Intra-operative monitoring of lower extremity motor-evoked potentials by direct cortical stimulation.

OBJECTIVE: Motor-evoked potentials (MEPs) are commonly recorded from upper-extremity muscles, whereas lower-extremity MEP (LE-MEP) monitoring has not been adequately established. The goal of the study was to develop a MEP monitoring method using direct cortical stimulation (DCS) for predicting motor deficits of lower extremities. METHODS: Intra-operative LE-MEP monitoring was performed in 22 patients. After craniotomy, a subdural electrode was placed on the cortex so that the optimal contact was positioned 2 cm lateral from the midline on the motor cortex. The electrodes for stimulation consi

Long-lasting depression of motor-evoked potentials to transcranial magnetic stimulation following exercise.

We used transcranial magnetic stimulation to study the modulation of motor cortex excitability after rapid repetitive movements. Eleven healthy subjects aged 24-32 years were evaluated. Serial motor-evoked potential (MEP) recordings were performed from the right thenar eminence every 5 min for a period of 20 min at rest and for a period of 35 min after repetitive abduction-adduction of the thumb at maximal frequency for 1 min. All subjects presented distinct changes in MEP amplitude after exercise with an approximately 55% mean maximal decrease compared with basal conditions and complete recov

Modification of cortical stimulation for motor evoked potentials under general anesthesia: technical description.

This pilot study presents a possible modification of direct cortical electrical stimulation technique for the recording of motor evoked potentials under general anesthesia. The exposed primary motor cortex was stimulated by a short train of anodal rectangular pulses at high frequency (300-500 Hz), while the compound muscle action potentials were recorded from the forearm and hand muscles. When compared with the traditional way of eliciting movement of the extremities by applying a train of pulses at lower frequency (50-60 Hz), muscle responses were obtainable at an intensity of much lower char

Sensory and motor trigeminal evoked potentials to localize the position of trigeminal electrodes.

Analysis of verbal responses to electrical stimulation of the trigeminal ganglion and rootlets has been the only method available so far to localize the electrode tip in the most appropriate trigeminal division or division segment, prior to thermocoagulation during percutaneous treatment for trigeminal neuralgia. A diversity of factors may lead to unreliable verbal responses, resulting in increased morbidity or even therapeutic failure. In an attempt to enhance the accuracy of electrode localization during Sweet's procedure, we describe an electrophysiological method complementary to clinical

Immediate plasticity in the motor pathways after spinal cord hemisection: implications for transcranial magnetic motor-evoked potentials.

The present study evaluates motor functional recovery after C2 spinal cord hemisection with or without contralateral brachial root transection, which causes a condition that is similar to the crossed phrenic phenomenon on rats. Descending motor pathways, including the reticulospinal extrapyramidal tract and corticospinal pyramidal tracts, were evaluated by transcranial magnetic motor-evoked potentials (mMEPs) and direct cortical electrical motor-evoked potentials (eMEP), respectively. All MEPs recorded from the left forelimb were abolished immediately after the left C2 hemisection. Left mMEPs

Facilitation of motor evoked potentials after repetitive voluntary hand movements depends on the type of motor activity.

Recent neurophysiological studies suggest that repetitive execution of identical movements is crucial for motor learning. During and after repetitive motor action, changes in motor cortical excitability have been demonstrated by means of transcranial magnetic stimulation. Nevertheless, the frequency and intensity of movement repetition that are necessary to achieve an optimal improvement in motor function are unknown. Fourteen healthy volunteers participated in the present study, which deals with the post-exercise facilitatory and/or inhibitory effects of 5 different motor conditions, includin

The study of recovery and modification of the evoked potentials and motor answers in the motor system.

After an electrical stimulus within the motor system there will follow a compulsion, a reflexlike process in functional dependency, naturally in illness (function disturbance) dependency as well. We can detect this process with the evoked potentials at the non-stimulated sites of the motor system and with the motor answers at the periphery. Because the motor system acts as a whole, it is certain that every process at a site has its correlates at the other sites, but the different part of the same process will be in a different grade pronounced (many times in opposite direction) at the differen

Effects of Lidocaine on Motor-Evoked Potentials and Somatosensory-Evoked Potentials in Patients Undergoing Intraspinal Tumour Resection: Study Protocol for a Prospective Randomized Controlled Trial.

PURPOSE: At present, it is believed that intravenous (IV) infusion of lidocaine can inhibit hyperalgesia, relieve postoperative acute and chronic pain, and accelerate the rehabilitation of patients. However, studies of its effects on necessary electrophysiological monitoring during neurosurgery are few, and the results are controversial. This study assumes that the propofol-remifentanil based anaesthesia combined with lidocaine in patients undergoing intraspinal tumour resection will not have adverse effects on motor-evoked potentials (MEPs) or somatosensory-evoked potentials (SEPs). False negative and positive motor evoked potentials in one patient: is single motor evoked potential monitoring reliable method? A case report and literature review.

STUDY DESIGN: A case report and literature review. OBJECTIVE: To report a false negative and delayed positive motor-evoked potential (MEP) in 1 patient. SUMMARY OF BACKGROUND DATA: An unreliable MEP can result in fatal outcomes because surgeons have recently begun to depend on the MEP for intraoperative decision-making. METHODS: We report a case of a false MEP during scoliosis surgery that showed false negative and positive MEPs during a series of operations. RESULTS: A 23-year-old man with a history of spondyloepiphyseal dysplasia presented with severe kyphoscoliosis. The initial neurolog

Spatiotemporal spinal integration of descending and spinal volleys in spinal motor circuits revealed by compound motor evoked potentials.

Descending corticospinal and afferent pathways underlying spinally evoked motor potential both contribute to motor output, yet how their interaction at the spinal and peripheral levels is organized spatially within a muscle remains unclear. This study investigated the spatiotemporal characteristics of descending modulation of spinally evoked motor potentials by combining subthreshold transcranial magnetic stimulation (TMS) with transcutaneous spinal cord stimulation (tSCS) delivered at the L1-L2 interspinous space in nine participants and at the T12-L1 interspinous space in one participant. Predictive value of motor-evoked potentials for motor recovery in patients with hemiparesis secondary to acute ischemic stroke.

BACKGROUND: Motor recovery following a stroke is related to the initial stroke severity and corticospinal tract integrity. One of the outcomes representing corticospinal tract integrity is the motor evoked potential (MEP). This study aimed to investigate the predictive value of MEP for motor recovery in patients with acute ischemic stroke. PATIENTS AND METHODS: Patients with hemiparesis secondary to initial acute ischemic stroke were enrolled. MEPs of the upper limb were assessed as preserved (MEP+) or absent (MEP-) response ≤10 days post-stroke. Fugl-Meyer assessment (FMA) was performed at b

A novel threshold criterion in transcranial motor evoked potentials during surgery for gliomas close to the motor pathway.

OBJECTIVE Warning criteria for monitoring of motor evoked potentials (MEP) after direct cortical stimulation during surgery for supratentorial tumors have been well described. However, little is known about the value of MEP after transcranial electrical stimulation (TES) in predicting postoperative motor deficit when monitoring threshold level. The authors aimed to evaluate the feasibility and value of this method in glioma surgery by using a new approach for interpreting changes in threshold level involving contra- and ipsilateral MEP. METHODS Between November 2013 and December 2014, 93 patie

Comparison of quadricep motor evoked potentials between different surgical positions during total hip arthroplasty.

The influence of intraoperative position on femoral nerve palsy after total hip arthroplasty (THA) remains unclear. Therefore, we evaluated the effect of intraoperative position on quadricep motor-evoked potential monitoring in patients undergoing THA using an anterolateral approach. We included patients who underwent primary THA using the anterolateral approach at our hospital between June 2021 and January 2024 with available data on intraoperative quadricep using transcranial electrical stimulation motor-evoked potential. Patient characteristics were compared between the supine and lateral p

Visualizing Intraoperative Transcranial Motor-Evoked Potentials During Glioma Surgery for Predicting Postoperative Paralysis Prognosis.

OBJECTIVE: The primary goals of glioma surgery are maximal tumor resection and preservation of brain function. Intraoperative motor-evoked potential (MEP) monitoring is commonly used to predict and minimize postoperative paralysis. However, studies on intraoperative MEP trends and postoperative paralysis are scarce. This study aimed to determine the relationship between intraoperative MEP trends and postoperative paralysis. METHODS: This retrospective study evaluated 229 patients with supratentorial glioma without preoperative paralysis who underwent tumor resection surgery under general anes
