Modeling the potential impact of vaccination on the epidemiology of congenital cytomegalovirus infection.

BACKGROUND: Understanding the potential for vaccination to change cytomegalovirus (CMV) epidemiology is important for developing CMV vaccines and designing clinical trials. METHODS: We constructed a deterministic, age-specific and time-dependent mathematical model of pathogen transmission, parameterized using CMV seroprevalence from the United States and Brazil, to predict the impact of vaccination on congenital CMV infection. FINDINGS: Concurrent vaccination of young children and adolescents would result in the greatest reductions in congenital CMV infections in populations with moderate an

Creating a process for incorporating epidemiological modelling into outbreak management decisions.

Modern computational models of infectious diseases greatly enhance our ability to understand new infectious threats and assess the effects of different interventions. The recently-released CDC Framework for Preventing Infectious Diseases calls for increased use of predictive modelling of epidemic emergence for public health preparedness. Currently, the utility of these technologies in preparedness and response to outbreaks is limited by gaps between modelling output and information requirements for incident management. The authors propose an operational structure that will facilitate integrati

Studies on Prn variation in the mouse model and comparison with epidemiological data.

The virulence factor pertactin (Prn) is a component of pertussis vaccines and one of the most polymorphic Bordetella pertussis antigens. After the introduction of vaccination shifts in predominant Prn types were observed and strains with the Prn vaccine type (Prn1) were replaced by strains carrying non-vaccine types (Prn2 and Prn3), suggesting vaccine-driven selection. The aim of this study was to elucidate the shifts observed in Prn variants. We show that, although Prn2 and Prn3 circulated in similar frequencies in the 1970s and 1980s, in the 1990s Prn2 strains expanded and Prn3 strains disap

Monitoring epidemiologic surveillance data using hidden Markov models.

The analysis of routinely collected surveillance data is an important challenge in public health practice. We present a method based on a hidden Markov model for monitoring such time series. The model characterizes the sequence of measurements by assuming that its probability density function depends on the state of an underlying Markov chain. The parameter vector includes distribution parameters and transition probabilities between the states. Maximum likelihood estimates are obtained with a modified EM algorithm. Extensions are provided to take into account trend and seasonality in the data.

A random effects model for some epidemiological features of dental caries.

We describe a random effects model for caries lesion development and progression based on considering the effects of the pH fluctuations over time in microbial dental plaque as a Wiener process with a single absorptive barrier. The model predicts that the period of greatest risk to developing caries occurs shortly after eruption, but thereafter the longer a surface survives without developing a lesion, the less likely will it be that a lesion will subsequently develop. The model is able to anticipate why the effect of water fluoridation on caries prevalence is most pronounced when caries is di

Utility of epidemiologic simulation models in the planning of trypanosomiasis control programs.

To understand the behavior of complex epidemiologic systems, it is useful to devise computer based simulation models by approximating the interactions via biomathematical expressions. Without doubt, these models could be over simplifications of complex interactions but they could be useful in comparison to classical laboratory and/or field experimental approaches which may not be practical or feasible. In the past obtaining solutions to biomathematical equations with any degree of complexity has been impossible. However, the availability of powerful computers has now made the quantitative anal

Integration of vitro models with machine learning and epidemiological data reveals PCB-induced glucose metabolism disruption linked to mitochondrial dysfunction.

Polychlorinated biphenyls (PCBs) have been reported to be associated with type 2 diabetes mellitus (T2DM); thus, the knowledge of their endocrine disruption mechanisms would be vital for assessing health risks. This study revealed the potential mechanism of abnormal glucose metabolism due to acute PCB-153 exposure in HepG2 cells through integrated transcriptome and DNA methylation analysis. Based on a joint analysis of two omics, the random forest machine learning model was established with 200 trees and cross-validated five times. Potential biomarkers identified by machine learning pointed to

Recent progress on wastewater-based epidemiology for COVID-19 surveillance: A systematic review of analytical procedures and epidemiological modeling.

On March 11, 2020, the World Health Organization declared the coronavirus disease 2019 (COVID-19), whose causative agent is the Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2), a pandemic. This virus is predominantly transmitted via respiratory droplets and shed via sputum, saliva, urine, and stool. Wastewater-based epidemiology (WBE) has been able to monitor the circulation of viral pathogens in the population. This tool demands both in-lab and computational work to be meaningful for, among other purposes, the prediction of outbreaks. In this context, we present a systematic revi

Epidemiology of Plague: Problems with the Use of Mathematical Epidemiological Models in Plague Research and the Question of Transmission by Human Fleas and Lice.

This article addresses the recent use of mathematical epidemiological SIR or SEIR models in plague research. This use of S(E)IR models is highly problematic, but the problems are not presented and considered. Serious problems show in that such models are used to "prove" that historical plague was a (1) Filoviridae disease and (2) a bacterial disease caused by Yersinia pestis which was transmitted by human fleas and lice. (3) They also support early-phase transmission (by fleas). They purportedly consistently disprove (4) the conventional view that plague is/was a rat-and-rat-flea-borne disease

The ovine cysticercosis as models for research into the epidemiology and control of the human and porcine cysticercosis Taenia solium: I. Epidemiological considerations.

A brief review is given of the transmission dynamics of the ovine cysticercoses Taenia hydatigena and T. ovis. The importance of the basic reproductive rate (Ro) and the parasite, host and socio-ecological factors determining the epidemiological steady states of Taeniidae are described. The transmission dynamics of these ovine cysticercoses have been used as models to assist in identifying the biological and epidemiological research needed for developing strategies for the control of porcine cysticercosis caused by T. solium.

Inter-model comparison of the landscape determinants of vector-borne disease: implications for epidemiological and entomological risk modeling.

Extrapolating landscape regression models for use in assessing vector-borne disease risk and other applications requires thoughtful evaluation of fundamental model choice issues. To examine implications of such choices, an analysis was conducted to explore the extent to which disparate landscape models agree in their epidemiological and entomological risk predictions when extrapolated to new regions. Agreement between six literature-drawn landscape models was examined by comparing predicted county-level distributions of either Lyme disease or Ixodes scapularis vector using Spearman ranked corr

Inferring epidemiological parameters under an infectious phylogeography model with visitor dynamics.

During an outbreak, infectious disease can spread among populations through host movement, potentially fueling local outbreaks with their own epidemiological dynamics. However, it is difficult to know how often infections between populations are transmitted by diseased travelers infecting healthy residents when abroad, rather than by diseased residents infecting healthy travelers, who later return home with the new pathogen. In this paper, we introduce a phylogeographic model where pathogens spread through visitor dynamics, whereby hosts visit other populations through short trips before retur

Coupled epidemiological and wastewater modeling at the urban scale: A case study for Munich.

Epidemiological modeling is critical to guide public health interventions, but model performance depends on data availability and quality. While clinical reports suffer from under-ascertainment and delays, wastewater-based surveillance (WBS) can rapidly capture community infection dynamics by detecting viral RNA from both symptomatic and asymptomatic cases. However, WBS data can be difficult to interpret. Here, we present a coupled model of infectious disease and wastewater dynamics designed for scalability to large cities. We calibrate the model to the first COVID-19 wave in Munich and quanti

Multiple Myeloma in Italy: An Epidemiological Model by Treatment Line and Refractoriness Status.

BACKGROUND: Multiple myeloma (MM) clinical management is challenging owing to its relapse and refractoriness to treatment. Understanding the treatment patterns and refractory dynamics is crucial for optimizing patient care. This study aimed to estimate the evolution of MM according to the treatment line and refractoriness status in Italy. MATERIALS AND METHODS: A new epidemiological model was developed using epidemiological and clinical data from literature. Prevalent MM patients were characterized by calibrating the model inputs. Incident patients were included starting in 2021, when antiCD3

Identification of an additive interaction using parameter regularization and model selection in epidemiology.

BACKGROUND: In epidemiology, indicators such as the relative excess risk due to interaction (RERI), attributable proportion (AP), and synergy index (S) are commonly used to assess additive interactions between two variables. However, the results of these indicators are sometimes inconsistent in real world applications and it may be difficult to draw conclusions from them. METHOD: Based on the relationship between the RERI, AP, and S, we propose a method with consistent results, which are achieved by constraining e θ 3 - e θ 1 - e θ 2 + 1 = 0 , and the interpretation of the results is si

Modeling the epidemiologic impact of age-targeted vaccination for drug-resistant tuberculosis.

This study used a calibrated mathematical model to evaluate age-specific tuberculosis (TB) vaccination strategies, for drug-resistant (DR)-TB management in China. Prioritizing elderly vaccination significantly reduced multidrug-resistant or rifampicin-resistant TB incidence and mortality, while avoiding the need for second-line treatment, offering a promising approach to mitigate DR-TB burden by 2050.
