric stimulations cannot be explained from this unique point of view. To correspond to these features, the SCC has t

BACKGROUND: Sound waves stimulate the saccule. Therefore, sound waves are transmitted from the perilymph to the endolymph, causing the otolithic membrane of the saccule to vibrate and the cilia of the hair cells to move. Nevertheless, dizziness does not occur in response to auditory stimuli. To investigate this mystery, we examined the presence or absence of dizziness and nystagmus when loud sounds were presented to healthy individuals and reconsidered the auditory reception mechanism.
METHODS: Ten healthy individuals without ear pathology were studied. Stapedial reflex testing was performed

Due to the development of magnetic resonance (MR) imaging processing technology, image-based identification of endolymphatic hydrops (EH) has played an important role in understanding inner ear illnesses, such as Meniere's disease or fluctuating sensorineural hearing loss. We segmented the inner ear, consisting of the cochlea, vestibule, and semicircular canals, using a 3D-based deep neural network model for accurate and automated EH volume ratio calculations. We built a dataset of MR cisternography (MRC) and HYDROPS-Mi2 stacks labeled with the segmentation of the perilymph fluid space and end

Cochlear sensory transduction depends on active extrusion of sodium ion (Na(+)) from the luminal fluid, endolymph. Reissner's membrane epithelium forms much of the barrier between cochlear endolymph and perilymph and we hypothesized that Reissner's membrane might be responsible for this function. We found that Reissner's membrane isolated from gerbil produced a short circuit current (I(sc)) directed into the apical side, consistent with cation absorption and/or anion secretion. I(sc) was inhibited by amiloride analogs in the potency sequence benzamil>amiloride>>ethylisopropylamiloride, consist

In animals, the enhancement of perilymph in the cochlea has been reported using 1.25 mmol/kg of Gd-DTPA, allowing the separate visualisation of perilymph and endolymph for the diagnosis of Meniere's disease. The purpose of this study was three-fold: (1) to determine the optimal timing for detecting cochlear fluid enhancement using 3D-FLAIR (fluid-attenuated inversion recovery) after intravenous administration of 0.1 mmol/kg of Gd-DTPA in healthy human subjects; (2) to examine the reliability of enhancement in multiple healthy subjects; and (3) to investigate whether endolymph and perilymph spa

Ear is a complex system where appropriate ionic composition is essential for maintaining the tissue homeostasis and hearing function. Ion transporters and channels present in the auditory system plays a crucial role in maintaining proper ionic composition in the ear. The extracellular fluid, called endolymph, found in the cochlea of the mammalian inner ear is particularly unique due to its electrochemical properties. At an endocochlear potential of about +80 mV, signaling initiated by acoustic stimuli at the level of the hair cells is dependent on the unusually high potassium (K+ ) concentrati

Deafness can result from a variety of gene defects. Some genes involved in the physiology of hearing encode membrane transporters that regulate the ionic composition of the fluid bathing the inner ear. The endolymph is an extracellular fluid with an atypical composition that resembles the intracellular milieu, high in K+ and low in Na+. Recent studies have emphasized the prominent role of K+ channels in endolymph secretion and mechanical transduction. Coupled electroneutral transport of Na+, K+ and Cl- is mediated by two isoforms of the Na-K-2Cl co-transporter: the absorptive isoform BSC1 (als

A study was made of the anatomical structure and functional significance of the ultriculo-endolymphatic (UE) valve in man. The material consists of 170 human temporal bones of which 75 were chosen to show normal structure through nine decades of life, 29 with endolymphatic hydrops, 22 with developmental defects and 44 with pertinent staining characteristics of the endolymphatic fluid. The studies show that the UE valve is ideally suited to preserve the humoral and anatomical features of the pars superior (utricle and canals) from the developmental, disease, and traumatic susceptibilities of th

The intercellular space in the stria vascularis (intrastrial space) is a closed space and isolated from both the endolymph and the perilymph in normal tissue. Loop diuretics such as bumetanide and furosemide cause an acute enlargement of the intrastrial space in association with a decline in the endocochlear potential. It is known that bumetanide inhibits the Na+-K+-2Cl- cotransporter, which is expressed abundantly in the basolateral membrane of marginal cells. We studied ionic mechanisms underlying the bumetanide-induced enlargement of the intrastrial space using perilymphatic perfusion in gu

HYPOTHESIS: High-resolution magnetic resonance imaging (MRI) at 1.5 T preferentially enhances the perilymph over endolymph after administration of contrast with gadodiamide, which allows for differentiation of the membranous labyrinth. Furthermore, this imaging allows for the detection of endolymphatic hydrops.
BACKGROUND: Endolymphatic hydrops is believed to be associated with a number of ear diseases, including Ménière's disease. Although the pathologic changes of overaccumulation of endolymph in the inner ear are obvious on postmortem histologic examination of the temporal bone, they have

The coexistence of otosclerosis and endolymphatic hydrops in the temporal bone have been described; however, the mechanism for the development of endolymphatic hydrops in otosclerosis remains unknown. Among 128 temporal bones with otosclerosis, involvement of the vestibular aqueduct by otosclerosis was observed in four temporal bones from two patients. In all four, the vestibular aqueduct was filled with active otosclerotic foci; the lumen of the endolymphatic duct and sac was narrowed as a result of fibrosis, and endolymphatic hydrops, more severe in the pars inferior than the pars superior,

A conception of the mechanism for initiating symptoms in Ménière's disease has been forwarded. According to this conception production of endolymph may vary also in normals. The hydrops in Ménière cases is brought about by a relative insufficiency in endolymphatic draining routes and the hydrops presents itself only at peak endolymph production periods. The attacks of vertigo are brought about at the distension of the sacs as well as of the regaining of normal shape of the sacs. The mechanism causing the vertigo could be an effect of mechanical push-and-pull effect on the sensory epithelium of

Ménière's disease (MD) is a chronic disorder of the inner ear characterized by vertigo, hearing loss, tinnitus, and aural fullness. The pathophysiology of MD involves endolymphatic hydrops, an abnormal accumulation of endolymph fluid, although the exact cause remains unclear, potentially involving genetic, environmental, and autoimmune factors. Recent advancements in magnetic resonance imaging have significantly enhanced diagnostic capabilities. This technique uses gadolinium-based contrast agents to differentiate between endolymph and perilymph. Imaging techniques such as 3-dimensional fluid-
