Renal health and transplantation: a focus on ethnicity.

It is widely acknowledged within the United Kingdom that there are significant inequalities in renal health and transplant services--in relation to demand for, access to and waiting times for these services--between minority ethnic groups in particular. This phenomenon is not unique to the United Kingdom and affects many other countries that have a strong tradition of immigration. The solutions to reducing these inequalities are multi-faceted and require both short-term and long-term policy and resource-driven initiatives. In the short term, there is an urgent need to increase the number of or

Nim1-related kinases regulate septin organization and cytokinesis by modulating Hof1 at the cell division site.

The septin scaffold recruits and organizes actomyosin ring (AMR) components; thus, ensuring faithful cytokinesis. The septin-associated kinases - Elm1, Gin4, Hsl1, and Kcc4 are thought to stabilize and regulate the septin architecture at the bud neck, but the underlying mechanisms remain largely unknown. Here, we present a comprehensive, quantitative analysis of these four septin-associated kinases and reveal major roles for Elm1 and Gin4 in septin stability and architectural transitions during the cell cycle. We find that Elm1 and Gin4 play a previously overlooked role in AMR organization and

Caught between a rock and a hard place: mental health of migrant live-in caregivers in Canada.

BACKGROUND: Canada depends on Temporary Foreign Workers (TFWs), also known as migrant workers, to fill labour shortage in agriculture, hospitality, construction, child/senior care, and other low-skilled occupations. Evidence shows that TFWs, especially women live-in caregivers (LC), constitute a vulnerable population. Their health is compromised by the precarious and harsh working and living conditions they encounter. There is a paucity of research on the mental health of LCs, their support systems and access to mental health services. METHOD: In this community-based exploratory study, we use

Evidence-based treatment for opioid use disorder is widely unavailable and often discouraged by providers of residential substance use services in North Carolina.

INTRODUCTION: Opioid agonist treatment (OAT) is the only treatment for opioid use disorder (OUD) proven to reduce overdose mortality, yet access to this evidence-based treatment remains poor. The purpose of this cross-sectional audit study was to assess OAT availability at residential substance use services in North Carolina. METHODS: We conducted a state-wide inventory of residential substance use service providers in North Carolina and subsequently called all providers identified, posing as uninsured persons who use heroin, seeking treatment services. Program characteristics, as reported in

What the world's religions teach, applied to vaccines and immune globulins.

For millennia, humans have sought and found purpose, solace, values, understanding, and fellowship in religious practices. Buddhist nuns performed variolation against smallpox over 1000 years ago. Since Jenner developed vaccination against smallpox in 1796, some people have objected to and declined vaccination, citing various religious reasons. This paper reviews the scriptural, canonical basis for such interpretations, as well as passages that support immunization. Populous faith traditions are considered, including Hinduism, Buddhism, Jainism, Judaism, Christianity, and Islam. Subjects of co

The global registry: hope for the future.

In 2014, there is unanimous agreement that kidney transplant is the optimal treatment for most patients who have end-stage renal failure. Increasing organ shortage is the main obstacle that delays transplant and might even cause death while the patient is on the waiting list for kidney transplant. Many innovations have been proposed to increase the number of organs for transplant in different countries such as increasing awareness about organ donation, based on different cultures and religions. Support of religious and faith leaders exists for procurement of organs for transplant from patients

Explainable multiple abnormality classification of chest CT volumes.

Understanding model predictions is critical in healthcare, to facilitate rapid verification of model correctness and to guard against use of models that exploit confounding variables. We introduce the challenging new task of explainable multiple abnormality classification in volumetric medical images, in which a model must indicate the regions used to predict each abnormality. To solve this task, we propose a multiple instance learning convolutional neural network, AxialNet, that allows identification of top slices for each abnormality. Next we incorporate HiResCAM, an attention mechanism, to

A novel alternative method for long-term evaluation of male reproductive toxicity and its recovery using a pre-pubertal mouse testis organ culture system.

Successful treatment of pediatric cancers often results in long-term health complications, including potential effects on fertility. Therefore, assessing the male reproductive toxicity of anti-cancer drug treatments and the potential for recovery is of paramount importance. However, in vivo evaluations are time-intensive and require large numbers of animals. To overcome these constraints, we utilized an innovative organ culture system that supports long-term spermatogenesis by placing the testis tissue between a base agarose gel and a polydimethylsiloxane ceiling, effectively mirroring the in

Mental health problems among ethnic minorities during public crises: a systematic review of coping strategies during the COVID-19 pandemic.

Public health crises exert a disproportionate psychological burden on ethnic minority populations, exacerbating pre-existing health disparities. Drawing upon the COVID-19 pandemic as a focal case, this systematic review synthesizes the coping strategies employed by ethnic minority communities, organized through the lens of ecological systems theory. Adhering to PRISMA guidelines, 65 peer-reviewed studies published between 2020 and 2023 were analyzed. The synthesis identified a multi-tiered coping response: (1) individual-level cognitive-emotional regulation and faith-based practices; (2) micro

Limiting homologous recombination at stalled replication forks is essential for cell viability: DNA2 to the rescue.

The disease-associated nuclease-helicase DNA2 has been implicated in DNA end-resection during DNA double-strand break repair, Okazaki fragment processing, and the recovery of stalled DNA replication forks (RFs). Its role in Okazaki fragment processing has been proposed to explain why DNA2 is indispensable for cell survival across organisms. Unexpectedly, we found that DNA2 has an essential role in suppressing homologous recombination (HR)-dependent replication restart at stalled RFs. In the absence of DNA2-mediated RF recovery, excessive HR-restart of stalled RFs results in toxic levels of abo

Opportunities and challenges to integrating mental health into HIV programs in a low- and middle-income country: insights from the Nigeria implementation science Alliance.

BACKGROUND: In Nigeria, there is an estimated 1.9 million people living with HIV (PLHIV), 53% of whom utilize HIV care and services. With decreasing HIV-related deaths and increasing new infections, HIV with its associated comorbidities continue to be a key public health challenge in Nigeria. Untreated, comorbid mental disorders are a critical but potentially modifiable determinant of optimal HIV treatment outcomes. This study aimed to identify the challenges and opportunities related to integrating mental health care into existing HIV programs in Nigeria. METHOD: Attendees at the Nigeria Imp

Indications for red cell transfusions in pediatric patients.

Red cell transfusions are amongst the most common therapeutic procedures in seriously ill children, particularly in the inpatient setting. This is despite the fact that there is no evidence base for most clinical settings, with the exception of patients with hemoglobinopathies, particularly thalassemia and sickle cell anemia. Obviously exsanguinating hemorrhage and life threatening anemia are urgent indications for which no other therapeutic approach is currently available. Most transfusions are, however, given prophylactically to prevent the complications of hypoxia or hemodynamic stability,

[On the ontogenetic origin of human beings: the scientific solution].

Every living being is the result of a genome-environment interaction. Neither human oocytes nor spermatozoids have human functional genomes, but the zygote that they constitute may have a human functional genome and other functional genomes such as those of the hydatidiform mole, polyploids, and non-human living beings. When the zygotic human functional genome is integrated and activated, the biotic humanity is acquired. This may occur when the paternal chromatin decondenses; the nuclear environment and envelope of both nuclei are changed to constitute pronuclei; the replacement of sperm prota

Legacy at the Bedside: A Nurse-Led Evidence-Based Framework for Memory-Making in the Pediatric Intensive Care Unit.

Children who die in the pediatric intensive care unit (PICU) and their families often face sudden deterioration, limited time, invasive technology, and emotionally overwhelming decisions. Memory-making can help families preserve connection, support parenting and sibling participation, and create tangible or digital keepsakes, yet bedside teams may feel uncertain about when and how to offer these practices without adding distress. This evidence-based practice article synthesizes recent pediatric palliative care, bereavement, and critical care literature and translates it into a nurse-led bedsid

Single-Molecule Analysis of DNA Branch Migration under Biomimetic Environments.

Branch migration (BM) of DNA Holliday junctions (HJs) occurs through base-pair rearrangements between homologous DNA molecules during the repair of double-strand breaks (DSBs) by homologous recombination. Despite the fact that BM is conserved among organisms and is essential to stabilize recombination intermediates, which occurs by avoiding the reversal of strand exchange leading to the faithful repair of DSBs, molecular insights into the BM process including kinetics, the effects of microenvironments, and the role of HJ-binding proteins are poorly understood. In this article, using single-mol

High resolution, contrast-enhanced X-ray microscopy of the ex vivo human cochlea: technical feasibility and biometric case analysis.

BACKGROUND: The human cochlea is a fragile, spiral-shaped sensory organ for hearing that is deeply embedded within the dense otic capsule bone, making accurate post-mortem anatomical studies difficult when using conventional histological approaches requiring sectioning. X-ray microscopy (XRM) is a non-destructive radiological imaging modality that combines geometric and optical magnification to achieve high-resolution 3D visualization. METHODS: Here, we evaluated the ability of XRM to resolve the entire inner ear, including delineating soft tissue and bony structures, in trimmed human tempora

Cell-Free Filtrates (CFF) as Vectors of a Transmissible Pathologic Tissue Memory Code: A Hypothetical and Narrative Review.

Cellular memory is a controversial concept representing the ability of cells to "write and memorize" stressful experiences via epigenetic operators. The progressive course of chronic, non-communicable diseases such as type 2 diabetes mellitus, cancer, and arteriosclerosis, is likely driven through an abnormal epigenetic reprogramming, fostering the hypothesis of a cellular pathologic memory. Accordingly, cultured diabetic and cancer patient-derived cells recall behavioral traits as when in the donor's organism irrespective to culture time and conditions. Here, we analyze the data of studies co
