Effect of postoperative early ankle motion exercise using hinged ankle-foot orthoses in clubfoot.

The authors proposed early application of hinged ankle-foot orthoses for improving postoperative range of ankle motion after the complete subtalar release operation for clubfoot. Forty-eight patients (74 feet) with clubfoot were divided into two groups: group A patients (20 feet) were immobilized in plaster casts for 6 weeks after surgery, followed by non-hinged ankle-foot orthoses, and group B patients (54 feet) were immobilized in plaster casts for 4 weeks after surgery, followed by hinged ankle-foot orthoses. The range of ankle plantarflexion in group B showed a significant improvement comp

Effect of two contemporary tone inhibiting ankle foot orthoses on foot-loading patterns in adult hemiplegics: A small group study.

This study compared the effect of the dynamic ankle foot orthos is (DAFO) and the Utley foot orthosis (UFO) on foot loading patterns in five hemiplegic subjects. An A-B-B-A single subject design was used. Foot loading data were collected in three sessions, which were separated by a two-week wearing time for each orthosis. The DAFO produced significantly greater force and force-time integrals through the lateral forefoot, with a progression of weightbearing through the central forefoot at toe-off. This study failed to demonstrate significant foot loading pattern changes relevant to use of the U

Foot orthoses and gait: a systematic review and meta-analysis of literature pertaining to potential mechanisms.

This article systematically reviews the available literature to improve our understanding of the physiological basis for orthoses under the kinematic, shock attenuation and neuromotor control paradigms. The propositions made under these three paradigms have not been systematically reviewed collectively, and as such, there is no single-point synthesis of this clinically relevant body of evidence and somewhat disparate findings. Our comprehensive search strategy yielded 22 papers. Under each paradigm, the role of orthoses with different design features including combinations of posting, moulding

The effects of fixed and articulated ankle-foot orthoses on gait patterns in subjects with cerebral palsy.

Twenty-one subjects with spastic diplegic cerebral palsy were studied to quantify the effects of fixed and articulated ankle-foot orthoses (AFOs) on gait and delineate criteria for their use. Children underwent gait analysis under three conditions, fixed AFOs (FAFOs), articulated AFOs (AAFOs), and shoes alone. Greater dorsiflexion occurred at initial contact with both FAFOs and AAFOs than shoes alone. Dorsiflexion at terminal stance was greatest in AAFOs. Plantarflexor power generation at preswing was preserved in AAFOs. No differences were found in knee position during stance. Knee-extensor s

A novel design conception of foot orthoses promotes normal walking in patients with pronated feet.

BACKGROUND: Foot orthosis (FO) is one of the most important treatments for pressure relief and gait rehabilitation in the foot misalignment population. The previous design conception was primarily focused on arch support and pressure reduction, but recent studies also questioned their efficiency. METHODS: Three bionic fish bone FOs based on the COP characteristics of healthy people were designed and manufactured. The orthoses were tested in 30 volunteers through a series of experiments including both static standing and dynamic gait, and evaluated through finite element (FE) simulations with a

The Role of Postural Assessment, Therapeutic Exercise and Foot Orthoses in Haemophilic Arthropathy: A Pilot Study.

Haemophilic arthropathy is caused by repeated joint bleeding episodes, primarily affecting knees, ankles and elbows. Conservative options should be considered prior to surgery, as well as postural evaluation, since any functional overload promotes the development of new bleeding. The aim of this study is to verify the use of foot orthoses in combination with postural rehabilitation, assessing the incidence of spontaneous haemarthroses and haematomas. In total, 15 patients were enrolled and randomly divided into two groups: 8 in group A, composed of patients who were prescribed foot orthoses an

Effect of foot orthoses on balance among individuals with flatfoot: A systematic review and meta-analysis.

Individuals with flatfoot have impaired proprioception owing to ligament laxity and impaired tendons, which can result in poor balance. Foot orthoses (FOs) have been reported to stimulate plantar mechanical receptors and are used to manage foot overpronation in individuals with flatfoot. However, the results of the use of FOs to improve balance are inconsistent. In this systematic review and meta-analysis, we aimed to identify and investigate the effects of FOs on balance in individuals with flatfoot. Electronic databases were searched for articles published before March 2023. Peer-reviewed jo

Bilateral Foot Orthoses Elicit Changes in Gait Kinematics of Adolescents with Down Syndrome with Flatfoot.

Background: Subjects with Down Syndrome (DS) are characterized by specific physiological alterations, including musculoskeletal abnormalities. Flat Foot (FF), caused by hypotonia and ligament laxity, represents one of the most common disabling disorders in this population. Conservative treatments promote the use of orthopaedic insoles and plantar supports. The aim of this study was to evaluate the impact of Foot Orthoses (FOs) on the gait pattern of subjects with DS, assessing the biomechanical effects associated with their use. Methods: Twenty-nine subjects were screened under two conditions-

The effect of foot orthoses on joint moment asymmetry in male children with flexible flat feet.

INTRODUCTION: It has been widely postulated that structural and functional misalignments of the foot, such as flat foot, may cause mechanical deviations of the lower limb during walking. The aim of this study was to investigate the effect of foot orthoses on lower extremity joint moment asymmetry during the stance phase of walking in children with asymptomatic flexible flat feet. METHODS: Fourteen volunteer male children, clinically diagnosed with flexible flat feet, participated in this study. Subjects completed 12 walking trials at a self-selected walking speed while 3-dimensional kinematic

Examination of knee joint moments on the function of knee-ankle-foot orthoses during walking.

The goal of this study was to investigate clinically relevant biomechanical conditions relating to the setup and alignment of knee-ankle-foot orthoses and the influence of these conditions on knee extension moments and orthotic stance control during gait. Knee moments were collected using an instrumented gait laboratory and concurrently a load transducer embedded at the knee-ankle-foot orthosis knee joint of four individuals with poliomyelitis. We found that knee extension moments were not typically produced in late stance-phase of gait. Adding a dorsiflexion stop at the orthotic ankle signifi

Static and dynamic pedobarographic evaluation after UCBL orthoses in pediatric patients with foot arch disorder.

Pes planus is one of the most common foot deformities in children. Pedobarographic assessment provides objective information on plantar pressure distribution and may be useful for monitoring treatment-related changes. This prospective pretest-posttest study evaluated static and dynamic pedobarographic changes following University of California Biomechanics Laboratory (UCBL) orthosis use in 90 children aged 4-16 years with flexible pes planus. All participants used a UCBL orthosis for 3 months. Static and dynamic pedobarographic measurements were performed at baseline and after treatment. Regio

Ankle-foot orthoses that restrict dorsiflexion improve walking in polio survivors with calf muscle weakness.

In polio survivors with calf muscle weakness, dorsiflexion-restricting ankle-foot orthoses (DR-AFOs) aim to improve gait in order to reduce walking-related problems such as instability or increased energy cost. However, evidence on the efficacy of DR-AFOs in polio survivors is lacking. We investigated the effect of DR-AFOs on gait biomechanics, walking energy cost, speed, and perceived waking ability in this patient group. Sixteen polio survivors with calf muscle weakness underwent 3D-gait analyses to assess gait biomechanics when walking with a DR-AFOs and with shoes only. Ambulant registrati
